About the role:

At Single Homeless Project (SHP), we’re driven by the belief that everyone deserves a place to call home and the chance to build a life that feels fulfilling and hopeful. In our Hammersmith and Fulham service, we provide that opportunity through one high-support hostel and two medium-support hostels, where 24 Londoners are rebuilding their lives while managing their mental health and other complex challenges such as substance use or past offending. As a Project Worker, you’ll play a leading role in helping clients understand and manage their emotional and mental health needs, while developing the confidence and skills to live independently. Through meaningful one-to-one work, you’ll carry out detailed needs and risk assessments, identify achievable short-term goals, and help clients strengthen relationships, expand social networks and access training, education or employment. Every day will bring new opportunities to inspire change - whether supporting a resident to manage a difficult day, celebrating progress towards recovery or working alongside a dedicated team to keep our 24-hour service running smoothly, including on weekends.

Please note that this role involves shift work, including a combination of early shifts (8:00 AM – 3:30 PM) and late shifts (2:30 PM – 10:00 PM), with shifts scheduled every other weekend.

HOPE worldwide provides a compassionate, person-centred housing service ‘Two Step’ that helps people find and sustain accommodation in the private rented sector and in supported housing.

The nature of the work will require someone who is resilient, quick to learn and is a good team player. Full training and regular support will be provided. 

Relevant experience is desirable but not essential.

Salary: £29,000 – £32,000 per annum

Working hours: Full-time (37.5 hours per week - hybrid role: can work up to 2 days from home by arrangement)

Annual leave and benefits: 25 days annual leave plus Bank Holidays, 5% pension contribution, Employee Assistance Programme.

Requirements - this job is for you if you:

• Have good written and oral communication skills in English and can liaise with external agencies in a professional manner 
• Can communicate compassionately and build trust to help people navigate challenging circumstances
• Have good IT literacy and organisational skills
• Can take initiative as a member of a team
• Can work under emotional pressure and respond to stressful situations professionally and with integrity

What will you do in the role?

• Assess client needs and suitability for accommodation in the private rented sector and/or supported housing via in-person meetings 
• Provide advice, guidance and support over the phone, in person, and via email to clients and stakeholders
• Arrange viewings of suitable properties with clients and liaise with agents and landlords
• Make referrals to hostels and supported housing
• Assist clients to access grants and with benefit claims
• Support clients with issues such as rent payments, liaising with landlords and dealing with neighbours
• Update client records and referrers using our CRM system, In-Form.
• Work with the team to keep improving the service

Through our Reset Outreach & Referral Service (RORS), we support people in Tower Hamlets affected by drugs and alcohol. Our team takes a harm-reduction approach: meeting people where they are, reducing stigma, and helping them access treatment, health services, and wider community support.

The ideal candidate

The ideal candidate is passionate about providing quality support to diverse groups of people in East London. You should have the drive and ambition to support your team to make positive lasting changes in people’s lives.

You might have some knowledge of substance use or transferrable skills from other experiences, like working in fast-paced environments where you had to respond to customer needs. The experience you have can be paid or voluntary and lived experiences of using substances is equally valued. 

You should be eager to engage with people about different drugs and alcohol and be understanding of how different barriers can impact a person’s access to services or education. Responsibilities

About the role:

Street Outreach & Engagement – Identify and build relationships with rough sleepers and beggars across various locations and times, including those who are disengaged from services.
Assertive Outreach & Support – Work closely with entrenched rough sleepers, encouraging engagement with services and providing rapid assistance.
Substance Use & Healthcare Access – Connect individuals with drug and alcohol support, mental health services, and other healthcare provisions.
Case Management & Housing Support – Oversee service users’ treatment journeys, assist in hospital discharge planning, and collaborate with housing providers to secure appropriate accommodation.
Partnership & Multi-Agency Work – Act as the key link between CGL services, rough sleeping partners, hostels, shelters, the local council, and other support agencies to coordinate care and maximise opportunities for recovery.
Crisis & Emergency Response – Participate in monthly overnight street counts, unsociable-hour shifts, and emergency interventions during severe weather conditions.
Administration & Best Practice – Maintain accurate records, attend case conferences, and contribute to partner tasking meetings to ensure service users receive consistent and effective support.

About you:  

• Have a strong understanding of substance misuse issues and experience of working within a similar field 

• Be an excellent communicator, both verbal and written with good IT skills 

• Proactive and able to work on own initiative with excellent time management and prioritising skills 

• Have a good working knowledge of mental health interventions, services, and good practice 

• Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders 

• Ability to manage change successfully in a way that prioritises the needs of service users 

• Understand the importance of information governance processes and commit to follow and apply all necessary safeguards 

• Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
