About the role:
As a Project Worker at our Stacey Street service in Islington, you’ll support adults who have experienced homelessness or rough sleeping to rebuild stability and move towards independent living. This is a hands-on role in a busy hostel environment where no two days are the same.
You’ll work directly with residents who may be navigating challenges linked to mental health, substance use, trauma or contact with the criminal justice system. Through strengths-based support planning and clear risk management, you’ll help individuals identify goals, overcome barriers and take practical steps forward. From supporting someone to engage with treatment, to sustaining a tenancy or accessing training and employment, your work will focus on realistic, sustainable progress.
Day to day, you’ll build trusted relationships whilst maintaining professional boundaries. You’ll collaborate with internal teams and external partners to coordinate the right support at the right time, ensuring residents are prepared for successful move-on. You’ll keep accurate records, respond calmly in challenging situations and contribute to a psychologically informed environment where people feel respected and motivated to change.
This role is about creating momentum. By helping residents move from crisis towards stability, you’ll play a key part in freeing up hostel spaces for others in urgent need and strengthening SHP’s mission to end homelessness for good. This post is office based working evenings and 3 weekend evenings on a four week rolling Rota (One full weekend, a Saturday or Sunday evening and two weekends off). Our office space is wheelchair accessible.

Working in partnership with Greater Manchester Mental Health Foundation Trust and MhIST (Mental Health Support Team), the Bolton Listening Lounge is a collaborative approach to providing mental health and wellbeing support from late afternoon into the evening. It provides a safe space for people who are experiencing mental distress or who feel that they are going through a personal crisis. Our service staff will offer low level calming interventions and one to one support to identify the cause of the emotional distress and to put in place a plan including signposting and referring on to other organisations and services.
We are looking for someone with knowledge and understanding of Mental Health conditions and their impact on daily living and a strong track-record of working with individuals and their families to deliver positive outcomes. You will provide leadership to a small team of Support, Time and Recovery workers and demonstrate a commitment to a one team approach. You will have demonstrable experience of supporting staff or volunteers in their role and aptitude for managing professional relationships.
Main Responsibilities:
1. To provide leadership, guidance, supervision and appraisal to a small team of STR workers and to work with the Service Manager and HR on any absence, conduct or performance issues as they arise.
2. To be committed to providing an integrated one team approach to people who use our service and to work as part of a team to provide support to people using the Listening Lounge
3. To support STR workers to develop good practice in service delivery, recording and risk management skills and to implement Quality Assurance activities to monitor practice.
4. To manage risk and safeguarding effectively, offering support and advice to staff and monitoring high risk cases, ensuring that escalation processes are appropriately and consistently applied, and updating the Service Manager and external agencies as required.
5. To support workers in delivering targeted work with individuals to plan their support and achieve the goals and outcomes that are important to them, and to work with the wider Listening Lounge team to support the development and implementation of an action plan to support the individual.
6. To actively promote the use of outcome tools and analyse relevant data to support the Service Manager in delivering on KPI’s as required. To produce outcomes focused reports which evidence achievement and to adhere to timeframes at all the times.
7. To be responsible for the Health and Safety of STR workers on a day-to-day basis, including ensuring local policies and procedures are adhered to.
8. To develop and maintain networks with other organisations and agencies in order to strengthen safe working practices for service users, in line with organisational and local policies and processes
9. To maintain accurate case records and up-to-date case files using the agreed case management and recording system (currently INFORM). To ensure information handling is compliant with GDPR law and FA policies and procedures.
10. To attend and when required to chair team meetings.
11. To make a commitment to regular supervision, including the ability to be responsive to critical challenge, advice, feedback and direction.
12. To have an understanding and to comply with, Family Action’s procedures for promoting and safeguarding the welfare of children and vulnerable adults.
13. To deputise for the Service Manager as and when required.

About this role
The role of Health Engagement Worker will involve supporting a caseload of service users that are experiencing homelessness. The role requires the use of specialist knowledge and relationship skills to influence service users with varying degrees of mental health, addiction and trauma related issues to engage with health services.
This will include advocacy in registering them for medical services and accompanying them to appointments; assisting them in addressing all aspects of physical and mental health or substance addiction needs, as well as advocating or them to get access to source housing and social security benefits. Working collaboratively with our partners Arch Healthcare the role is dependent on good communication and joint working with a variety of partners and professionals.
The role requires a high degree of motivation to support complex and vulnerable people, often in a lone-working situation. With a passion to work collaboratively with others for the good of our service users, and with some experience of working in a related field in a voluntary or paid capacity.
A high degree of emotional intelligence is required to support service users, many of whom have complex emotional needs, with backgrounds of trauma, neglect, abuse, crime and low self-worth. A role of this nature will experience a range of emotions from joy to grief and as such requires a high degree of resilience to cope with the emotional demands of the role.
Making decisions, “in the moment” to do the right thing to mitigate risk with service users can place high emotional demands on the job holder. The role is highly reactive, unpredictable and at times, chaotic. The role therefore requires a high degree of self-awareness to one’s own resilience levels and exercising appropriate levels of self-care.

