We are looking for a Community Support Worker to join our team. The Primary Care Network Service works in partnership with GP surgeries in Hertfordshire to provide advice, information, onward referral and holistic support to individuals who are experiencing mental ill health or need support with their mental wellbeing. Supporting people in the community in their own homes and our wellbeing centres.

Community Support (PCN) Service Objectives

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To remain a source of independent support for all clients.
• To ensure correct access via Primary Care into the HMN service therefore reducing the pressure on GP services
• To contribute to an improvement in the proportion of people with mental ill health who are accessing specialist support via their GP surgery
• To provide an assertive outreach approach for clients that may have complex or multiple needs.

The Successful Candidate will

• Provide advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing.
• Maintain and enhance service delivery standards and effectiveness.
• Promoting the service, increasing visibility and being responsive to changing needs.
• Provide practical and emotional support to encourage service users to develop their independence within their local community.
• Ensure that the safety and wellbeing of service users using the service is monitored and reviewed regularly.
• To remain a source of independent support for all clients.
• Embed a person centred and recovery orientated approach in all aspects of the roles and responsibilities.

Key Accountabilities

• To receive referrals a wide range of referral pathways.
• To undertake initial contact with clients and complete a holistic needs assessment, making use of assertive outreach work to achieve the initial client contact, if required.
• To provide holistic support both outreach and in our centres and advocacy services to clients experiencing mental ill health.
• Ensure all person-centered risk assessments and support plans are completed as appropriate.
• To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services.
• To liaise with other agencies to ensure that clients receive appropriate support and continue to remain engaged with the service.
• To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working.

Join GMWSA and help women build positive futures after prison

The Greater Manchester Women’s Support Alliance (GMWSA) is looking for a passionate and motivated Link Worker to support women who have experienced multiple recalls to custody and are preparing for release from HMP Styal.

This is an exciting opportunity to join a specialist, women-centred organisation committed to delivering trauma-responsive, gender-specific support to women facing multiple disadvantage.

Working directly with women in custody, you will support them to overcome barriers to successful resettlement, reconnect with services in their local communities, and build the foundations for a more stable future.

About the Role

As the HMP Styal Link Worker, you will provide one-to-one support to women from Greater Manchester who have been recalled to custody. Many of the women you will work with will have experienced trauma, domestic abuse, poor mental health, homelessness, substance misuse, family separation, and other complex challenges.

You will:

• Build trusting relationships with women whilst they are in custody.
• Assess individual needs and develop person-centred support plans.
• Support women to prepare for release and reintegration into their communities.
• Link women into local Women’s Centres, housing providers, and specialist support services.
• Work closely with prison staff, probation, housing providers and community organisations.
• Provide practical and emotional support before and immediately after release where required.
• Advocate on behalf of women to ensure they can access the services and support they need.

This role requires someone who can combine empathy, resilience and strong professional boundaries while working in a challenging but highly rewarding environment.

About You

We’re looking for someone who is passionate about supporting women and understands the challenges faced by those involved in the criminal justice system.

You will ideally have:

• Experience of supporting vulnerable adults with complex needs.
• Experience in advocacy, support work, case management or related roles.
• An understanding of trauma-responsive and strengths-based approaches.
• Knowledge of issues affecting women involved in the criminal justice system.

About the role

This role is based in Pontypool Police Station, Pontypool, it is offered on a hybrid working basis and there is a requirement for travel throughout the area.

As a Caseworker- Children and Uong People you will be:

The postholder line manages the reception team and Volunteer Drivers while coordinating business equipment, office consumables, non-clinical cleaning, secondary catering coordination and waste management. The role ensures strong governance, safeguarding awareness, cost control and operational resilience across all non-clinical facilities services.

Working closely with clinical, corporate and estates colleagues, the Support Facilities Coordinator ensures that the Hospice presents a calm, compassionate and professionally managed environment from first point of contact through to internal workplace experience.

In summary, the Support Facilities Coordinator is accountable for:

Community Support Lead: Partnerships 

Reports to: Community Support Manager 
Contract: Full-time 

Role Purpose 

As Community Support Lead: Partnerships, you will play a key role in strengthening the relationships that underpin HOST’s work, ensuring every hosted partner (HP) receives consistent, responsive, and values-driven support. 

