The Hepatitis C Trust has developed projects nationally whereby peers use their lived experience of injecting drug use and hepatitis C, to provide education and training to service users and staff in drug and alcohol, hostel and homeless services, increasing hepatitis C awareness, access to testing and referral to treatment for people living with hepatitis C.

Working under the guidance of the Peer Programme Manager, this post will continue the development of the project, recruiting and coordinating peers across Cornwall.

Working in partnership with hepatitis C clinical teams, drug and alcohol services, homeless service providers, primary care and community pharmacies, and targeting marginalised and disadvantaged communities, the post will coordinate delivery of educational workshops, hepatitis C testing and harm reduction interventions to ensure that service users are referred and supported through hepatitis C treatment and care. 

This post will require extensive travel across the region and the post holder is required to hold a clean driving licence and have their own vehicle for work related travel.

Working under the guidance of the South West Peninsula Peer Programme Manager, the post-holder will work alongside other identified partners to help develop a community outreach liver surveillance program and carry out HCC early detection activity and interventions across Plymouth.

We are a patient-led organisation – you will be working in an environment where the patient/service user/client is placed at the centre of all that you do. This post will require extensive travel across the region and the post holder is required to hold a clean driving licence and have their own vehicle for work related travel. 

We are growing our counselling service and we will be offering more generalised counselling as well as our Self Harmony service which specialises in self-harm and self-injury reduction.

As Strategic Lead (Cancer & Inequalities) you’ll continue our work with GM Cancer to achieve a permanent reduction in inequalities and inequity within Greater Manchester, addressing the social, environmental, and economic determinants of health and wellbeing, with the aim of ensuring active VCSE participation.

This role will suit someone with a passion for improving the health and wellbeing of people, for tackling health inequalities, and who enjoys working in fastmoving and complex systems. Now, nearly half of them are - or recently have been - sleeping rough and with the numbers of young rough sleepers in the capital growing at an alarming rate, New Horizon Youth Centre has had to take decisive action to unlock support and housing for this group. The Head of Services - Rough Sleeping is a brand-new role for the organisation and will lead on developing a new internal strategy for young people facing rough sleeping, as well as holding operational oversight of our established youth-specific street outreach function and emergency accommodation hub, both of which are unique in London. You will need to be strategic in approach and an excellent relationship builder. You will act as our lead colleague on relationships with the Greater London Authority and London Councils’ rough sleeping teams, working with them to ensure our learning and insights inform policy and ultimately open up access to emergency support for those under 25. Alongside this, you will need a keen eye for operational detail, have experience of delivering homelessness services, and be passionate about piloting new solutions to prevent homelessness among young people. Felstead street is a CQC registered care home that provide care and support for 24 adults with high level mental health needs but also with complex physical health needs.

As a Waking Night Mental Health Support Worker, you will play an integral role as a part of the team by offering the continuity of care and support in every aspect of their needs in line with individual support and risk management plans, such as medication administration, emotional support and regular welfare check, personal care and so on.

Shift: 10.15pm-8.15am. Must be able to work some weekends and bank holidays. 4 working days (10 hours x4) a week, according to the rolling rota.

Buildng supportive, trusting relationships with customers and creating a positive atmosphere in the home.

About the Role:

The primary purpose of this role is to provide debt advice and money education on a case management basis and to support the provision of advice by volunteers. The post holder will be expected to manage their own cases and lead appointment sessions.

The organisation’s mission is to ‘Restore Dignity and Renew Hope’ to those in our communities brought down by poverty and debt. The job holder will play their part in bringing this vision to reality, supporting clients through what at times can be emotionally difficult situations, to get back on their feet and realise their full potential.

The role combines both technical and pastoral elements. The role will involve building a client case, untangling sometimes complex situations, liaising with creditors, providing guidance for volunteers, and discerning and communicating the appropriate choices and strategies for the client. We are looking for someone with prior debt advice experience who ideally has experience in leading and supervising volunteers.

Essential skills and experience of the Case Manager include:

• Previous experience in providing regulated debt advice on a case management basis
• Being a DRO intermediary would be an advantage
• Demonstrable experience in providing person-focused support to clients who may be vulnerable
• Excellent interpersonal skills and an ability to show empathy, compassion and patience
• Sound IT skills with a strong command of the Microsoft Office suite

About the team 

Shelter North East has been based in Newcastle for over 25 years and we provide advice across 12 local authorities We have an advice team, a legal team, support services which are made up of a Housing First project, along with a DIYSA service and hub administrators. We also have a number of national staff who support our work, such as a Community Fundraiser, Community Organiser and an Involvement Officer. Our services are provided either from our hub base or through outreach locations,  to give communities easier access to support and advice.

