As Project Worker Complex Needs, you will work creatively and collaboratively with adults who have experience of homelessness and/or Rough Sleeping. You will provide an effective high quality support service in an accommodation setting, working with clients facing Multiple Disadvantage, and living with complex needs.

About the role:

SHP’s aim is to improve outcomes for service users both within the service and by enhancing their ability to utilise external specialist, statutory and community services.

The support you provide will ensure that clients are able to:

• Access and sustain their accommodation within our services
• Access and feel safe in engaging with specialist support in relation to their needs (i.e., substance use, health, mental health, complex trauma, offending, street activity}
• Access primary health care, and improving their wellbeing and process of recovery.

• Develop and maintain positive personal relationships

You will be part of a dynamic team in a hostel setting, holding a caseload of clients with the opportunity to draw on specialist roles within the team, the organisation more broadly, and working in tandem with specialist external agencies. Your ability to forge meaningful relationships will provide the basis of the work to empower your clients, helping them to engage with a range of community resources and build a robust network of support.

About you:

• Ability to maintain motivation for a high level of contact with clients on a day-to-day basis.
• A non-judgemental approach to working with multi-disadvantaged / complex needs clients and to promote a strengths-based approach.
• Familiarity with IT applications and basic keyboard skills and the ability to maintain accurate records and manage own administrative tasks.
• Strong team-working and interpersonal skills, maintaining a collaborative approach to delivering service objectives across work specialisms.
• Strong time management skills, ability to work on own initiative, manage competing priorities and maintain high standards.
• Be expected to operate primarily in your supported accommodation service, as well as some off site work duties; to facilitate client appointments and conduct professional meetings.
• Willingness and ability to work on a rota system of early and late shifts, which may include some variable hours including some evenings and weekends.
• Understanding of professional boundaries and their importance when delivering trauma informed support.

What you'll do:

If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate

Support customers to undertake all domestic tasks wherever possible, including practical assistance where customers have not yet developed the skills, to ensure they enjoy a high quality of accommodation

Participate in the support planning and risk management, as facilitated by the lead Support Worker

Carry out security duties to ensure the safety of the customers and premises

Monitor the CCTV throughout the shift

Enable customers to make full use of community facilities by providing support as directed

Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities

Report any observations relating to customers welfare

Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements

We are looking for a part time Information & Advocacy Community Support Worker in Scotland.  You will be home based living in the North of Scotland.

You will ensure that service users (children and adults) and their families, receive high quality support, from the point of diagnosis and throughout their journey, and will assist them to access funding, equipment and care and support that they are entitled to. You will join a team that makes a real difference to the lives of the people living with muscle-wasting conditions, and their families, in Scotland. The role will be primarily home based, but there will also be a requirement to attend outpatient clinics within the region, attend home visits where appropriate and to support responding to queries through the helpline and information inbox. About the Role

The Young Adult Support Worker will work as part of a team delivering an assessment, support and move-on service to vulnerable clients in supported accommodation. The successful candidate will have a caseload of 8 clients aged 18-25, in a male-only supported accommodation in Waltham Forest.

You will be required to work from our offices in Lambeth once a week approximately or as the business need arises. Your work will be underpinned by the Depaul Endeavour model of assets-based, psychologically informed delivery. The aim is to ensure that every client leaves a Depaul service with the skills and resilience to be confident and self-determining in their lives.

Working under the direction, guidance and support of a manager/senior worker you will be responsible for a caseload of clients and share responsibility for the safety and wellbeing of all clients, volunteers and staff in the service. In accommodation-based services, you will ensure that the accommodation is properly maintained and support the client to sustain their tenancy and participate in a positive and successful move-on.

Key responsibilities include:

· You will carry a caseload of clients for whom you will be the named key worker, and you will also take joint responsibility with your team members for the well-being and day-to-day support of all clients in the services

· You will lead on risk and needs assessment and support planning for your key clients and contribute to ongoing needs and risk assessment of other clients in the service

· You will develop and implement SMART outcomes based support plans for your key clients

· You will deliver one-to-one and group support opportunities within their accommodation and in the local community aimed at increasing resilience in the clients we work with.

· You will proactively work in partnership with multiple agencies including statutory teams, to support the well-being of Depaul clients following data protection and information sharing protocols.

About the Role:

The Assessment Coordinator is the conduit for young people to access the youth accommodation pathway in Greenwich.  As well as triage and intake, the role supports young people by navigating their journey either by facilitating their safe return home, or into longer term suitable supported accommodation.

You will provide specialist assessment and interventions for a designated caseload. You will provide mediation where needed and advise/ facilitate restorative work with the young person and their family. 

About you:

• You have two years’ experience of assessing the needs of young people in a residential/ accommodation setting. 
• Proven experience and skills in delivering casework and planned support for young people. 
• A strong understanding and practical knowledge relating to issues that typically disrupt progress in journeys towards independence.  E.g. mental health, substance use, self-harm, domestic violence, anger management etc 
• Evidence an ability to navigate between and across specialties whilst working with clients to create a multi-agency support plan. 
• The ability to work within systems and families, to enable young people (that can) to return home.

Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?

Our client provides a rapid response service, intervening to ensure no one needs to sleep rough, their work is ongoing and they are currently looking for people to join their services across London. 

As an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with people new to, or at risk of rough sleeping. lly involves the client.  th

• Build and maintain effective relationships to support clients throughout their recovery.

• Develop a knowledge of relevant legislation such as, welfare benefit and housing legislation.

• Work on a weekly rota including early, mid and late shifts (some evening and weekend work may be required). 

About you

They are always on the lookout for enthusiastic people to join them.
