The Peer Support In-Reach Service is a partnership between several local Minds. Senior Peer Support Workers and Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards living independently in the community.
We are seeking a Peer Support Worker who has personal lived experience of mental health challenges. This individual will act as a role model for service users, utilizing their own recovery journey to instil hope and resilience. By sharing their life experiences and the lessons learned throughout their recovery, they will motivate others.
The Peer Support Worker will collaborate with the multidisciplinary team within the Unit, assisting in-patients in developing essential skills and coping mechanisms, while promoting overall wellness through strength based and non-directive conversations. They will provide both practical and emotional support, facilitate access to community resources to help individuals maintain their independence while ensuring their wellbeing.
Additionally, this role includes supporting service users after discharge in the community for up to 12 weeks, aiding them in fostering independence within their local community, connecting with various agencies, enhancing life skills, providing guidance related to employment or educational opportunities and improving their ability to manage challenges effectively to prevent re-admission.
This position requires building connections with service users through shared experiences of mental health challenges, making lived experience a crucial requirement for applicants.

ECHO’s Outreach Coordinator will provide support to families of children and young people aged 0-25 affected by CHD (Congenital Heart Disease) and patients.
This role will have around 20% of time to focus on finding out the needs of 18-25 patients, parents, and siblings, and developing support and information for them.
We are an independent charity who support families treated by the Evelina, London & Royal Brompton, and Harefield hospital networks, and the CHD network of 47 local hospitals, and are based close to the Evelina London Children’s Hosptial.
Can you… work in a hospital environment, on the phone, and online; plan visits or events such as a zoo trip or a party; support children, young people, and families through diagnosis, treatment, care, and, at times, the loss of a child; and provide needs-led emotional, social, and practical support to children with heart conditions, their siblings, parents, and carers?
Can you also research the needs of 18–25-year-olds, develop peer support and programmes for them, and deliver this on a small budget with patient and carer input throughout?
This role provides high-quality emotional, social, and practical support to children with heart conditions, their siblings, parents, and carers, from birth to 25 years. You will deliver 1-to-1 outreach in the hospital and the community, lead play and wellbeing activities, and maintain a visiting programme across children’s departments, including PICU, Cardiac, and NICU. 20% of your focus is supporting young people aged 18–25, identifying their needs, assessing numbers, and developing peer support programmes. The role also involves creating resources and accurate information for families, offering guidance for those facing bereavement or loss, promoting wellbeing across hospitals and networks, planning events, contributing to closed social media groups, maintaining records, and capturing impact through case studies and testimonials.

- Research the support needs of younger adults, from 18 – 25 years across the CHD community, including patients, siblings/young carers, and parents/carers.
- Ensure EDI is a primary focus for ECHO – help us ensure everyone can access our services
- Be there from diagnosis throughout childhood – work with our Youth Worker & Families and Volunteer Coordinator to ensure our families are supported
- Create ways to utilise the ECHO database to understand our own membershipsProvide information and support to families of children with heart conditions and those with additional needs
- Review and upgrade information for families, build strong relationships with referrers by attending meetings, providing talks and outreach across the network hospitals, delivering paediatric Cardiology services via Evelina London and the Royal Brompton
- Seek to offer support services to families treated locally in network hospitals – Annual mail out & visit
- Represent families throughout the community and consider the varied needs of ECHO members and how they might be included in our service offering
- Research what do young parents under 25 need, what about the needs of patients who are 18-25
- Provide fun sessions to children, young people and families

The Role
This service focuses on improving the wellbeing, confidence and engagement of fathers and male caregivers, particularly during pregnancy, early parenthood and times of family stress or transition.
This role will focus on engaging fathers who may be less likely to access traditional services, including through outreach, hospital-based engagement, family hub delivery, one-to-one support and home visiting. About You
We are looking for someone who understands the importance of supporting fathers and male caregivers, and who can build trust with people who may be unsure about asking for help. You will be confident working with families, able to manage a caseload, and comfortable delivering support in a range of settings including hospitals, family hubs, homes and community spaces.
You will need to be flexible, relational and practical, with the ability to work some evenings or weekends to reach fathers at the right time.
You will bring:
· Experience of working with families, fathers, parents or caregivers

About the Role:
- Provide unwavering support to service users from their initial contact with our service throughout their treatment and recovery journey.
- Implement tailored interventions aimed at reducing the harm caused by substance use, either through group sessions or one-on-one engagement.
- Ensure service users receive a comprehensive, recovery-focused detoxification plan, including pre-detox preparation and post-care support.
- Collaborate with other organisations and the community to enhance the recovery resources available to service users by referring them to mutual aid groups and other networks.

