Job description

As an Intensive Support Case Coordinator, you will play a key role in delivering our Intensive Support Programme, ensuring that people living with HIV who have a detectable viral load receive personalised, responsive support tailored to their individual needs.

Working closely with colleagues, healthcare professionals and partner organisations, you will coordinate a person-centred service that helps people overcome barriers, access the right support, and improve their wellbeing. Through effective partnership working, you will connect individuals with the services, opportunities and resources they need to thrive.

This is an exciting opportunity to join an established and highly respected programme that makes a real difference in people's lives. You will help build on our strong reputation for delivering high-quality support, while contributing to our vision of a world where HIV holds no one back. Every person living with HIV deserves the opportunity to live well, regardless of viral load, and your work will help make that a reality.

Please note that the successful applicate must have a full, clean driving licence and access to a car.
