The Autism Practitioner will work with people across Swindon with Autism and mild to moderate mental health concerns at pre/post assessment stage. Your role is all about providing a collaborative and tailored approach.

Your role:

• Provide excellent delivery, teaching and course leadership to learners across TreeHouse School.

Centrepoint, the UK’s leading youth homelessness charity, is looking for experienced Support and Progression Officers to join our Berwick Street and Westminster Housing Support services based in Westminster. We help vulnerable young people by giving them the practical and emotional support they need to find a job and live independently. Together with our partners, we support thousands of homeless young people each year.

Berwick Street service is a high support needs 24hours service for people aged 16 to 25yrs. Many clients will be care leavers or have experienced homelessness.

Our Westminster services is a low support service at the final stage of the Westminster’s Pathway. The service aim is to improve the life chances of single homeless or vulnerably housed young people, supporting them to develop independent living skills in supported temporary accommodation. The service works with young people in shared flats.

In this role you will:

• Work face to face with young people and support them to achieve their goals and independence.
• Work independently and as part of a team to ensure the highest possible standards of accommodation
• Undertake initial and ongoing needs/risk assessments for young people
• Manage your own caseload of young people, providing holistic support in line with their needs and aspirations
• Liaise with multiple agencies to ensure effective communication across the service and externally including social work teams, YOS, health, and other statutory services and voluntary agencies
• Make internal and external referrals to a wide range of statutory and voluntary agencies for specialist support and Employment and Training opportunities

About Us

Centrepoint, the UK’s leading youth homelessness charity is looking for dynamic, passionate and motivated Support and Progression Officers that can provide quality support to our young people to join our Barnet Foyer service in Cricklewood. We help young people by giving them the practical and emotional support they need to find a job and live independently. Together with our partners, we support thousands of homeless young people each year.

The service accommodates 24 single young people, usually, between the ages of 16 and 21. Young people stay at the service for up to 2 years before moving to more permanent accommodation. Barnet Service is quite a large building and offers a number of additional resources including a fully equipped gym. Barnet is a diversified environment with young people and colleagues from diverse ethnic backgrounds, and the service and other stakeholders celebrate and maximize the opportunities that diversity present.

This role involves working 37.5 hours per week on a rota Monday to Friday (not including Bank Holidays). All staff must be available to work the hours of 8:00 am to 4:00 pm and 1:00 pm to 9:00 pm. You will be working with a team of three other Support and Progression Officers and the manager. Two staff members work together for each shift.

In this role you will:

JOB PURPOSE

To inspire people with mental health conditions to progress, and to gain and sustain paid employment.  Some Employment Specialists will support clients with common (mild to moderate) mental health conditions (e.g. anxiety, depression), whilst others will support clients with more severe and enduring mental illness (e.g. schizophrenia, bipolar, personality disorders).

Depending on what clients are right for you to work with, you will either work across the local NHS primary or secondary care network alongside NHS clinicians to secure client referrals.

To work with clients with more serious mental illness, you will need to be comfortable supporting people who have experienced / are experiencing serious mental health challenges, as well as possibly other serious difficulties in their lives.  Although you will receive full training and support, it’s important you are aware of the potential impact this may have on your own mental wellbeing.

All Employment Specialists will follow the 8 principles of IPS, adhere to the IPS Fidelity Scale and achieve monthly targets and KPIs with the required administration and compliance.

Responsibilities and Duties

1. Engage a caseload of clients with mental health problems and establish trusting, collaborative relationships to support them into employment in line with contract targets and IPS fidelity.

2. Assess clients’ employment support needs; implementing and adjusting employment plans as necessary to support each client’s desired outcomes along IPS requirements.

3. Develop and deliver a range of practical services to meet clients’ needs including career guidance, job searching, CV preparation, interview skills, individual coping techniques or work coaching.

4. Build a constant flow of referrals to ensure a dynamic caseload.

5. Conduct weekly employer engagement activity in line with IPS Fidelity.

6. Understand the complex issues a client may face, recognise holistic support needs and work in conjunction with clinical staff.

7. Facilitate access to expert financial advice on welfare benefits and ‘access to work’ resources.

8. Provide support and reasonable adjustments if required, to employed clients to support them to stay and progress in work.

9. As requested, attend clinical team meetings to provide advice and information on employment and IPS.

10. Collaborate with community partners to raise awareness of employment and mental health issues and promote access to the service.

11. Maintain accurate and up to date records of activity and outcomes in line with service requirements, ensuring the IT database is up to date and paperwork compliant.

12. Update and maintain NHS database (IAPTus).

13. Receive regular supervision and training to meet individual, team and organization’s needs.

14. Contribute to the development of a service that is locally responsive and supports minority/disadvantaged communities.

About this key role
Working as part of a multi-agency team, we will count on you to lead by example, be the day-to-day expert within the team, and be a positive role model to the team of caseworkers.  Using a strength-based approach, you will provide support, advice and advocacy for young people and their families regarding accommodation, working through person-centred support plans designed to promote choice and control for young people.  You will ensure, on a day-to-day basis, that all monitoring information and evidence is being recorded and collated correctly, and that you promote inter-agency collaboration in the assessment and planning process and include appropriate agencies in the delivery of the service.

