The Hepatitis C Trust develops projects nationally where peers use their lived experience of injecting drug use and hepatitis C, to provide education and training, increase hepatitis C awareness, and access to testing and treatment for people living with hepatitis C. 

Working with the South West Peninsula ODN, The Hepatitis C Trust are now expanding this commitment through the introduction of the HCC (Hepatocellular Carcinoma) surveillance and early liver disease detection programme.

We are looking for a self-motivated individual who can work on their own, engage with stakeholders and have a desire to make a difference in promoting liver screening in community settings, and by increasing access to specialist care services.

Working in partnership with local hospital and community outreach clinics, the post holder will ensure that patients are supported through initial liver assessment and ongoing surveillance as required. The post will coordinate delivery of educational and awareness workshops and promote the delivery of liver screening in community settings 

Working under the guidance of the South West Peninsula Peer Programme Manager, the post-holder will work alongside other identified partners to help develop a community outreach liver surveillance program and carry out HCC early detection activity and interventions across Plymouth.

Administration

You will support the whole organisation with a range of administrative tasks which will include:

· Completing minutes for key meetings.

· Processing of Expenses on behalf of the Senior Leadership Team.

· Arranging room bookings both internally and externally.

Cover Support

You will provide cover on the front of desks of both our sites: Cancer Support Centre, Northumberland Road and our Charity Hub, Whitham Road which will include:

· Meeting and greeting visitors including supporters and patients and acting as their first point of contact for any queries.

· Dealing with telephone, email and postal enquiries across the organisation

· During cover period, you may be required to open and close the patient spaces, as part of a wider team rota.

· Booking in and logging patients onto the relevant records and databases to provide accurate data.

· Sending appointment reminder calls/texts/letters to clients as required

· Co-ordinating the Transport service, including supporting and co-ordinating the volunteer drivers.

· Managing the consumables within this patient space and ordering replacement stock as and when required.

Job Description

Communications:     

• Develop, contribute to and implement impact-driven and strategic communications campaigns that build Kineara’s fundraising capacity and raise awareness of the issues
• Develop brand identity for Kineara’s new charitable arm, ensuring it is consistent across all activities in the organisation, while maintaining Kineara’s overall ‘look and feel’ to ensure coherence in branding, colours, messaging, style and approach.

As a Waking Night Mental Health Support Worker, you will play an integral role as a part of the team by offering the continuity of care and support in every aspect of their needs in line with individual support and risk management plans, such as medication administration, emotional support and regular welfare check, personal care and so on.

Key Responsibilities

• Complete a holistic assessment with individuals to establish their employment needs.
• Be able to offer short-term advice and guidance in relation to a work situation.  
• Engage with employers to support both them and their employees, provided the employer takes up this offer. 
• Facilitate group sessions with employers and their employees as and when required.
• Provide accompaniment to Occupational Health appointments; employment meetings; oversee reasonable adjustments and phased return to work; subject to agreement from all parties.
• Support or provide information for individuals to complete an employment WRAP (Wellness Recovery Action Plan), which may be shared with their employer.  
• Develop relationships with multiple agencies and partners to provide holistic approaches to support individuals e.g. NHS mental health/wellbeing, services and with participant consent undertake case conferences with external agencies to jointly optimise support packages.
• Work within Pluss and community outreach locations (sometimes alongside other services e.g. housing, health and primary care centres) to increase service accessibility.

The Refugee and Migrant Centre (RMC) is an established, multi award winning charity that provides a safe, welcoming environment to thousands of refugees and migrants living in the Black Country and Birmingham.

Our highly skilled and diverse staff team provide a broad range of services including advice and support on immigration (OISC regulated level 1, 2 & 3), employment, welfare, education, ESOL, housing, homelessness, destitution, citizenship, health and wellbeing.

RMC have been assisting beneficiaries to access and sustain appropriate accommodation and improve their housing and living conditions for nearly two decades. RMC also deals with many aspects of homelessness, destitution, rough sleeping, exploitation, etc.

What you will be doing
