About the Role

• Community Resettlement Workers support clients who have moved in to independent accommodation, usually from the street or hostels. The accommodation is across London and your role is to work alongside individuals as they transition towards independence.
• By adopting a flexible, creative and personalised approach you will support individuals to manage their tenancies in the community and improve their quality of life.
• This service is a stepping stone towards independence and you will continue to work with these clients for up to two years to prevent them from returning to the street, to support them to build networks of support in their local community and to help them to move on into permanent accommodation.
• This role is offered as a fixed term contract until 31st March 2026.
• This role will require travel to various London boroughs.

We are looking to recruit an empathetic Palliative Care Social Worker to join our team of dedicated Social Workers at Birmingham Hospice.

Our Palliative Care Social Workers at Birmingham Hospice specialise in working in partnership with patients who require specialist palliative or end-of-life care in addition to supporting their families, those they are close to and their communities. Using their specific skills and knowledge to help people to deal with the impact of a palliative diagnosis, including emotional support, to have a good life and a good death.

In this role you will be responsible for ensuring the provision of a coordinated family support service across both Hospice sites. Working as part of a multi-disciplinary team (MDT), you will support the senior palliative care social worker to ensure that the service provides high standards of support, to patients, their families, carers, loved ones and communities,  You will help to influence and enhance best practice in palliative and end-of-life care involving both internal and external professional providers and agencies.  You will be well-versed in Safeguarding, Deprivation of Liberty Safeguards (DoLS), have a thorough understanding of the Mental Capacity Act, and other relevant legislative frameworks.

As part of the social team, you will help to provide the assessments and applications for both Social and Fast Track CHC packages of Care to ensure that the highest standards of care and support to our patients and loved ones are provided at the right time.

You will be a registered social worker with Social Work England and hold a full clean driving licence.  You will be a sound communicator and will be authentic, open, honest, transparent, and have a track record of working inclusively with a genuine appreciation of the value of diversity.

About Shooting Star Children’s Hospices

We have an exciting opportunity for a family support to join our Family Support Team at Shooting Star Children’s Hospices.

Shooting Star Children’s Hospices provides specialist care and support to families who have a baby, child or young person with a life-limiting condition, or whose child has died. Rated ‘Outstanding’ by the Care Quality Commission, we support families across Surrey, north-west London and south-west London from diagnosis to end of life and throughout bereavement with a range of nursing, practical, emotional and medical care.

About the role

Family Support Workers (FSW) are the frontline of our psychosocial care. Each FSW holds a caseload of end-of-life and bereaved families. They get to know their families, and check-in regularly with phone calls and home visits and can provide enhanced emotional support where required through more regular contact and interventions. Our FSWs inform families about the different psychosocial care and many services they can access at Shooting Star, assess their needs and make recommendations to our specialist psychosocial MDT. They work hard to support families of different ethnicities and families living in poverty, so that our service is as equitable as possible.

Our FSWs work closely with our Therapists, Counsellors, Social Workers, Transition Team and our Care Events Team. Weekly specialist psychosocial MDT meetings bring the team together to think of ways to help families in crisis. The work is varied and creative, taking place at the hospices, in family homes and hospitals, allowing the support to be where the families need it. Our Family Support Workers are hugely appreciated by the families we support.

The ambition of our strategy is to ensure every family affected by a life-limiting condition, or the sudden death of a child, has access to the specialist care that they need. Day to day duties are:

• Being part of the duty rota over both sites monitoring 3 email inboxes and answering the Family Support Line.
• Supporting the events team with admin tasks and attending various events
• Providing on the spot emotional support, sometimes in a crisis where a calm and empathic response is required.
• Support the in-house team with families staying at Christopher’s for assessed nights and end of life care.
• Attend various sites over the course of a week, hospice, hospital, family homes. Planning your own diary effectively.
• Building connections with families from different backgrounds, delivering culturally competent care.
• Acting as an ambassador for Shooting Star’s Specialist Bereavement Service, supporting other care team members with their learning and delivery of the bereavement pathway, and continually seeking ways to improve quality.

About you

This role requires experience of working with children and families who have experienced hardship, presenting with complex needs. You should have a calm positive manner, be consistent in your approach, and convey empathy whilst maintaining professional boundaries.

Role in Context

Jigsaw4u’s Carers Peer Support Service supports adult carers in the London Borough of Merton through emotional support and access to information, opportunities and practical advice.

Working alongside other VCSE peer support providers in the Borough, other carer organisations or providers of statutory carers work, the post holder will work on improving pathways and coordinating services for adult carers experiencing mental health issues, often as a direct result of caring.  The post holder should have lived experience of being a carer, or of mental health challenges experienced by themselves or a close friend or family member

1. Purpose of the job

Be responsible in the designated area for:

• Delivery of one-to-one and group peer support sessions designed for and by adult carers
• Collecting data and reporting on direct work delivered with adult carers
• Collaborative working with the Merton Peer Support Partnership
• Developing and enhancing relationships with strategic partners
• Maintaining and striving to improve service delivery standards and effectiveness

1. Main duties

• Providing emotional and wellbeing support for carers through one-to-one, person-centred interventions
• Working flexibility to support carers aged 18 and over, who are caring for someone with mental health difficulties or experiencing mental health issues, often as a direct result of caring
• Empower and support carers to become involved in local Mental Health developments, particularly within SWLSTG
• Encourage and assist the uptake of Carers Assessments and the ability of carers and their families to assess their own needs to develop solutions and manage resources
• Assist carers in accessing opportunities for breaks from caring through activities within the partner organisations and external agencies
• Delivering time-limited, outcome-focused interventions to support personal outcomes so carers feel emotionally and practically supported
• Group-based support to address intended specific outcomes
• Encouraging access to local services to promote community inclusion and connectedness, enabling sustainable recovery and support
• Practical support to help carers access the right services and support based on needs, preferences, and the options available

Family Support Foster Carer

Overview

The family support foster carers (FSFCs) mentor their families and offer short breaks for children, offering stability and giving parents time to address their challenges with support. They provide feedback on their involvement with the families during review meetings.

