About the role
Fareham and Gosport Wellbeing service is looking to recruit a PCN Wellbeing Advisor, to be based in PCN surgeries in the Fareham and Gosport Localities.
In this role you will hold a caseload of patients offering assessments & 1-1 sessions to help manage mental health Wellbeing. You will attend meetings to enable patients to get the best service for them and will support people to access groups and activities within our wellbeing centre and the community. You will use your skills and experience to work directly with people with mental health issues one-to-one settings, include telephone, face-to-face and online engagement; this may on occasion include group facilitation.
30 hours per week (to include some evenings and weekends).
About you
You will need to have a good understanding of a mental health issue, as well as experience of working with people with mental health needs, excellent communication skills and a strong understanding of the key difficulties and challenges faced by people on their recovery journey. You will need good IT skills and be able to navigate different IT systems.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.
As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.
You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.
There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

About the role

About LMK
Good relationships shape our health and happiness, yet we are taught so little about them. LMK is a registered education charity on a mission to change that. We run workshops educating young people about healthy and unhealthy behaviours, so that they can avoid abuse and thrive in relationships.
Using film and guided discussions, our workshop leaders support children and young people in honest conversations about relationships and leave them better equipped to spot the early warning signs of abuse and inspire them to enjoy healthy, fulfilling relationships. We explore the 10 signs of healthy and unhealthy relationships and include practical lessons young people want to learn about (like consent) and practice scenarios in a safe, supportive and non-judgmental environment.
LMK is a learning organisation and prides itself in supporting leaders to develop their skills. We offer training and Continuous Professional Development (CPD) opportunities, as well as peer-to-peer learning and reflective practice sessions.
Job Description
LMK is looking for passionate and organised educators (LMK Leaders) to run community workshops, aimed at young people aged between 11 and 24. Through engaging, relatable, workshops you will help young people identify signs of healthy and unhealthy relationship behaviours, provide strategies for them to recognise the early warning signs of abuse and give them tools to keep themselves and their friends safe.
The minimum hours required per workshop is 3 hrs. This includes arriving 30 minutes before delivery, 1.5 to 2 hrs delivery and then remaining back 10 minutes and being accessible to young people who would like further information or a one-to-one conversation, surrounding the subject of relationships.
There is also 30 minutes admin, which is usually completed off-site, such as inputting student surveys and a Leader survey.
Responsibilities

Always adhere to LMK and partner safeguarding practices and procedures.

Respond to comms (emails/WhatsApp) sent out by LMK core team members, in a timely manner, to support planning and organising of sessions.

Meet with co-facilitators to carry out pre-session planning, minimum two working days before delivery.

Plan and deliver interactive LMK workshops in educational, sports and community settings in line with LMK delivery style and ethos.

Use your experience and knowledge of the issues affecting young people to tailor the workshops accordingly.

Ensure all youth participants and the adults in school/community organisations who attend a LMK workshop complete the post session feedback surveys.

Complete reflective post session Leader surveys to support practice and programme development, after each workshop.

Debrief with LMK’s Programme Manager and provide ongoing feedback about your sessions, through LMK’s reflective practice processes.

Undertake relevant training associated with LMK’s work including but not restricted to child protection, safeguarding, violence against women and girls (VAWG).

Continue to learn about and develop your skills on the issues affecting young people’s relationships including domestic violence, abuse, adverse childhood experiences, trauma informed practice, gang culture and grooming, mental health, sexual exploitation, consent, sharing of nudes /sexting, bullying in person and online, tech and online safety, pornography.

Provide feedback and co-design training and educational resources on issues affecting young people and their relationships, including opportunities to share your knowledge and expertise at peer-to-peer learning sessions.

Attend termly virtual Leader huddles to remain connected to peers and organisational values, mission, and purpose.

Contribute towards and keep up to date with LMK internal/external communications platforms, such as LMK Newsletters and social media.

