We have a vacancy for a Mental Health A&E Link Worker to join our team.

About the Project

Our Nightlight Service is proud to provide urgent support, crisis intervention and emotional support to people experiencing a mental health crisis in Hertfordshire as an alternative to statutory pathways.

We provide support to adults across Hertfordshire 7 days a week, 365 days a year through our helpline, crisis centres, crisis cafes and a weekend Crisis House. We aim to provide a safe space for people in crisis to talk and feel supported, be listened to without judgement, access relevant support with problem solving and facilitate access to external services when necessary.

Service Objectives

The objectives of Hertfordshire Mind Network’s (HMN) Nightlight Crisis services are:

• To improve the mental wellbeing of people experiencing a mental health crisis in Hertfordshire
• To increase early access to help for people experiencing a mental health crisis in Hertfordshire, by providing a clear and effective pathway to services provided by HMN and other third sector and statutory providers.
• To contribute to an improvement in an individual’s mental wellbeing.
• To remain a source of independent support for all clients.
• To provide an alternative pathway to people in distress and crisis, to support better outcomes and reduce the demand on statutory crisis services and emergency services.

About the Role:

The Mental Health A&E Link worker will work closely with individuals in a mental health crisis currently in A&E, by using a person-centred approach to ensure that everyone is supported with dignity and respect.

Key Responsibilities

• To provide a consistent presence in A&E for individuals with mental health needs to access, as well as partner agencies, including A&E staff, integrated discharge teams, mental health liaison teams, CGL, Bounce Back, to refer into.
• To facilitate a clear and easily accessible pathway into the wider Nightlight Crisis Alternative services for individuals, to support timely A&E discharge, as an alternative to statutory crisis intervention, or as a supplement to wider care and treatment plans.
• To provide immediate information and advice about managing wellbeing, reducing risk and onward signposting and referral into wider HMN services and VCFSE organisations.
• To undertake assessments of needs and risk, devising crisis and risk management plans, and liaising with services, to facilitate referrals daily.
• To develop sound and extensive knowledge of local resources to support in enabling appropriate referrals where necessary.
• To promote and raise the profile of Nightlight and other HMN services within the hospital to ensure greater community awareness of crisis alternative provision.
• To participate in daily check in meetings with Nightlight at 17:45 and 21:00 through Microsoft Teams, communicating appropriately and sharing information with staff following assessment and onward referral into the service.
• To develop relationships across both Bounce Back and Nightlight teams
• To ensure that a collaborative approach is used, with effective communication links developed with external professional NHS services and to work as an effective member of any multi-disciplinary team. This will require stringent relationships.
• To record accurately and timely tasks done; utilising our client management system to capture the outputs and outcomes achieved, to monitor service impact.
• To represent Herts Mind Network at external meetings, and network locally to develop contacts, services and raise Herts Mind Network’s profile.
• To encourage a culture of continuous performance improvement at both an individual and service level.
• To build a service that is flexible and adaptable to changing requirements.
• To work in partnership with Mind in West Essex and Mind in Mid Herts to ensure that the crisis alternatives pathway model is consistent across the Hertfordshire and West Essex ICS.
• To participate in training and development opportunities as agreed with your line manager necessary for the role and/or your own development as part of an agreed personal development plan.
• To contribute to reviewing the effectiveness of appropriate crisis interventions for all service users, including but not limited to, contributing to the co-development of the service by supporting service user, family, and carer involvement by obtaining feedback and supporting where necessary chances to be involved in wider service reviews.
• To act as a positive role model showing professional and caring attitudes and behaviour towards other team members, service users and carers.

What will I be doing?

The role of Community Youth Worker is, by nature, highly relational and is a position of great influence that enables you to nurture, inspire, connect and empower young people from all walks of life, so they can make the most of their lives. • Research of young people’s needs in the local community

Youth Café

The Youth Hideaway is our flagship project and is a fun open access youth café run by our team after school at our social enterprise café - The Hideaway.

