In this role, you will manage an existing caseload and new referrals, meeting with families and Young Carers to complete statutory assessments and create action plans. You‘ll provide support for Young Carers to reduce the impact of their caring role and improve their resilience, educational chances and wellbeing, through signposting to local services, in-school support, one-to-one sessions, and workshops.

There will be opportunities to work closely with schools, Children’s Services and other agencies to raise awareness, identify hidden Young Carers, and ensure Young Carer needs are understood and acted on.

You will need to be confident, approachable and self-motivated, with the ability to quickly establish rapport with children and young people. Applicants should have relevant experience of working or volunteering in education, health or social care, or be looking to start a career working with children or young people.

This is a full-time role (including occasional early evenings and Saturdays).

Purpose of the Job

Physical health and mental health are inextricably linked. The purpose of this role is to support people who have significant mental health issues and those from diverse ethnic backgrounds who may face barriers in accessing physical health checks. The role will provide longer term support in some cases, so people can address some of the barriers which might be in the way for them to consider their physical health positively. The roles will provide a service across Hammersmith and Fulham and Hounslow.

The role will be delivered within Primary Care and therefore you will be expected to build relationships within those settings and with community VCSE organisations to ensure that the service supports people within their own communities.

Key Responsibilities

• Get to know and build relationships with staff within Primary Care to enable a supportive and productive referral route into and out of the service.
• Work within communities and Primary Care to find out what services are available for people to access.
• Build relationships with staff in Primary Care, VCSE organisations, local communities that don’t access services and local community health teams.
• Build relationships with those who are accessing the service and work in a person-centred way to find out their needs. Particularly working to engage people from diverse ethnic backgrounds.
• Build an accessible route into the service for people which will lead to an increase in physical health checks and longer-term support if needed.
• Be inclusive and make reasonable adjustments where necessary, i.e., interpreters, Easy Read literature, disability accessible buildings for meetings, etc.
• Conduct basic physical health checks following robust training from health professionals.
• Support people in connecting to other services both VCSE and Primary Care.
• Alert Primary Care staff and the local authority/mental health teams if there are any concerns that have not already been identified.
• Signpost and refer clients to other services during and after the intervention.
• Promote health education and the service across the borough.
• Input monitoring information to the Service Manager.

• Experience of conducting physical health checks.
• Experience or working with those who have serious mental health issues.
• Experience of developing kind relationships to create a sense of belonging.
• Knowledge of health literacy, i.e., weight management, diabetes, blood test, smoking, etc.
• Experience of motivational interviewing.
• Experience of supporting people in a way which recognises and develops potential.
• Experience of involving people in the development of services and support.
• Good understanding / awareness of the barriers people with mental health issues face in terms of achieving good or improved physical health.
• Ability to promote health engagement.

The team work holistically with Carers to understand their individual circumstances and level of need. You’ll carry out home visits and community-based meetings, co-produce support plans, and develop effective relationships with Carers, families, and professionals. You will also support Carers to complete written forms and applications, draft letters, and advocate with third parties.

You’ll build on your local knowledge to signpost and facilitate access for Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. The role also requires you to support the development of escalation plans so that Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Together with colleagues, you will plan and deliver the service’s activity package, including coffee mornings, drop-in sessions, activities, online and face to face groups, and peer support forums. Hector's House was founded in 2016 by the family and friends of Hector Stringer, who died by suicide aged 18.

Today, we are a trusted local charity dedicated to preventing suicide and supporting people affected by suicide across Hertfordshire, Bedfordshire and Buckinghamshire

The hours are 9am - 5.18pm Monday, Tuesday, Wednesday, Friday and 12pm - 8.18pm on Thursday. 

What do I need to bring with me?

You'll need to be able to demonstrate the core skills this role requires as well as match our values and mission. We also value lived experience of the areas we support, so if you feel comfortable, please do mention this on your application.

You have experience supporting people with mental health needs and complex circumstances, as well as working with older adults, senior adults or other vulnerable groups. You understand how to identify and report safeguarding concerns, recognise signs and symptoms of mental health issues, and respond appropriately within professional boundaries. You are confident facilitating learning and development activities, group work or support groups, and you are able to engage with individuals who may display a range of emotions or behaviours that can sometimes feel challenging.

You communicate clearly and professionally, both verbally and in writing, and you can accurately receive, process and record detailed information while maintaining confidentiality. You are IT‑literate, comfortable using Microsoft Office and able to learn new systems or apps. You bring strong time‑management skills, attention to detail, and the ability to work both independently and as part of a team. You liaise confidently with colleagues and external partners, represent Hestia positively, take a solution‑focused approach to problem‑solving, and can identify and report any health and safety concerns within a day‑centre environment.

We recognise the value of social media in communicating with all our audiences; whether unpaid carers or their friends and families, employers, politicians, service providers, supporters and all those people who may become a carer for a partner, family member, friend or neighbour and will need information, support, and to know that there are people working to make their lives better.

