Role Requirements

• Support planning and changes in the administration of children and young people’s timetable for children and family services.
• Support with photocopying of notes for subject access/ solicitor requests (SARS).
• Manage the SARS and redacting process with appropriate members of the Therapies teams.
• Update logs and coordinate the Clinical and Safeguarding Supervision across the Therapies teams, escalating non-compliance where required.
• Monitor mandatory training compliance and administer booking processes for all relevant training sessions/course.
• Coordinating specialist clinics, including spine and hip clinics; liaising with the Physiotherapy team to ensure the correct children are on the schedule. Liaison with external Consultants to organise the clinic lists.
• Carry out audiotyping of clinic letters and distribute as required.
• Update absences within the teams onto the SMI (staff care) system, as requested by the Heads of Service.
• Place orders for any required equipment and/or resources for departments and for the children and young people as deemed necessary.
• Take overview of clinical asset ordering, linking with the clinical assets team and management system, in line with MHRA guidelines and the medical devices policy.
• Track and monitor equipment orders with Heads of service, escalate delays and provide a central overview for all equipment re-charges and ordering.
• Raise and receipt PO’s and invoices for the therapies directorate and liaise closely with the finance team.
• Undertake other or additional duties that are within your skills and abilities, as the organisation may reasonably require from time to time.

With experience of working in a complex environment, across a large and diverse workforce, you will be exceptionally organised with a high-level of attention to detail. You will naturally possess excellent inter-personal skills, and an ability to consult and positively engage with key stakeholders across the organisation.

This is a very special role to provide holistic health and wellbeing of those who visit our open drop-in at the Vineyard Community Centre in Richmond. To us, they are our guests, not 'clients', 'patients' or users'.

We have a vacancy for a Flourish (Refugee and Asylum Seekers) Comminity Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.

Complex needs are defined by the National Complex Needs Alliance as a combination of health needs and social needs.

About the Service

‘Flourish’ is a highly flexible and responsive outreach service developed to meet the needs of refugees and asylum seekers in Hertfordshire. The aim of the service will be to support refugees and asylum seekers in the community with multiple needs who have mental health support as a primary need. 

Purpose of Post

• To provide advice, information, onward referral and holistic support to clients with mental ill health.
• Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
• To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• To remain a source of independent support for all clients.
• Embed a person centred and solution focused approach in all aspects of the role.
• To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.

Key Responsibilities:

• Working closely with all key delivery partners across the system to ensure the service is responsive, inclusive and easy to access.
• To take allocated referrals from any agencies including the Community Navigators, Borough and District Councils, Herts Help, Housing Providers, Herts Welcomes Refugees, the Refugee Council and self-referrals including walk in contacts at any of the Mind wellbeing centres across the county.
• Hold a caseload of clients, supporting for approx. 10 weeks
• To create a safe space for the individual to be able to explore their feelings and improve their understanding around their mental health.
• To complete a holistic needs assessment.
• Ensure all person centred risk assessments and support plans are completed.
• Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
• Support clients to build their emotional resilience, promoting self-management through building an understanding of their own triggers/coping strategies.
• To provide safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
• To work alongside translators when required to provide a robust package of support for clients.
• To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services in the Borough.
• To provide advocacy services to clients experiencing mental ill health.
• To assist clients to access appropriate mental health services, housing/tenancy services and other services according to assessed needs.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working, working with other third sector and VCSE organisations to provide signposting and continuity of care.

The Role

The Community Support Worker will be required to work alongside the full-time area Co-ordinators to support activities and groups and carry out one-to-one initial appointments for people residing in Surrey.

About the Service

The Refuge Service provide emergency accommodation to women and children fleeing domestic abuse and sexual violence. Refuge Case workers provide emotional and practical support this can include housing support, legal options, reporting to the police, help around child contact, benefits, financial advice and accessing therapeutic support services.

About the Role

You will provide advice and support to women who have experienced domestic abuse in a refuge setting working across two sites.  You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You should have a minimum of two years’ experience of working with this or a similar client group. An in depth understanding of the needs of women and children from a diverse background is essential.

About You

The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.

You will have experience of managing risk and following case management procedures to meet the needs of the client group, and the ability to work with women to help them to understand their needs, risk assess, develop safety plan, and support them in linking with other agencies.  

You will have a working knowledge of the available legal remedies, housing, and welfare benefits, as well as experience of providing workshops.

In addition to case management, you will be required to undertake housing management responsibilities including health and safety and maintenance of the refuges.  You will have excellent administrative and time management skills.

About this role…

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for an  Assistant Support Workers  to join the Street Outreach Rough Sleepers Team

Thames Reach welcomes applications from people with transferable skills.