As a Community Youth Worker some of your time will be spent assisting the Youth Café, Project Lead and Head of Youth Work in delivering this project and it may include:

• Helping to create a safe, fun and inspiring environment for the young people that attend
• Plan, prepare and deliver quality youth sessions
• Build healthy relationships with young people that attend the sessions, identifying and responding to the needs that may arise

Monthly Youth Hubs

Youth Hubs are fun open access youth clubs run by our youth workers in the community. These are run on a monthly basis on rotation in different areas within Guildford Borough. As a Community Youth Worker you will be responsible for planning, organising and running at least one HUB, which may include:

• Creating a safe, fun and inspiring environment for the young people that attend

• Planning, preparing and delivering quality youth club sessions

GASP Motor Project

Matrix will often work in collaboration with partner organisations and following a successful run of pilot sessions we are pleased to confirm that from September 2022 we will be extending our work with GASP Motor Project.

Working in this setting is very similar to the mentoring work that we offer in schools and you may be expected to:

• Conduct one-to-one sessions to listen to, analyse and respond to the physical, intellectual, emotional, social and spiritual needs of young people
• Manage, monitor and evaluate caseloads as part of the one-to-one support sessions
• Develop and run a variety of contact points for the young people attending the project, including group sessions
• Build good working relationships with the GASP team

Schools Work

Our work in schools is varied and you may be expected to:

• Conduct one-to-one sessions to listen to, analyse and respond to the physical, intellectual, emotional, social and spiritual needs of young people
• Manage, monitor and evaluate caseloads within your allocated school as part of the one-to-one support sessions
• Develop and run a variety of contact points and projects for young people across the school, including group sessions and drop-ins and after school projects
• Alongside the Secondary Schools Project Lead, supervise and build a team of volunteers to work within the school
• Build good working relationships with school staff, CAMHS and other relevant local services

Personal Specification

Essential

• A strong youth work background in a relevant field e.g.: youth work, teaching, social work
• Able to relate well to young people in all environments including (but not exclusively)school, 1:1, open youth clubs and partner projects
• Capable of building relationships with and liaising with other professionals
• Experience of leading workshops and / or groups sessions with young people
• Able to set appropriate boundaries with young people, in all situations
• Agree with and able to support and work within the Christian ethos of The Matrix Trust. (This job has a “genuine occupational requirement” (GOR) for the post to be filled by a Christian).

New Horizon provides a range of Youth Work opportunities for young people experiencing homelessness. One stream of work is our Jobs, Education and Training (JET) team. The JET team support with young people to support them into meaningful and appropriate employment, training opportunities, apprenticeships, volunteering, and education.

In this role, you will work with young people 1-1, holding small caseloads, as well as working with young people in groups and delivering employment or education-focussed sessions on a regular basis. The role of Triage and Early Interventions Officer (TEIO) is a fast paced one which requires the post holder to answer incoming calls and make outgoing calls to victims of crime and those accessing the service in Cumbria.

As a TEIO, you will;

• answer incoming calls, speaking to those affected by crime & manage a caseload
• make outgoing calls, assess needs and risks and action plan with callers
• work on the case management system to allocate cases, provide administration on cases and assess/ensure data quality
• liaise with other agencies where required to find out more information, give information or make referrals or give signposting information
• work with the wider team to ensure an effective, trauma informed service is given to those affected by crime

You will be a confident, compassionate and empathic person who can work well under pressure. You will be speaking to people who may be upset, angry or in emotional difficulties and will need to be composed, empathic and be able to quickly assess situations.

You will be confident in using your IT skills to work on a case management system, access phone and online systems, email and Microsoft Office programmes.

Day Care Officer – Day Care Centres 

Age UK Camden Day Care Centres are looking to recruit Day Care Officers to cover a range of hours and shifts across our two Day Care Centres. 

Our Centres provide a wide range of activities as well as being a specialist Day Care Centres caring for older people with complex needs. The Day Care Centres also provide a hot lunch and a range of special events. 