This role works with colleagues across the organisation - but also in partnership with external organisations and agencies, with targets set for engagement (and other indicators of the success of our campaigns). We're working on multiple campaigns simultaneously; whether that's Carers Week, campaigns to influence changes to the law to better support carers, encouraging unpaid carers to join us as members, updating on activities in the devolved nations or getting people to fundraise for Carers UK. And we use a mix of owned, earned and paid communications to drive engagement in a challenging and crowded environment in order to get our messages to cut through. It's a busy, but varied role, in a supportive and collaborative team.

About you

Working within the communications and marketing team, you'll be confident in managing our social media accounts (we have many), ensuring we're using platforms to get our messages out to both wide and targeted audiences with engaging and timely content.

You'll be happy using tools for social listening, interacting with our audiences and building our reach. You'll be familiar with key metrics for social engagement and will be reporting back to colleagues and other stakeholders - and using this data to constantly improve our communications.

You'll be skilled in writing longer form content for our members' magazine, Caring, proof reading articles, posting content on our websites, creating graphics and making sure our content is engaging, audience focused and dynamic. Oh, and you probably know your way around an emoji set and are not scared of a well timed pun. We're a small but effective communications team and pride ourselves on getting stuck in and supporting each other when we need to deliver big on fast moving projects, so you'll need be able to juggle lots of demands while recognising the importance of always hitting deadlines.

About Black Thrive Global

We exist to change the odds stacked against Black people by embedding race equity into systemic change so that thriving is not the exception but the norm.

Black Thrive Global evolved from the work of the Black Thrive Lambeth Partnership, which was established in 2016 to address the inequalities that negatively impact the mental health and wellbeing of Black people in Lambeth. Black people’s cumulative exposure to negative experiences and poor outcomes are not unique to Lambeth and Black Thrive Global was founded in 2020 to address the detrimental outcomes for Black people of African and Caribbean descent wherever they may be located.

Our work covers all life stages – children and young people, working age adults and older adults. Our guiding principle is to centre the lived experiences of Black communities in all that we do, and our strategic priorities are to embed race equity in systems change and to decolonise the evidence landscape.

The Thriving Futures Scaling Systems Change programme will involve scaling what we have tested and learned locally in Lambeth, Haringey, and Birmingham to achieve UK-wide impact through Black Thrive Global, with the ambition to extend our work into Wales and Scotland.

Role Description

The Leadership Academy Coordinator will play a key role in the planning, coordination, and delivery of the Leadership Academy, a programme designed to equip participants with the skills, confidence, and knowledge to engage with systems, drive change, and develop as inclusive, multi-skilled leaders, with a strong emphasis on valuing lived experience.

The Academy will bring together learning and development opportunities across a range of areas, including the Patient and Carer Race Equality Framework (PCREF), system navigation, community-led research, and leadership development approaches. Through this programme, grassroots community groups, people with lived experience, and system leaders will be supported to strengthen their leadership capabilities and contribute to anti-racist system change.

The postholder will be responsible for coordinating the development and delivery of the Leadership Academy, working collaboratively with Black Thrive localities, internal teams, people with lived experience, and external specialists to shape and deliver a high-quality learning offer.

Acting as the central point of coordination for the Academy, the postholder will oversee programme planning, stakeholder engagement, curriculum development processes, logistics, and delivery timelines. They will bring together contributions from multiple workstreams and partners, ensuring that content is aligned, relevant, and delivered effectively to meet the needs of participants.

The postholder will work closely with facilitators, trainers, and subject matter experts to coordinate sessions and learning activities, while ensuring effective quality assurance, participant engagement, monitoring, and evaluation. Responsibilities will include managing programme schedules, coordinating communications, tracking participation and outcomes, and maintaining accurate records to support reporting and continuous improvement.

The Leadership Academy Coordinator will be responsible for ensuring the successful delivery of the Academy's vision, objectives, and outcomes. They will support the ongoing development of the programme by identifying opportunities for improvement, strengthening partnerships, and ensuring the curriculum remains responsive to the needs of communities, stakeholders, and the wider system.

The role requires excellent organisational, project management, and relationship-building skills, alongside the ability to coordinate multiple priorities and stakeholders. The postholder will act as a key connector across the programme, ensuring all elements of the Leadership Academy come together effectively and provide a positive and inclusive experience for participants.

Key responsibilities

• Coordinate the end-to-end delivery of the Leadership Academy programme, ensuring all activities are well-planned and executed.
• Design and support the development of engaging training sessions and learning materials.
• Liaise with internal and external trainers to schedule, prepare, and deliver sessions effectively.
• Facilitate training sessions where appropriate, ensuring a psychologically safe, positive and inclusive learning environment.
• Track participant attendance, engagement, and progression, maintaining accurate and up-to-date records.
• Act as the main point of contact for participants, trainers, and stakeholders involved in the Leadership Academy.
• Monitor programme delivery against agreed timelines and outputs, ensuring milestones are met.
• Support the evaluation of sessions and gather feedback to inform continuous improvement.
• Contribute to the ongoing development and refinement of the Leadership Academy offer.  
• Ensure all programme delivery aligns with organisational values, including equity, inclusion, and accessibility.
• Provide regular updates and reports on programme progress to Programme Manager.
