Working in partnership with Surrey and Borders Partnership NHS Foundation Trust, you will coordinate holistic support alongside clients, families, carers, clinical teams and community partners.

Location: Community-based across Guildford, Waverley and Woking, with travel across Surrey and occasional cover in North East Hampshire
Salary: £29,000 per year
Hours: 35 hours per week
Working pattern: Monday to Friday, 9am–5pm
Contract type: Permanent
Target start date: As soon as possible

Key responsibilities:

  • ·        Manage a caseload and build trusted relationships with clients, their families and carers to identify needs, barriers and goals.
  • ·        Develop and regularly review holistic support plans, using motivational interviewing and behaviour-change approaches.
  • ·        Act as the lead professional, working collaboratively with clinical teams, care coordinators, GPs and voluntary-sector partners to connect clients with appropriate services.
  • ·        Arrange and attend multidisciplinary meetings and supervision, make onward referrals and support safe transfers between services.
  • ·        Maintain accurate case records, risk assessments and service data, and contribute to monitoring, evaluation and quarterly reporting.
  • ·        Travel across the service area to provide community-based support, including occasional cover in North East Hampshire.

About the organisation

Through Community, Specialist and Outreach services, we support recovery, mental health, and wellbeing—together, every step of the way.

 About you

Essential:

  • ·        A relevant diploma or degree, such as counselling, psychology, social work, probation, mental health or Level 3 Health and Social Care, and/or at least two years’ community experience supporting adults with mental health, wellbeing or behaviour change.
  • ·        Strong knowledge and understanding of mental health issues, safeguarding, confidentiality, consent, information sharing and GDPR.
  • ·        Experience of collaborative working within multidisciplinary statutory and voluntary-sector teams.
  • ·        Effective engagement, communication and written skills, with the ability to maintain professional boundaries and accurate confidential records.
  • ·        Ability to work safely and independently in the community, manage risk and challenging behaviour, and use supervision appropriately.
  • ·        Competence in Microsoft Word, Outlook, email, basic Excel and database-based recording and reporting.
  • ·        Ability to travel across Surrey and work flexibly across the service area.

Desirable:

  • ·       Experience of motivational interviewing and person-centred behaviour-change approaches.
  • ·       Knowledge of local community, employment, technology and recovery opportunities available to people experiencing mental health challenges.

  • Pension scheme and access to Simply Health (including optical, dental, counselling sessions)
  • Birthday leave, employee discounts, and flexible working where possible
  • Supportive probation, sick pay after probation, and wellbeing/EAP resources

Safeguarding & Checks

·        This role is subject to an enhanced Disclosure and Barring Service (DBS) check, police vetting and other standard pre-employment checks.

·        A past history of drug or alcohol use or criminality will not necessarily prevent an applicant from being considered for the role.