You will lead on partner onboarding, engagement, and ongoing support, ensuring needs are met with professionalism, empathy, and efficiency. This includes managing communications around invoices and payments, responding to general queries, and proactively identifying and escalating potential risks. 

Blending strong attention to detail with a people-first approach, you will shape and continuously improve the partner experience — from initial onboarding through to long-term collaboration — ensuring every interaction reflects HOST’s mission to support the world’s change-makers. 

Core Responsibilities 

1. Partner Engagement and Onboarding

• Lead Welcome, Onboarding, and regular check-in calls with hosted partners, building clarity, trust, and consistency in every interaction. 

• Provide clear guidance on HOST’s systems, processes, and expectations, ensuring each partner journey is well-defined and smoothly implemented. 

• Identify risks, issues, or emerging support needs during interactions, and promptly share these with the Community Support Manager. 

• Support escalations and follow-up actions to maintain a high-quality, responsive partner experience. 

• Maintain accurate and up-to-date records of all communications, ensuring relevant information, issues, and risks are clearly documented and shared across the team. 

• Negotiate PAYF contributions, add-on pricing, and additional services, ensuring partner needs are met efficiently and effectively. 

Reporting cadence: Weekly summary and inclusion in monthly CS reporting. 

2. Query Resolution and Partner Care 

• Respond to general hosted partner queries with accuracy, clarity, and empathy, achieving a minimum 90% satisfaction score. 

• Ensure at least 90% of queries receive an initial response within 24 hours. 

• Maintain consistently high standards of written and verbal communication, reflecting HOST’s values of integrity, care, and responsiveness. 

• Collaborate with Finance and Operations teams to ensure partner needs are resolved promptly and effectively. 

Reporting cadence: Weekly dashboard updates and monthly performance reporting. 

3. Partner Relationship and Invoice Management 

• Work closely with hosted partners to amend invoices and resolve payment queries accurately and in a timely manner. 

• Ensure all payment-related communications and outcomes are clearly documented in ClickUp and Zendesk. 

• Monitor and report on invoice corrections, identifying trends and opportunities for process improvement. 

Reporting cadence: Monthly 

4. Risk Identification, Screening and Escalation 

• Identify and assess potential financial, operational, or reputational risks arising from partner interactions. 

• Conduct initial screening of partner-related concerns in line with HOST’s due diligence processes. 

• Escalate identified risks promptly to the Community Support Manager or relevant stakeholders for review and mitigation. 

• Contribute to monthly Community Support risk reporting and ongoing team learning. 

Reporting cadence: Real-time escalation of risks, with consolidated monthly reporting. 

5. Community Support Administration 

• Maintain accurate, up-to-date records across ClickUp, Zendesk, and internal systems. 

• Support the review and updating of standard operating procedures (SOPs) to ensure consistency and clarity across processes. 

• Contribute to internal administrative tasks, ensuring operational readiness for audits, reviews, and ongoing compliance. 

Reporting cadence: Monthly 

6. Community Support Resources and User Journey 

• Contribute to the development and continuous improvement of Community Support materials, guidance, and onboarding resources. 

• Support the enhancement of the hosted partner user journey, ensuring all touchpoints are clear, consistent, and aligned with HOST’s values. 

• Work collaboratively with colleagues to identify gaps and opportunities to strengthen the overall partner experience. 

Reporting cadence: Quarterly reviews with recommendations for improvement. 

7. Growth and Learning Areas (Development Focus) 

• Take ownership of more complex hosted partners with advanced operational or financial requirements. 

• Support the development of new Community Support Leads through training, mentoring, and sharing best practice. 

• Deliver or support hosted partner Health Checks, ensuring all follow-up actions and documentation are completed effectively.

You Bring 

• Experience in operations, client service, or relationship management within HOST. 

• Strong organisational skills with a focus on accuracy, responsiveness, and care. 

• Excellent communication and interpersonal skills, with confidence engaging across cultures and time zones. 