We have a dedicated, specialist team supporting people with care needs, including those living with mild - moderate dementia and complex needs.

New Horizon provides a range of Youth Work opportunities for young people experiencing homelessness. One stream of work is our Jobs, Education and Training (JET) team. The JET team support with young people to support them into meaningful and appropriate employment, training opportunities, apprenticeships, volunteering, and education.

In this role, you will work with young people 1-1, holding small caseloads, as well as working with young people in groups and delivering employment or education-focussed sessions on a regular basis.

As part of our service, we also aim to deliver a weekly group conversation class for young people looking to improve their spoken English, many of whom are young refugees and asylum seekers. While you will work with all young people in the service and support regularly with our day centre delivery, this role will focus on expanding our JET service to young refugees and asylum seekers, developing specialist networks and partnerships, and pathways to appropriate opportunities.

What will you be doing?

As a Service Manager at Hft, you’ll be supporting adults with learning disabilities to live the best life possible. You will be a role model to others through demonstrating excellent support to those we support and you will help the Regional Service Managers to coach, support and manage staff teams effectively. About Us

Centrepoint, the UK’s leading youth homelessness charity is looking for dynamic, passionate and motivated Support and Progression Officers that can provide quality support to our young people to join our Barnet Foyer service in Cricklewood. We help young people by giving them the practical and emotional support they need to find a job and live independently. Together with our partners, we support thousands of homeless young people each year.

The service accommodates 24 single young people, usually, between the ages of 16 and 21. Young people stay at the service for up to 2 years before moving to more permanent accommodation. Barnet Service is quite a large building and offers a number of additional resources including a fully equipped gym. Barnet is a diversified environment with young people and colleagues from diverse ethnic backgrounds, and the service and other stakeholders celebrate and maximize the opportunities that diversity present.

This role involves working 37.5 hours per week on a rota Monday to Friday (not including Bank Holidays). All staff must be available to work the hours of 8:00 am to 4:00 pm and 1:00 pm to 9:00 pm. You will be working with a team of three other Support and Progression Officers and the manager. Two staff members work together for each shift.

In this role you will:

• Work face to face with young people and support them to achieve their goals and independence.
• Work independently and as part of a team to ensure the highest possible standards of accommodation
• Undertake initial and ongoing needs/risk assessments for young people. Be able to identify safeguarding risks and understand how to escalate any concerns
• Manage your own caseload of young people, person centred support in line with their needs and aspirations
• Liaise with multiple agencies to ensure effective communication across the service and externally including social work teams, YOS, health and other statutory services and voluntary agencies
• Make internal and external referrals to a wide range of statutory and voluntary agencies for specialist support and Employment and Training opportunities
• Support young people to manage their rent accounts through liaison with the DWP, Social Services CenterPoint’s rent team and building their own personal budgeting skills

The Mental Health IDVA will be employed by FearFree to develop pathways and processes for the Mental Health IDVA working in tandem with staff from Devon Partnership Trust to identify both patients and staff who are currently experiencing Domestic Abuse.

The Mental Health IDVA will work within a dynamic, fast paced, crisis intervention, advocacy and support service embedded within DPT to ensure all staff are aware of Domestic Abuse, and equipped to ask the right questions and encourage both patients and staff to engage with the support the Mental Health IDVA will bring to this environment.

The post holder will assist health practitioners to respond more effectively to domestic abuse, particularly those with complex and multiple needs, and will bring specialist knowledge and skills in empowering clients to seek the right support, and move forward.

The work will include providing specialist knowledge and expertise to other health care practitioners to ensure victims of domestic abuse (and their children) are safeguarded from further harm, to provide a strong partnership approach to service provision.

Priority will be given to medium and high risk cases that are employees and patients of DPT – those who do not live in Exeter/Mid Devon area to be sign-posted to specialist support within FearFree Devon, or referred to partner NDADA if living in North Devon. To work with all victims of abuse (male or female) who are currently registered with formal DPT support.