Since 2003, we have been harnessing the power of skiing & snowboarding to break down barriers, broaden horizons and address the biggest social issues facing underserved young people today.

Our accredited programmes are unique and attractive to young people who are less likely to engage with mainstream provision. Taking them from beginner to qualified snowsports instructors in just one year with volunteering and apprenticeship progression routes available at the end. Alongside learning to ski or snowboard, young people also have access to wellbeing support with the emphasis on improving young people’s mental health and wellbeing.

This is a really exciting opportunity to play a key role in Snow Camp’s development in London, with programmes running from The Snow Centre, Hemel Hempsted. So, if you love working with young people and have solid experience delivering youth programmes, combined with a passion for snowsports – and if you are looking for a challenging and rewarding role working with a committed team of staff at Snow Camp and some amazing young people – this job could be the job for you!

Experience of working successfully with underserved young people in a range of settings will be essential to the post. Furthermore, experience within a snowsports field and passion for supporting young people generally will be a huge advantage!

Prospectus is proud to be working with Stop Domestic Abuse to recruit a part-time Refuge Worker. Stop Domestic Abuse support all those affected by domestic abuse by providing innovative and unique services. They ensure that victims are supported to recover, and perpetrators are supported to change their behaviour.

This Refuge Worker role is on a permanent contract, part time basis (22 hours a week) and will be based in East Hampshire. 

Refuge workers will provide safety planning, support, advice, and assistance to children who have experienced domestic violence and abuse and are staying in the refuge. The work will focus on preventing domestic abuse by providing support and facilitating safe space for children and young people and peer support networks in the refuge and supporting children and young people to keep safe in/maintain their refuge accommodation.

Additionally, you will establish and maintain links with statutory and voluntary agencies; to promote and support the welfare of families who have experienced domestic abuse. Form and maintain relationships with local specialist domestic abuse services, and finally, ensure that child welfare is ensured in accordance with Stop Domestic Abuse’s Protection from Abuse Policy.

This role includes the opportunity to be on call out of office hours as this is a 24-hour, 7 day a week service running outside of usual service hours.  The on-call out of hours service may involve the following duties and responsibilities, this is not an exhaustive list and is subject to change to ensure contract and service delivery requirements are met by the organisation and to provide a high-quality service to people affected by domestic abuse. Stop Domestic Abuse is committed to running a 24-hour service to people affected by domestic abuse that is accessible, supportive and welcoming.

BACKGROUND: 
For many families there are issues that precede a young person’s gang involvement. Personal circumstances typically involve poverty, unsuitable housing, separation, family conflict, domestic abuse, substance misuse, mental health and imprisonment.

Gang involvement increases the risk of violence and victimisation to the young person, their friends and their family, therefore, it essential when supporting young people to exit gangs we adopt a ‘whole family’ approach.

A ‘whole family’ approach involves working with the family to assess risk, ensuring all family members feel safe, mentoring young people who are involved in or at risk of gang and serious youth violence and offering holistic support to enable the family to achieve their potential and raise aspirations.

Without the right support, families can become trapped in a cycle of homelessness, crime, unemployment and poverty that is passed on from generation to generation.

St Giles Trust solution to this stark reality is our Children & Families Service. The C&F team offer an intensive and holistic approach that seeks to understand the complex and inter-related family issues behind poverty in order to resolve them. Children & Families Caseworkers break the cycle, unlocking chaotic families’ potential, and empowering them to raise themselves out of poverty.

Key Deliverables:

• To develop and maintain relationships with other agencies, ensuring a steady flow of referrals to the project; in particular schools, PRU’s, Youth Offending Services and Children’s Social Care. 
• To support families using a  1:1 tailored and holistic practical & emotional support to improve mental health & wellbeing, housing, finances, living environment and increase their readiness to engage in training and employment 
• To complete initial needs assessments with families and support them to develop individual action plans and regularly review to assess progress 
• To promote inter-agency collaboration in the assessment and planning process and to include involved agencies in the delivery of the services
• To carry out and review thorough risk management plans with families ensuring each family member has an age-appropriate safety plan
• Provide a client led support service that responds to individual’s needs. Key elements will include supporting with housing, finances, schooling, parenting, health and wellbeing, ETE and criminal justice
• To support families who are affected by domestic violence, this will include working with victims and perpetrators
• To deliver the service in a timely and targeted manner, working through identified issues on the action plan, promoting case closure as an achievable outcome
• To close cases effectively and positively, identifying a survival plan for the client that can be used for ongoing support and agencies that can be used if serious problems develop in the future
• To support the Team Leader  with other tasks as and when required
• To carry out any other similar duties as required, including providing support to other St Giles Trust projects on occasion. 