• Familiarity with digital tools such as ClickUp, Zendesk, and cloud-based systems.

Performance Indicators 

• 90% of hosted partner queries receive an initial response within 24 hours

At Walking With The Wounded, we support those who have served with mental health, employment and wraparound care coordination programmes tailored to the unique needs of the individual. Whether mentally, socially or physically wounded, we provide bespoke services dedicated to getting those who have fallen the furthest back on track and moving forward with their lives.

As part of Op COURAGE, the veterans mental health and wellbeing service, we will be employing a new Veteran Liaison Support Officer. This role will work in collaboration with Pennine Care NHS Foundation Trust to support veterans in the mental health pathway.

The successful candidate will ideally be located in the Greater Manchester area with the ability to travel where required.

WWTW’s Care Coordination service involves the assessment of needs for your client, the sourcing of appropriate support and the ongoing navigation of that support. You remain the central point of contact as you support your service user to address their challenges, which may include a lack of secure housing, debt, social isolation, relationship breakdowns, lack of education and training, welfare issues or substance misuse.

We are looking for a Veteran Liaison Support Officer (VLSO) who can work within a muti disciplinary team and fully appreciates the patience, resilience and self-motivation that is required to support a dispersed and diverse caseload of individuals. You must be someone who is able to work independently, particularly in managing your time and caseload, whilst recognising that you are one crucial element of a wider team of support who will need to work collaboratively to get the best outcome for clients. We welcome applicants who have experience or specialise in housing, although it is not essential.  Experience of working with veterans would be beneficial although again, not essential.

The Veteran Liaison Support Officer (VLSO) will have a caseload of ex-servicemen and women who may, at times, be facing multiple complex needs. Working with local delivery partners, you will make referrals for support, whilst retaining the overall caseworker responsibilities. About the role

As a Children and Young People Caseworker you will:

• Deliver excellent services to Children and Young People who have been victims of crime through the direct provision of information, practical and emotional help from the point of crime and where appropriate.
• Help Children & Young People cope and recover from the effects of crime.
• Act as a single point of contact, undertaking effective need and risk assessments.
• Coordinate the overall delivery of the cope and recovery plan and recovery journey, working closely with other service providers to meet the needs of victims.

You will need:

• Previous experience delivering a service and working directly with service users in a statutory, voluntary or community work setting.
• Previous experience of working with vulnerable CYP.
• To be able to work without direct supervision, prioritising work and managing competing demands.
• The ability to complete needs and risk assessments.
• To demonstrate empathy.
• Knowledge of the Criminal Justice System and Safeguarding principles.

St. John's is a non-maintained specialist provision, working with autistic people, the majority of whom also have learning disabilities. Friends of the Elderly is a charity dedicated to providing excellent care and support to older people for over 120 years. We do this via our grant giving service, which provides vital financial support to older people who are living in poverty. We also deliver services personalised to individual needs and integrated with local communities at our care homes and day centres.

We now have an exciting opportunity for an experienced fundraiser to play a vital role in our small, busy fundraising team.

In this role, you’ll play a key part in delivering outstanding supporter care, ensuring everyone who donates or fundraises for the charity has a positive and rewarding experience.

You’ll be responsible for processing all fundraising income accurately and efficiently, taking ownership of essential administrative tasks that keep our fundraising activity running smoothly. You’ll manage income from individual supporters, including regular donors, legacy supporters, event participants and seasonal campaigns such as our Winter Appeal. Working closely with our homes and services, you’ll also help to support and grow their fundraising efforts, making a meaningful contribution to the success and impact of the organisation.

We are looking for someone with experience in supporter care, customer services or fundraising along with the ability to build and maintain relationships with funders / customers.

You will be confident using databases to accurately record information and generate reports, as well as handling data, numerical information, and financial administration tasks effectively.

Good attention to detail and a high level of accuracy will be required and you’ll have the ability to manage a number of projects, prioritise your own workload and meet deadlines.

Good computer skills including Word, Excel, Outlook and database are essential.

Working in the not-for-profit or care sector and proven success in achieving and exceeding fundraising targets would be preferable.