Knowledge, Skills and Experience

Bank Support Workers have the ability to accept any shifts available on an ad-hoc basis across from all of Look Ahead's specialisms including Mental Health, Learning Disabilities, Homelessness and Young People. Bank Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices. Bank Support Workers have the ability to accept any shifts available on an ad-hoc basis across from all of Look Ahead's specialisms including Mental Health, Learning Disabilities, Homelessness and Young People. Bank Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices. Within Learning Disability services, this will include providing physical, domestic, emotional and social care. Within Mental Health and Homelessness services they will enable vulnerable people living in a variety of different tenures to manage their tenancies/ licences and move towards greater independence within the community by providing a flexible and individually tailored support package. While carrying out activities, Bank Support Workers should deal with all customers, visitors and staff in a professional and appropriate manner and report to Look Ahead staff and management any observations relating to customers' welfare.

What you'll do:
Working proactively with other members of the team to handle the service caseload and administrative responsibilities
Building supportive, trusting relationships with customers
Conducting key work sessions that are innovative and engaging in order to achieve Support Plan goals
Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
Proactively manage risk and safety both in and outside of their physical living environment

Headway East London is a charity supporting people affected by brain injury. Working across 13 London boroughs we offer specialist support and services for survivors and their family. We offer therapies, advocacy, family support and community support work alongside our day service: a community venue where people can make the most of their abilities and interests.

About the Role
We are looking for a Day Service Co-ordinator to join our art studio team and support people living with brain injury in our day centre in Haggerston.
You will take responsibility for organising and delivering activities in the Submit to Love art studio, such as supporting members in creating new artworks, planning and delivering events and outings, and engaging members in a variety of other activities in response to their needs and interests.
The role also involves providing advocacy and support (key working) for up to 12 members and ensuring the smooth running of the day service as a member of the co-ordinating team.
Principal Duties and Responsibilities
Activities in the art studio:
- Support members to engage creatively and develop their personal artistic practise.
- Work alongside the studio manager, day service members and volunteers to plan and deliver events, including artist led workshops, exhibitions and open studios.
- Plan trips to cultural events and experiences.
Key working:
- Act as key worker for up to 16 members, checking in regularly about their engagement in the centre and their general wellbeing, and supporting them with information, advice, and advocacy as appropriate.
- Establish and maintain contact with members’ families, carers, and relevant statutory partners to facilitate good communication and better provide appropriate support.
- Maintain accurate and up-to-date records, including comprehensive individual risk assessments.
Day Service co-ordination:
- Work closely with colleagues and volunteers in the day service and other teams at Headway East London to create a welcoming, supportive, and stimulating environment for members.
- Share responsibility for practical tasks in the day service to ensure its smooth running.
- Work sensitively with volunteers, supporting them alongside the volunteer co-ordinator to take on meaningful and rewarding roles in the day service.

About the job
We are seeking a confident and experienced Modern Slavery Case Worker Assistant with a wide range of skills and importantly, a ‘can do’ approach. As a Modern Slavery Case Worker Assistant, you will provide high quality practical and emotional support to potential victims of modern slavery within a safehouse setting.
You will work as a member of the team providing a safe temporary environment for vulnerable adults who have experienced trauma and exploitation. The role will involve assisting service users with increasing practical skills and self-confidence to empower and enable independent and safe living.
This service is open 24 hours a day, seven days a week. Applicants must be willing to work flexibly, which will include on call duties, evenings, weekends and bank. Both roles will work between two properties approx. 4 miles apart on a Rota basis. A full induction and continuous training will be given.
The 30 hour per week post will be shifts from 4pm-10pm, 5 days per week, however, the initial induction will consist of day shifts until knowledge of safe house is comprehensive.
You must have experience of working with vulnerable adults, the ability to work within a ‘risk associated’ environment and ideally experience of working within a residential or community setting with non-UK citizens. Relevant social care qualifications or suitable experience of supporting vulnerable people is essential.
