Support Coordinator
This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We are looking for an innovative, passionate, and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Service based in Macclesfield.
Position: S11313 Stroke Support Coordinator
Location: Home-based – Macclesfield, East Cheshire. However, extensive travel will be required as part of this role (may include team meetings or other work-related meetings)
Salary: Circa £21, 948 per annum (FTE circa £27,400 per annum)
Hours: Part-time, 28 hours per week
Contract: Our services are contracted; we currently have funding for this contract until 31 March 2026. While continued funding beyond this date cannot be guaranteed, we have maintained regular contracts in this area for many years and are confident in the likelihood of securing future funding

The Role
Reporting to the Service Delivery Coach, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
- Supporting new stroke survivors and their carers from hospital discharge into the community.
- Provide personalised information, advice and support.
- Supporting stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
- Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
About You
You will have experience in:
- Experience/background in a caring profession, ideally supporting people with disabilities
- Excellent IT skills and an ability to maintain accurate records.
- An affinity with the values of the Association.
- A flexible approach and an ability to effectively manage a caseload.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role

About the role
We are seeking a Dual Diagnosis Worker who has a specialism in complex mental health and substance misuse to join our team in Croydon. This is a residential service which provides specialist mental health, complex needs, and dual diagnosis support to our residents. You will utilise your previous experience and knowledge, and provide specialist support to those who have been diagnosed with mental ill health and have challenges with substance use. You will deliver evidence-based interventions based on holistic assessments to support personalised recovery, integration, and promotion of independent living skills.
Within this role, you will act as a specialist for comorbidity, supporting the team to achieve positive outcomes for our residents. Some of the duties may include:
- Undertake joint holistic assessments, risk assessments, care plans, and interventions to support harm reduction and minimisation.
- Set up realistic and flexible strengths-based support plans, working closely with colleagues and support teams to support and advise on interventions and approaches to meet individual needs.
- Help reduce episodes of crisis and assist residents/participants to access services which can offer alternatives to crisis support.
- Work closely with support staff and clinical teams to support and advise them on interventions and approaches to meet resident/participant comorbid, complex mental health and substance misuse needs.
- Support multi-disciplinary teams and the wider community teams with support, advice, signposting, and move on care planning.
About you
We’re looking for someone who has specialist knowledge on how to support individuals with comorbid and complex needs, with previous experience in a similar role. We look at individual characteristics to be able to show resilience, solve challenges, and be proactive in your approach. You will have a passion for supporting people to achieve their personal goals, and achieve positive outcomes, this includes having the ability to engage with people who may be hard to reach and engage with. We further are looking for someone who has:
- An appropriate qualification which is relevant to the role which could include: National Diploma, Bachelors, or master’s in psychology, Dual Diagnosis, Substance Misuse or any relevant
- Previous experience in a similar role, providing holistic assessments and support for people experiencing mental health challenges
- Applied knowledge of the principles of risk assessment and risk management, particularly in relation to harm minimisation.
- A commitment to promoting recovery, harm reduction, and active involvement in care planning.
- Ability to apply relapse prevention models to promote sustained recovery and harm reduction.

As a Family Court IDVA you will take referrals from CAFCASS to arrange support for victims of domestic abuse entering or going through family court proceedings.
Key Responsibilities:
- Direct support to victims of domestic abuse and coercive and controlling behaviour.
- Report writing for CAFCASS and other partners.

We are seeking a passionate and proactive Family Support Coordinator to develop and deliver a comprehensive programme of support aimed at reducing inequalities and the risk of higher-level conflict and violence in communities and between children, parents and families living in Edlington, Doncaster.
In this exciting role, your focus will be on improving family dynamics, promoting safe and nurturing relationships, reducing school exclusions and preventing the need for referrals to youth justice departments. You will use your experience, knowledge and skills around positive communication, behaviour coaching and negotiation and assertiveness techniques to support parents, children and families to build resilience, increase their confidence and develop coping strategies for managing mood, stress and conflict better. Your ability to work in partnership with statutory and voluntary organisation is crucial to the success of this project.
The service will provide a programme of support for families including face-to-face befriending in the home and community, community group activities, support by phone and video as well as signposting activities. You will lead a small team of peer volunteers working and travelling across Doncaster. The service will require the use of simple evaluation tools and the collection of impact data for reporting purposes.
The successful candidate must have experience of engaging in risk reduction activities associated with conflict and violence alongside experience of working with parents within communities. You must be able to work respectfully with empathy and sensitivity. Experience in safeguarding issues and working with a diverse range of clients including isolated individuals and communities are essential.

The Voluntary, Community and Social Enterprise (VCSE) Health and Wellbeing Alliance is funded by the Department of Health and Social Care and is made up of 18 VCSE Members that represent communities who share protected characteristics or that experience health inequalities. Reporting into the Sizewell C Community Fund Project Manager, the Sizewell C Community Fund Grants Officer will work as part of the Foundation’s grants team which ensures the effective and efficient delivery of the Foundation’s grantmaking through established funding programmes.