The FSFCs could be experienced foster carers within the service who want to develop their skills and explore a new opportunity within fostering. Alternatively, they can be recruited specifically to this role in SUSD but must gain approval prior to commencing in the role. Is it imperative that the FSFCs have the skills and commitment to working with both parents and children as part of the SUSD programme. A role description for the FSFC is included in this pack.

Role and Responsibilities

The key aspects of the role are to

• provide weekly mentoring support for parents who are referred to SUSD. There is maximum caseload of 4 families at any one time.
• support the family to set and review goals, using our Person Centred Planning app.
• provide one planned short break each month for each referred child.
• offer crisis response support to families in the form of phone calls, visits or crisis short breaks.
• attend training provided by The Fostering Network & the Local Authority, enabling the FSFC to provide training and support to families.
• help with family activities, such as outings, family activities, summer schemes and residentials

The Family Support Foster Carer will be required to record details of the support provided, sharing with the supervising social worker as appropriate. During weeks that the FSFC has attend a review or support meeting for a family, they will not be required to complete another mentoring session with that family.

Working with parents and families

Families are referred to Step Up Step Down by their social worker. The scheme is voluntary, and families are empowered to decide whether they want the support. SUSD support has 4 primary aspects:

• mentoring for parents
• short breaks for children
• family activities
• community supports.

SUSD Mentoring Sessions are based upon areas of identified need. These are decided through the referral process and the initial planning meeting and are reviewed every three months throughout the duration of SUSD support. The key areas of support are: Routines, Budgeting, Healthy Living, Mental and Physical Wellbeing, Behaviour Management, Access to supports, Family Relationships, Dealing with Crisis Situations, Play and Stimulation.

The FSFC will also be required to give updates and share progress at review meetings. Information required will be:

• number of parent mentoring sessions since the previous SUSD review (including cancelations), and details of the work covered with the parent;
• number of short breaks since the previous review, and details of activities;
• any and all other support given to the family.

Support for the Family Support Foster Carer

Personal development and self-care is important, as we know that the best supporters are well supported themselves. There are several ways that SUSD will support the FSFCs.

• There will be regular supervision where they can discuss complex cases or raise any issues.
• FSFCs will be encouraged to attend training that will support them in the role, and to take an active role in identifying what this training should be.
• Quarterly carer support sessions are organised to enable the FSFCs & other SUSD team members to meet and learn from each other.
• FSFCs are also encouraged to form informal peer support networks.

Service: Children's Services

Salary: To receive a minimum allowance of £19,700, up to a maximum of £32,700 per annum

Please note, this is a self employed role and you will receive a weekly allowance. As a registered foster carer you will qualify for Qualifiying Care Relief and can earn up to £18,000 before paying tax.

Our Youth and Play Workers will play a pivotal role in the Children, Young People and Families team, working as a core team of practitioners to ensure the smooth running of our programmes and services. In this role you will be part of a small and dynamic team which delivers a wide range of programmes and activities, both from Tulse Hill Adventure Playground and within local schools. The work of the Children, Young People and Families team is diverse and varied. About the Role

• Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accommodation for 15 women who have experienced homelessness and multiple disadvantage.  
• The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation. The accommodation also includes overnight safe spaces.
• In this role, you will provide support to all the women, including running co-designed activities and accompanying women to appointments.
• You will work closely with Specialist Support Workers who manage individual resident support and safety plans.
• You will model person-led, psychological, gender, and culturally informed support, will be skilled at building trust and you will be a great team player.
• You will undertake 8-hour shifts on a 7-day rota, covering the hours 07:45 am to 20:15 pm. Please be aware that the building has several floors and no lifts.

Family Support Worker

We are looking for a Family Support Worker to act as a keyworker delivering packages of support with children, young people, and whole families to prevent escalation to more intensive services.

Position: Family Support Worker (5 positions available)

Location: Harborne/Community Based (the families you will be supporting will be based in Edgbaston & Northfield. This is an outreach position and you will be expected to be based in the community)

The Role

Through undertaking an Early Help Assessment and by working with families in a person-centered way, you will help people on your caseload to make positive changes and to develop the skills to manage their issues in the future.

You will work within the Early Help Locality multi-disciplinary team to provide a timely and front-line response to the needs of children, young people, and their families, acting in accordance with local policies, procedures and priorities. 

Key duties include:

• Work in partnership with other practitioner/professionals, children, young people, and their family. 

• Work within a multi-disciplinary and professional integrated Early Help service to provide a timely response to the needs of children, young people and their families.

• Deliver evidenced based interventions.

• Provide a timely response to the identified needs of children and young people. 

• Promote and use an Early Help Assessment framework.

• Be responsible for a high standard of case recording. 

• Engage with children, young people and their families and actively promote their participation in assessments and support plans.

• Manage and prioritise an allocated caseload.

• Organise and participate in planning programme delivery of evidenced based programmes.

Reporting to the Kentown Project Lead out of our Lancaster team base and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.
