About the Service
Community Outreach Workers support people with complex needs who have Housing issues as a primary need. This includes people who may have drug and alcohol difficulties. Particularly when they are coming into repeat contact with Police, Tenancy Sustainment Services, Housing and Homelessness Services and Environmental Health Services.
The five-day post will be delivered in West Hertfordshire. With the flexibility to travel Hertfordshire County.
The purpose of the Hertfordshire Mind Network Community Outreach Worker role is to:
- Provide advice, information, onward referral and holistic support to clients with coexisting drug and alcohol difficulties and mental ill-health. These will be clients who are presenting to the Police, Anti-Social Behaviour, Tenancy Enforcement, Housing and Environmental Health Services within Hertfordshire.
- Ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
- Remain a source of independent support for all clients.
- Embed a person centred and solution focused approach in all aspects of the role.
- Raise greater awareness of complex needs and the effects of Adverse Childhood Experiences (ACEs) amongst local service providers. Supporting them to gain confidence in working positively and consistently with people who have multiple needs.
The Community Outreach Worker will:
- Receive both self-referrals and those allocated by Police, Borough Council, Housing Providers, and other relevant services.
- To complete a holistic needs assessment, making use of assertive outreach work to achieve the initial client contact, if required.
- Assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
- Work with multi agency partners to support the delivery of co-ordinated services for clients.
- Work with partners both internal and external to ensure that vulnerable person’s cases are monitored and risks evaluated in order to promote community safety.
- work with the third sector and signpost to other agencies as necessary to improve the health and wellbeing of clients referred to the service.
- Proactively recognise the indicators of deteriorating mental health and provide crisis/safeguarding intervention support
- Support clients to build their emotional resilience and crisis planning tools, promoting self-management through building an understanding of their own triggers/coping strategies.

The role

About our Complex Needs & Community Services
The successful candidate will manage and develop the Complex Needs and Community Teams at Hertfordshire Mind Network. The Services Manager will be responsible for leading a variety of services currently including the Flourish (asylum seekers and refugees) contract and the Bounce Back (hospital services) contracts. As the services grow and develop, other services/projects may sit under this post.
About the role
The role requires travel across Hertfordshire to our Wellbeing centres, community partnerships and external organisations and is responsible for a team of staff and volunteers who work within Complex Needs and Community teams across Hertfordshire county.
As a Services Manager you will be an integral part of the Senior Management Team (SMT) and work in partnership with Complex Needs Team Leaders and the other Service Managers across the Charity. - Support the COO with effective running of the complex needs and community services.

- Ensure the embedded influence & participation of lived experience and service users and to actively seek the views and feedback from clients in order to shape and improve delivery.

- Embed HMN Safeguarding policies and be a strong safeguarding level 3 champion and manage/support referrals.
- Ensure all policies and procedures are current, understood by staff and that they are working within these policies and procedures.

- Process referrals and manage and prioritise all administration tasks associated with the service, including Payroll and expenses submissions.
- Be part of the Senior Management on call rota (out of hours), attend SMT meetings as required and support the work of the wider SMT.

We’re looking for a L&D Training Facilitator to join our team in Vauxhall (This is a primarily remote role, with occasional travel required to visit services/sites as needed.), working part-time (28 hours per week over 4 days). This is a hands-on facilitation role where you’ll deliver engaging, inclusive and consistent learning across the organisation—helping staff build the therapeutic knowledge and practical skills they need to support clients effectively.

About The Role
We are looking for a Peer Coordinator to join our Integrated Substance Misuse Service, working within an integrated healthcare framework to support adults aged 18 and over who use drugs and/or alcohol.
The Integrated Substance Misuse Service provides a combination of clinical, psychosocial substance misuse, health and wellbeing interventions and accredited treatment programmes for people entering prison who have been identified as having an alcohol or drug problem.
As a Peer Coordinator, you will play an important role in supporting the service and the prison to develop a strong peer support network, helping service users to take greater responsibility for their health and wellbeing.
What You'll Be Doing
- As a Peer Coordinator, you will:
- Champion the Integrated Substance Misuse Service within the prison.
- Support and work alongside the wider Substance Misuse Services team.
- Train and support service users to become Peer Supporters.