Key elements of the role include:  

· Providing priority support to both patients and staff at the hospital.

· Delivering Domestic Abuse training to all staff at the hospital equipping them to recognise all forms of Domestic Abuse and ensuring they are confident in starting those conversations, and seeking additional support from the Mental Health IDVA.

· Risk assessing and helping to keep service users safe through safety planning.

· Enabling victims to access and engage with statutory and other services.

· Engaging with and supporting the MARAC process, to ensure that the voice of victims is heard.

· Managing a caseload of medium to high-risk victims and working proactively to support them and their families.

· Develop and maintain working relationships with additional services relevant to this field.

· Ensure services and resources are appropriate and tailored to services users needs.

· Support the relationship build between client and statutory services.

Key Responsibilities:

· Hold a caseload and provide high-quality telephone and face to face crisis intervention, information, advocacy and support to both staff and patients at the hospital who have been identified as victims of Domestic Abuse.

· Proactively assess the needs and risks of clients regularly in line with FearFree policies and procedures, having full regard for case management, MARAC and safeguarding adults and children policies and procedures.

· Complete risk and safety plans for all clients, ensuring risk management and safety is at the heart of everything we do in line with company standards.

· Ensure all records comply with compliance and quality standards and information is recorded accurately and in a timely manner

·  Engage with all relevant support agencies,  acting as the “voice” of both staff and patient victims to seek support to address specific identified barriers.

· Support all caseload to access external support and sign-post where appropriate to enable caseload to engage with other organisations who can support them on their road to recovery.

· Work proactively to raise awareness of Domestic Abuse  and upskill all staff within the hospital to be able to identify potential Domestic Abuse, and seek support from the Mental Health IDVA.

· Attend regular safeguarding and domestic abuse meetings within DPT and health related settings to discuss active cases as required by the manager.

· Proactively assess the needs and safety of any children of the client and ensure that any risks/needs identified are addressed directly with them, having full regard to Safeguarding Children’s policy and complying with the Local Safeguarding Children requirements.

· Advise clients of their rights and options for seeking help and support from other agencies, making referrals, attending appointments with them when required, co-ordinating the provision of multi-agency support where necessary, and proactively advocating to ensure barriers to accessing support and protection are reduced.

· Proactively look for and implement innovative and creative ways of engaging vulnerable and hard to reach victims, particularly those with multiple needs, and sustain the engagement.

· Participate in multi-agency conferences and meetings in respect of children and adults at risk as required, providing reports and undertaking actions as necessary.

· Work in partnership with key agencies, particularly with external support agencies, to ensure effective joint working.

· At all times protect the safety and security of service users, staff, volunteers and buildings, and the confidentiality of records and other information in line with data protection requirements.

· To be flexible with meeting clients which may involve working outside of normal working hours.

· Produce data, reports, evaluations and undertake research, as requested and directed by the Manager.

General

· Remain up to date and concordant with organisational procedures, policies and professional code of conduct, upholding standards of best practice.

Age UK Camden Day Care Centres are looking to recruit two Kitchen Assistants across our two Day Care Centres based in Kings Cross and Hampstead. (Staff are expected to be able to work at both locations as required) 

We have a dedicated, specialist team supporting people with care needs, including those living with mild - moderate dementia and complex needs.  

Our Centres provide a wide range of activities as well as being a specialist Day Care Centres caring for older people with complex needs. The Day Care Centres also provide a hot lunch which enjoyed by our clients. 

The successful candidate will: 

• Support the Chef and other Day Centre staff in food service delivery during lunch time periods 

• Support the Chef and other Day Centre staff in the preparation of food and drink 

• Be responsible for keeping kitchen areas clean, tidy and sanitized 

• Be responsible for washing up of all dirty dishes, cooking utensils and other cooking equipment 

• Supervise kitchen trainees in appropriate skills. (when applicable) 

• Ensure that the kitchen is clean, orderly and managed to the highest standards of food hygiene and safety at all times.