What we are looking for from you – Person Specification

Experience:

• Experience of working with socially excluded children, young people and adults. (A) 
• Experience of providing advice and guidance and/or mentoring to Families.  (A) 
• Experience in negotiating and networking with partner agencies to establish links to further the aims of a project. 
• Experience of successfully engaging with challenging families and young people, for example people those who have complex needs, people who are reluctant to discuss their needs, and people who are angry and confused. 

Qualifications:

• Mentoring Level 3 qualification (or be willing to work towards this). (A) 
• Any other level 3 and up qualifications.

Knowledge: 

• Knowledge of anti-discriminatory working practices, and the implications of both within the working environment. 
• Practical application of diversity awareness and unconscious bias in employment.(A) 
• Knowledge of and commitment to safeguarding practices and policies, and an ability to promote safeguarding among vulnerable clients and colleagues. (A) 

Skills & Abilities:

• Ability to be a flexible and co-operative member of a team. 
• Ability to demonstrate knowledge and awareness of the issues faced by our client group, in particular barriers faced by people with lived in experience. (A) 
• Ability to work alongside other professionals, professionally representing St Giles Trust
• The ability to use and develop monitoring systems to record all aspects of the project, and the ability to write case studies on sessions with young people.
• Ability to calculate risk and implement safety procedures when engaging with clients. 
• An ability to recognise the signs of child sexual and criminal exploitation (CSE and CCE) and, following safeguarding policies and measures, take appropriate action. 
• Strong IT skills including proven experience of using Word, PowerPoint, Excel and Outlook in a similar work environment.  Ability to work with case database (e.g. Inform) on a day-to-day basis.

Prospectus is proud to be working with Stop Domestic Abuse to recruit two full-time Refuge Workers. Stop Domestic Abuse support all those affected by domestic abuse by providing innovative and unique services. They ensure that victims are supported to recover, and perpetrators are supported to change their behaviour.

Refuge workers will provide safety planning, support, advice, and assistance to children who have experienced domestic violence and abuse and are staying in the refuge. The work will focus on preventing domestic abuse by providing support and facilitating safe space for children and young people and peer support networks in the refuge and supporting children and young people to keep safe in/maintain their refuge accommodation.
 
Additionally, you will establish and maintain links with statutory and voluntary agencies; to promote and support the welfare of families who have experienced domestic abuse. Form and maintain relationships with local specialist domestic abuse services, and finally, ensure that child welfare is ensured in accordance with Stop Domestic Abuse’s Protection from Abuse Policy.

This role includes the opportunity to be on call out of office hours as this is a 24-hour, 7 day a week service running outside of usual service hours.  The on-call out of hours service may involve the following duties and responsibilities, this is not an exhaustive list and is subject to change to ensure contract and service delivery requirements are met by the organisation and to provide a high-quality service to people affected by domestic abuse. Stop Domestic Abuse is committed to running a 24-hour service to people affected by domestic abuse that is accessible, supportive and welcoming.

You will be working as part of the Older Looked-after Young People (OLYP) team in Horsham looking after young people aged 16 -17 in a warm and homely environment. Young people come to us from a variety of backgrounds and have various and often complex needs. The team is made up of passionate and caring individuals, who work together to provide holistic support to the young people in our care, 24 hours a day, seven days a week. Our specialist supported accommodation is a safe and secure environment where young people are supported to acquire the necessary skills in preparation for living independently and can safely explore their increased freedom and develop the responsibilities associated with adulthood.

As a Supported Housing Support Worker you will hold a caseload of residents, working with them on a one-to-one basis with also facilitating group work sessions. You will identify their needs by listening to them, with employing psychologically informed approaches to enable residents to make better choices and take responsibility for their actions. You will work closely with other services such as, Social Services, Police, Probation, education providers and local organisations. Our projects already have excellent working relationships with these services, and your role will build on these. You will be helping them to improve their practical living skills such as cleaning, budgeting, managing behaviour, seeking, and securing employment and making wise decisions. You will help to contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations. 

Based on site, this role is on a seven-day rolling rota, including evenings, weekends and bank holidays.

About this key role

Our successful candidate will lead by example, be the day-to-day expert within the team and be a positive role model to the team of caseworkers, escalating any concerns to the Team Leader or Service Manager as required.  Working as part of a multi-disciplinary team, we will rely on you to provide support, advice and advocacy for young people and their families regarding accommodation, and to produce support and risk management plans based on assessments, while also delivering a holistic support service that will include practical help such as social and housing support, ETE options, benefits work, debt advice, liaising with utilities, appearing in court, and cleaning.

We will also rely on you to ensure, on a daily basis, that all monitoring information and evidence is being recorded and collated in line with agreed processes, that you contribute to the services’ out of hours rota as required, plus work closely with the Team Leader and Service Manager to ensure the team can work effectively e.g. identifying training opportunities and peer-supervision.

• Proven experience of engaging positively with vulnerable young people or other vulnerable groups with understanding and knowledge of relevant policies and legislation
• Experience of using support plans to enable young people to successfully access and sustain accommodation and support services