About you:

Allsorts is seeking an enthusiastic, dedicated and dynamic Youth Worker to help us deliver our vision of a world where LGBTQ+ children and young people are free to be themselves. The Youth Worker will primarily deliver high-quality, inclusive and responsive youth groups and short programmes that empower LGBTQ+ young people to thrive. This includes planning, facilitating and evaluating sessions alongside Youth Group Workers and working collaboratively with local youth organisations to be present in their youth spaces. The role will also have a focus on supporting trans and non binary young people through dedicated programmes such as Evolve and Waiting Well.
Working collaboratively with colleagues, volunteers and partner organisations, the Youth Worker will ensure that young people have access to safe spaces, meaningful activities and holistic support that build confidence, wellbeing and life skills. They will champion youth participation throughout service delivery, maintain high standards of safeguarding and record keeping, and contribute to ongoing monitoring, evaluation and impact reporting

About You:
You have experience working with children and young people in a youth or similar setting. You bring strong energy, enthusiasm and creativity to frontline youth provision. We offer groups, one-to-one support, activities, information sessions, and much more for parents & carers of LGBTQ+ children and young people across Sussex.
Training and Consultancy Service
We provide LGBTQ+ Inclusion Training for a wide range of organisations, with a focus on organisations working with children and young people. Our goal is to help build people’s knowledge of LGBTQ+ issues and their confidence in being effective allies to LGBTQ+ children, young people and adults.

Our Youth and Play Practitioners will play a pivotal role in the Children, Young People and Families (CYPF) team, working as a core team of practitioners to ensure the smooth running of our programmes and services. In this role you will be part of a small and dynamic team which delivers a wide range of programmes and activities, both from Tulse Hill Adventure Playground and in conjunction with local schools. The work of the Children, Young People and Families team is diverse and varied. Our current services include open access adventure play and youth activities at our Adventure Playground, school holiday programmes with off-site trips and activities, mentoring, a Young Leaders programme offering paid work experience to young people, and working together with our youth partnerships Building Young Brixton and Lambeth Peer Action Collective.

To support our range of children and young people, our team will be made up of specialist Youth Workers and Play Practitioners bringing in relevant skills and experience. Whilst you will work across all of our Children, Young People and Families services you will have a specific focus:
As a Youth Worker you will:
- Focus on supporting our secondary age cohort
- Plan and run appropriate and engaging activities
- Provide structured interventions to support vulnerable young people
- Work with referral partners to provide additional support and activities
- Foster relationships with families and local schools
As a Play Worker you will:
- Focus on our primary age cohort
- Use your knowledge of Playwork Principles to plan and run appropriate and engaging
activities - Foster relationships with families and primary local schools
Both roles will include an element of mentoring, relevant training will be provided to give you the necessary skills to deliver this.
To be successful in this role, you will act as a trusted practitioner in all our service delivery, advocating for children’s right to play throughout our programmes. You will work as part of the team on the delivery and planning of all sessions, ensuring that the children’s and young people’s ideas are central to the construction of a varied and engaging play environment. You will understand the wide-ranging challenges facing young people and will be flexible in adjusting your practice to meet these needs. While practitioners may have a primary focus (primary or secondary age), all staff are expected to work flexibly across all age groups, programmes and delivery model including open access play, targeted youth work, mentoring and school link sessions.

Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accommodation for 18 women who have experienced homelessness and multiple disadvantage. The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation.
In this role, you will provide support to all the women, including running co-designed activities and accompanying women to appointments. You will work closely with Specialist Support Workers who manage individual resident support and safety plans. You will model person-led, psychological, gender, and culturally informed support, will be skilled at building trust and you will be a great team player.
The post holder will undertake 8-hour shifts on a 7-day rota, covering the hours 07:45 am to 20:15 pm. Please be aware that the building has several floors and no lifts.

Community Outreach Worker – Physical Health

About the Service
The Community Outreach Worker plays a vital role in delivering the Physical Health Checks programme for individuals with severe mental illness (SMI). This position focuses on engaging hard-to-reach individuals, facilitating health checks in healthcare settings, community settings, and supporting follow-up care to enhance physical health outcomes. The post holder will collaborate with health professionals and stakeholders to reduce health inequalities and promote recovery
About the Role
The role ensures that individuals with severe mental illness receive timely and effective physical health checks, overcoming barriers to engagement and supporting continuity of care. The post holder will provide proactive outreach to improve health outcomes, promote engagement with primary care and support individuals to access and sustain appropriate health and community-based support.
The post holder will manage their workload independently, maintain accurate documentation, and ensure high standards of service delivery. They will also contribute to monitoring and reporting to support the success and sustainability of the Physical Health Checks programme.
Key Responsibilities
- Support the delivery of the Physical Health Checks programme
- Target and engage individuals on GP mental health registers who are difficult to reach
- Deliver physical health checks in healthcare and community settings, supported accommodation, day centres, and through home visits
- Follow up on health check results and liaise with GPs and social prescribers for onward referrals
- Support clients to access follow-up appointments and ongoing physical health treatment
- Provide one-to-one support tailored to individual needs (in person, via telephone, or email)
- Signpost and assist access to relevant health and wellbeing services
- Facilitate effective liaison between families and professionals, including GPs
- Hold a caseload of clients with SMI, delivering support through home visits and community-based interventions
- Monitor and review the safety and wellbeing of clients on a regular basis
- Maintain accurate documentation including participant notes and data required for monitoring and reporting
- Actively promote the service through GP practice meetings, events, and outreach activities
- Build and maintain effective relationships with health care professionals and community partners to support referrals
- Collect feedback from participants to improve service quality
- Encourage and support clients to access volunteering opportunities that build skills, confidence, and community connection
- Embed a person centred and solution focused approach in all aspects of the role
- To remain a source of independent support for all clients