Role Description
Service Delivery
Using frontline practical and procedural knowledge of working with complex service users to support them to engage with health care services during their housing vulnerability. This could include, but is not limited to;
· Assisting them in addressing housing, financial, health or wellbeing needs

· Connecting them with appropriate support services such as substance misuse services or food banks
· Collaborating with agencies such as the local authority, housing providers and support services
· Advocating on behalf of the service user to access medical care, housing and benefits.

· Helping increase their self-confidence and self-efficacy to improve their chances of moving on from homelessness

Night Mental Health Rehabilitation Worker

Job Title: Night Mental Health Rehabilitation Worker
Location: Based in Catford in a well connected area near Bellingham, Catford, and Catford Bridge stations, with nearby bus stops. Please be kindly aware there is no step free access at this location and some of our other sites.
Salary: £27,000
Shift Pattern: 37.5 hours per week, Monday to Sunday on a rota which includes 3 to 4 night shifts a week between 20:00 - 08:30. There may be some flexibility required around these hours and work bank holidays as part of a rota, as per service requirements.
About the Role
We're hiring a Mental Health Rehabilitation Worker to join our residential Rehabilitation Service team based in South London and Maudsley NHS Foundation Trust (SLAM). The Rehabilitation service works with adults aged between 18 and 65, from Croydon, Lewisham, and Southwark. You will be part of a service which offers 24 hour support across 2 buildings in Catford. The service offers specialist intensive rehabilitation for people who require more enhanced interventions than that which is offered within community services. Our residents typically stay with us for up to nine months upon being discharged from acute wards, avoiding admission into inpatient rehabilitation. We provide residents with support to step down into less supported accommodation, becoming more independent and integrated into the community.
You will work directly with our residents to support them to overcome their personal barriers and challenges to achieve positive outcomes. You will do this through one to one support sessions, group sessions and activities, and general support with their daily living. This is not a personal care role, but is a role to rehabilitate and empower our residents towards independent living.
Key Responsibilities Include:
- Supporting residents with taking their correct medication
- Carrying out various health and safety tasks, including reporting repairs and maintenance
- Keeping support plans up to date using our online portal
- Remain alert and monitor CCTV throughout the shift
- Complete all routine night duties and any tasks handed over by the day shift or service manager
- Plan and deliver various activities which includes within the service and outside in the local community. This can include days out to museums and other areas of interest by our residents
- Carrying out housing management support including welfare benefits support
About You
We are looking for someone who has a true passion to support those who face challenges with their mental health. You will be able build rapport and trusting relations with others in a professional setting, and able to build positive outcomes. You will be adaptable and flexible in your approach, and understand that each individual has different level and types of support needs, you will be able to flex your approach to suit the needs of our resident groups. You will have an understanding and ability to help others overcome their personal barriers, and will be motivated to want to make a difference to people's lives. This is a fulfilling role, and is the perfect fit for someone who has a real passion in helping others.

We are seeking a Senior Peer Support Worker to join the team in Lewisham and line manage the Peer Support Workers.
You will use your lived experience whilst on the ward to help people to build skills to manage their home, finances, connect with family and friends, pursue social or vocational interests, to get more involved in their local community and to stay physically and mentally well.
Your support will be person-centred and may include mentoring, coaching, emotional support and facilitating access to community activities, practical support, work or study. The role will be ward-based initially until the patient is ready for discharge; you will then work with them to develop their support plans and goals. You will support them with the transition into the community for up to 6 weeks, helping them to connect with community resources to ensure they are well-supported in the community and working towards their goals.

The Vulnerable Adults service (VAS) consists of two branches - Lead Worker and Supported Accommodation. These sister services work in close partnership to provide consistent, flexible support across Birmingham 365 days a year from 9AM-9PM.

Our client has a opportunity for an approachable and communicative individual to work with clients in a holistic person-centred way to build independence, community networks and improve quality of life. You will work with people experiencing a variety of mental health conditions, including individuals with dual diagnosis, complex needs and challenging behaviour and will ensure that they feel supported, as the embark on their journey to improve their wellbeing and build interdependence. It takes a village to help them grow, so you will be there to ensure that they engage in their community and build social networks. You will encourage them to constantly improve their physical and mental wellbeing, by attending relevant appointments and ensure access to health services. It is more than that too, it is also about helping them with finances, reducing debt, and understanding the importance of budgeting, as well as finding new skills through employment or education. You will work closely with each individual, reviewing personalised support plans and maintaining relevant assessment documentation, identifying any new barriers or changes in their circumstances that may have an impact on the achievement of their goals.
With experience of working in the mental health field, you will demonstrate a good understanding of the obstacles that can be faced and how to overcome them to reach certain goals. You will be able to empathise with the daily struggles that people face and will have a commitment to user involvement and empowerment. As an ambassador for our client, you will share their values and ethos, as you deliver each support plan and remain focused on the end result.
