1. Client Support
●Carry out initial assessment of client needs to identify and plan the support needed to address issues and prevent any problems from escalating.

Person Specification

The Learning and Development Coordinator will have responsibility for the coordination and administration of a range of learning and development activities and events for the Agbero2100 London programme.
The Learning and Development Coordinator will develop robust learning and development systems and processes. As a Ubele Programme Assistant, you will be a member of the Community Wealth Building hub supporting our Programme Manager with the Agbero2100 London programme by overseeing administrative tasks, communicating with our partners ensuring resource availability for the project team from project inception to completion.
Main duties & Responsibilities
- To act as the first point of contact for the Agbero2100 London programme, responding to and redirecting enquiries as appropriate.

You will be in a specialist role designed to work with people with complex support needs, relating to mental health, multiple physical health needs, poly-substance use and offending.
As a Project / Support Worker Complex Needs, your role will be to provide support to clients to enable them to understand their support needs and to manage these effectively. You will hold a caseload of clients and be responsible for helping them to identify short term objectives, meet their goals and complete risk assessments. Black-and-Racialised groups face many challenges that inhibit their ability to thrive and deliver quality services that meet the needs of their users. These challenges include lack of core sustainable funding and limiting the capacity and financial/governance skills. We are seeking an experienced Deputy Finance Director who will play a pivotal role in establishing a Finance Centre of Excellence - providing infrastructural and financial management support to Black and Minoritised community-led organisations in 8 London boroughs. Key responsibilities
1. Manage WheelPower’s social media output
- Plan, create, write and schedule social media posts. Analyse activity and respond to comments and tagged posts
- Create paid and organic digital campaigns for social media which support events and encourage activity and engagement. Plan and design creative elements including use of images, graphics, animations and video
- Spot trends and advise on best social media content and new technologies
- Be aware and engage in social media content from other organisations that relate to disabled people and the organisations who support them. Have a good understanding of current events, sporting opportunities and re-share or comment on where appropriate

3. Marketing and communications
- Produce innovative content for the delivery of creative, successful and cost-effective marketing and media campaigns targeting online and off-line audiences.
- Target specific communications which promote our work to wheelchair users, those who support wheelchair users, disabled groups, disabled children, their families and our supporters.
- Manage and produce regular outbound communications which promote WheelPower and our partners’ events and activities.
- Maintain and develop good relationships with local and national media organisations.
- Produce high quality press releases for written media, arrange interviews with radio, and be the point of contact to organise and arrange TV media to attend our events.

About MenWalkTalk
MenWalkTalk is a registered charity in England which aims to support an improvement of mental health in men of all ages by engaging in a walking group which is proven to be beneficial to their mental and physical wellbeing. The Stigma around Men’s mental health needs to be broken. MenWalkTalk also aims to reduce social isolation for men and provide signposting to specialist support for those who may be experiencing mental health problems. We want to provide volunteer-led walking groups in every county across England and then the UK. Our volunteers have mental health first aid training as a minimum and their aims are to discuss mental health within their walking groups, in a caring, supportive, and non-judgemental manner. We currently have 22 walk-leaders and 5 campaigners, all of which are volunteers.

The Willats Trust gives grants to Christian workers to share the Good News of Jesus in some of the poorest and most marginalised communities in the UK and Ireland.
As a young man in the nineteenth century, William Willats, observed that the poorest people in the city of Bath, where he lived, didn’t go to church simply because they didn’t have suitable clothes to wear to a religious place of worship. As a Christian he felt the injustice of this and set about taking the ‘church’ to the people. We are a strong PCN leadership team consisting of a Clinical Director & Head of Business Development & Transformation who are both aspirational and transformational. We require Care Coordinators who will complement the existing team through an enhanced passion and commitment to the delivery of high-quality patient services and to the wider agenda of team working.
We have a rich diversity of PCN staff who aim to provide a personalised care approach for our patients, including Administrative Ops Team, Pharmacists, Nursing Associate, First Contact Physiotherapists, Pharmacy Technicians, Care co-ordinators, Speciality Care Coordinator, Health and Wellbeing Coaches and Social Prescribers working alongside our GPs, nurses, and extended practice teams

Your role
As Shop Manager of Scope's Ilkley shop, you’ll have autonomy to run your shop using your creativity and flair to flourish in our retail environment. Our retail roles are full of variety, and every day is different! One day you might be generating and processing stock and designing an eye-catching window display. The next day you might be recruiting and training our wonderful volunteers. Every day there will be a different challenge that you’ll find extremely rewarding. 3.5 million people in the UK have chronic kidney disease (CKD), a diagnosis that can turn your world upside down.
For almost 50 years, Kidney Care UK has been at the forefront of supporting people with kidney disease. From our early days when we campaigned to introduce donor cards in the UK, we have worked hard to support and represent the interests of everyone affected by this lifelong condition. We continue to provide practical, emotional, and financial support for individuals and their families, while also working with healthcare professionals to improve care services and campaign for change.

About the role
You will provide key inbound referral management and administrative support to the Money & Energy Advisers and within our Patient Support Services team for kidney patients struggling with their energy and water bills and other costs of living.
To raise awareness of the service and build strong relationships with people living with kidney disease and affected by kidney disease, professionals and other kidney stakeholder groups in your defined geographic region.

Key Responsibilities
• Responsible for the day-to-day administration and allocation of inbound referrals into the Money & Energy Advice service across the region, effectively managing initial contact, appointments and assistance for food / fuel vouchers and other grant service administration.
• Update and maintain manual and computerised database and case management systems and records in accordance with the service’s internal policies/procedures and organisational standards.
• Collate and supply information and correspondence to internal and external contacts in accordance with required deadlines.
• Liaise with referring agencies to improve referral information and the general referral process for patients.
• Act as a gateway to local information and access to:
◌ Kidney Care UK patient grants programme, raising awareness of the programme and supporting applications.
◌ Kidney Care UK Patient Support & Advocacy service, to ensure patients have access to reliable and accurate patient information relating to kidney disease, patient pathways, models of care, and patient choice.
◌ Kidney Care UK Counselling service and other appropriate services to meet patients’ emotional and physiological needs.
• Work closely with local Kidney Care UK Patient Support & Advocacy Officers to ensure that patients receive expert advice in all areas of need, with smooth, effective handovers ensuring that patients feel supported and part of the Kidney Care UK ‘family’ at all times.
• Maintain records on the Kidney Care UK database to support service delivery and communications with patients, and to facility effective reporting to Kidney Care UK and to our funders.
• Raise awareness of Kidney Care UK and the benefit of its full range of Patient Support Services amongst people living with disease and affected by kidney disease and professionals across the region.
• Provide insight on patients’ concerns and unmet needs to inform Kidney Care UK research, campaigns, and new service development activity.
• Work in a way that respects the personal, social, cultural and spiritual needs of the individual and maintains the confidentiality of information they may have shared with you.