You will:

• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Work alongside senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Maintain and support relationships with partner agencies 
• Accurately record all work completed within given timeframes

The role is within a small team that has a focus on Employer Relationships, Job Finding programmes (including our Alumni Job Club and Jobseekers Plus) In Work support for adults in Employment and Employer Training.

Key Responsibilities: 

1. To lead on running our employability training and Alumni Job Club.

2. To develop workshop structures and learning materials for these in close collaboration with the Development Director and ensure that participant feedback from earlier programmes is incorporated.

3. To line manage Development Job Coaches as needed.

4. To manage and deliver job search sessions.

5. To lease with Employability Lead when participants are ready to move over in In Work Support team.

6. To advertise project opportunities to autistic people throughout London and manage project recruitment and onboarding.

7. To ensure processes are in-place for each project to:- assess each participant’s individual skills and gather baseline data; ensure job matches are appropriate to the individual’s needs and aspirations; and that targets set are relevant and promote new skills and learning; acquire and complete necessary risk assessments to ensure safety of clients, staff and partners.

8. To oversee the set up and settling in of jobseekers into work placements, when needed. Providing support and training to employers where required; providing personalised on the job training to participants and employers as required and develop strategies and advise on reasonable adjustments.

9. To coach, train and help manage new project staff to support jobseekers and put in place monitoring protocols.

10.  To ensure jobseekers are progressing towards their individual targets; understand protocols and interpret work culture correctly; understand the health & safety in the work environment and adopt appropriate workplace behaviour.

11.  To ensure good working relationships are maintained with all employers/ work experience placements and that feedback is regularly sort from all stakeholders to help improve our service delivery and future opportunities.

12.  As a key member of the Development Team to work closely with the Development Director to actively, engage with employers and external organisations to promote CareTrade and our job seekers and to pursue opportunities to work collaboratively.

13.  To attend events, to market CareTrade to autistic jobseekers and to liaise with Disability Employment Advisors at JobCentre Plus, Disability Services teams, Social Workers and others to promote and support employment opportunities as needed.

14.  To organise and promote social groups for jobseekers.

15.  To collect data and use data analysis to help produce reports, internally, for funders and partners.

16.   To help seek and apply for appropriate awards and grants when required.

17.  To be a Designated Safeguarding Lead (DSL)  and First Aider and complete regular training working alongside the Development Director to ensure the safety of participants and staff.

18.  To complete relevant health and safety training and be a named fire marshal.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.

• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

Responsible for:  CHWS Crisis Support Workers and Recovery Space Workers

Hours: 36 hours per week (full time) worked within a 7-day rota (7days a week/365 days a year) The current service operates a daytime service, 9.00am – 6.00pm and evening service 3.00pm – 11.00pm

(This will include weekend work, approximately one in three weekends)

Location: Main place of work will be Fairfield House, 10 Altyre Road, East Croydon.  Some of the team will be based at Croydon Health and Wellbeing Space at the Whitgift Centre, Croydon (and other locations that may be used in the future) and the Outreach Worker will be predominantly working from the Community Hubs around the Borough.

Mind in Croydon is a charity concerned with the welfare of people with mental health problems living in the London Borough of Croydon and the surrounding areas. Mind in Croydon delivers Mental Health Transformation Program services alongside Croydon BME Forum, these include Croydon Health and Wellbeing Space and Mental Health Personal Independence Coordinators. Other services include Recovery Space, Advocacy, Active Minds, Carers Support, Peer Support In-Reach, Social Networking, Counselling, and an Information Service.

This exciting role will manage the provision of a non-clinical Recovery Space (crisis café) funded by South London Partnership Commissioning. This project operates in the day and evenings, people are referred by our Trusted Assessors in various Teams such as Croydon University Hospital, Psychiatric Liaison, Assessment and Liaison, Croydon College, Home Treatment Team and others.

The aim of the service is to divert people, who would be better supported elsewhere, away from Accident and Emergency (A&E) and towards services that can support with their immediate crisis within the Borough of Croydon. The Recovery Space Service Manager leads this service which is non-clinical but a warm and welcoming setting to support and hold individuals during the day and evening. The Recovery Space service referrals will not be accepted after 10pm for attendance at night.

The role will work closely at senior operational level with external partners, clinical, commissioning, and voluntary sector organisations to plan, develop and promote the highest quality service. They will manage a team of nine staff, three are Crisis Workers, operating a day service, one outreach crisis worker also operating during the day and five Recovery Space Workers in the evening.  The role will need to produce reports, develop, and implement policies and procedures in line with best practice and to ensure the service is making a difference to people’s lives.  The role will work with other service managers including those managers within the Transformation services to ensure a One Team approach to provide an effective and efficient service for clients.
