This is an excellent opportunity to truly make a role your own and get involved in the project from the start, which will be supporting 10 adults with mild/moderate to severe learning disabilities, as well as additional mental and physical health needs.
You will play a pivotal ambassadorial role, spearheading Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.
This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. This exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.
Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.
The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be based expected to work across all three sites
- Work on 7 day rolling rota inclusing early, mid and late shifts
- (K&C): medium – high support accommodation.
- (Lambeth): medium support accommodation.
- (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.
Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.
In this role you will jointly manage the safe day to day running of the project with other members of the team and to provide basic support and advice to clients.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.
As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.
You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

As the post-holder, you will be working in our Camden Women's Recovery Service hostel providing specialist interventions for a designated caseload and to the wider client group, via our in-house recovery programme.
Your role will be working intensively with the residents on their supports needs. The role of Complex Needs Project Worker is a specialist role, designed to work with people who may be more difficult to reach and establish working relationships with. As such, you will role model good practice for working with those who have experienced multiple disadvantage by adopting a trauma informed approach.
Within this setting, you will also work to establish a culture of co-production where residents are at the forefront of the work that is carried out, helping to promote autonomy and build individual confidence and interpersonal skills.

We're looking for a Children and Family Worker to join the team in our West London Women's refuge. The service is a refuge accommodation for women and children surviving domestic abuse many of whom are living with trauma. we are looking for someone who really cares about the rights of women and their children.
As a Children and Family worker, you will support in providing a much needed safe and nurturing environment for children and their mother's, providing pathways, support and appropriate interventions to enable resettlement. You will support various teams to co-produce a tailored support plan which is altered to the individual's needs through a recovery model providing trauma informed and enabling environments to achieve this.
Shift Pattern: 37.5 hours per week, working on a rota which is over 7 days including evenings. You will be based on site within the service.

- Support with reducing barriers to reintegrating into communities, and re-developing confidence, skills, and coping mechanisms to support those with previous trauma.
- Develop and sustain therapeutic relationships with our residents, providing practical and emotional support with a trauma informed approach to ensure they are always treated with respect and dignity. Deal with sensitive information in a professional manner.
Additional Responsibilities
- Attend appointments as and when required in support of residents.
- Undertake shift lead responsibilities on rotation, taking responsibility for ensuring day-to-day support delivery is effective, with staff carrying out their designated responsibilities, alongside ensuring resident and service-specific follow up actions are completed.
- Housing Management responsibilities, ensure residents and staff are safe, and the surroundings are kept clean and well maintained.
Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position.
KEY CRITERIA
What we are looking for:
- Understanding and/or experience of working with people of complex backgrounds
- Previous experience and/or in-depth knowledge about working with children who have experienced trauma
- IT Proficiency, ability to learn new software programs, basic Microsoft experience
- Understanding of the trauma children and families may go through due to domestic violence and other previous experiences
- Understanding of the social needs and support required for people with multiple and complex needs
- Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions
What we would like, but not essential:
- Previous experience creating co-produced support plans, key working, and care planning, providing appropriate interventions for service users
- Previous experience providing housing support and practical assistance within a residential support role
- Appropriate Professional Qualification in: Social Care, Early years Education, Certificate in Childcare or other relevant
- Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health
- Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis

This role will suit a support worker looking to working throughout the night. You will report to the team leader and engage with/ support customers during anti-social hours.
This is a great opportunity for a Waking Night Support Worker, wanting to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT). We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.
You will be based at Hope House. We have two sites: Hope House and Doddsfield Rd. Hope House/ Place is a 10 bed supported housing service, providing support for 16 customers overall. The support hours for Hope House are 24 hours, 7 days a week, with one waking night.
Doddsfield Rd has 6 independent living flats with floating support.
This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.
What you'll do:
- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager.
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
- Develop and maintain links with all key agencies and service providers in the local community.
- Empower customers to ensure they receive the service and benefits they are entitled to
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team.
- Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
- Adhere to Look Ahead's Policies and Procedures.
- Engage in learning and development activity to increase knowledge and skills.
- Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned.