As an Engagement Worker, you will  be pivotal in delivering Shelter’s purpose to defend the right to a safe home by enabling those we assist to enforce their housing rights. You will be a key member of our local team, delivering intensive support to a small number of individuals with multiple disadvantage to move from homelessness to sustaining a tenancy. The vacancy

Here at Peabody, we have an opportunity for a Team Manager - Young People on a Permanent basis for two new Supported Housing Schemes in Lewisham, due to open in June 2024.

You, as the Team Manager will support a team of housing support workers to support our young customers to remain in their home and take an active part in the community.

Both schemes are part of Peabody's well established Young Persons and Complex Needs service in London and will provide temporary housing for young people who are homeless aged between 16 to 17 years. 

What we’re looking for:

• Are you experienced in providing support as a senior support worker or as a team manager to people who have experienced trauma and have complex and challenging support needs?
• Do you have any previous experience in the housing support sector and a good knowledge of statutory and community services?
• Do you have some experience of managing and motivating a team to deliver a high-quality support service?

Could you be a Caseworker for vulnerable young people? Could you give them the support and guidance they need to get back on their feet and live independently?

The young people we work with have become homeless for a number of reasons – isolation or mental ill health, abusive relationships, substance misuse, involvement in gangs/county lines - their issues are complex so you will need to be able to build strong, trusting relationships so that our customers feel safe and supported. 

We are opening two new supported housing schemes in Lewisham.  Both schemes are part of our well-established Young Persons and Complex Needs service in London and will provide temporary housing for young people aged 16-17 who are experiencing homelessness.

As a Caseworker, you will be working with a team of highly motivated, compassionate, and dedicated colleagues supporting a caseload of young customers with complex needs. You’ll need to meet each customer weekly to help them with their goals and discuss their progress. And, when someone’s ready to move on, we'll rely on you to help them access the housing register or look for suitable rented accommodation.

What we’re looking for:

You may have worked with vulnerable young people in a paid or voluntary capacity, maybe as a Youth Worker or in an education setting providing pastoral care. You’re a great relationship builder, who can create trust so that our young people feel comfortable sharing their issues with you, you listen but in a non-judgemental way and you are able to spot signs for concern and act on them.

You’ll also need to be able to work on your own, sometimes dealing with challenging situations and diffusing them to avoid escalation. Is this you?

East London Apartments provides 24-hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).

Role Purpose

RISE strives for a world where people can lead safe, purposeful lives as a result of better relationships within families and communities; where harmful behaviour is marginalised and fewer people are victims of crime. In order to create safe communities, RISE empowers people to break their cycle of harmful behaviour. RISE’s programmes challenge attitudes and facilitate long-lasting behaviour change and are driven by the belief that people can make better decisions, given the right support. RISE are specialists in designing and delivering evidence-based, transformative behaviour change courses within the criminal justice system and wider community.

With the main objectives of reducing re-offending and helping to protect the public, the RISE practitioner will deliver a range of interventions (individual and group), that RISE may design, commission or be commissioned to deliver in the future in the community.

Avon and Somerset Police has commissioned us to deliver ‘Chance to Change’, a new informal out of court resolution (OOCR) Deferred Prosecution for 18–24-year-olds in Avon and Somerset. The Support Worker is key to the successful delivery of the Deferred Prosecution Scheme, and will manage cases from the point of referral, tracking progress and compliance. The Support Worker will act as a single point of contact to secure the offender’s engagement, working alongside other partners to address the offender’s needs, with the aim of eliciting behaviour change and reducing reoffending.

Key Accountabilities

Delivery:

· Manage cases with a range of complex needs and behaviours.

· Deliver a range of short interventions which include group work, one to ones, embedding a multi-agency approach in Avon and Somerset.

· Undertake holistic assessments that identify risk and protective factors, and develop bespoke support plans.  

· Work collaboratively with the Chance to Change partnership, including attendance at multi-agency meetings where required.

· Track service user’s progress and compliance against their individual Support Plan, reporting back to the ASCEND team.

· Adhere to relevant group work programme manuals and deliver programme sessions to ensure the integrity of programme delivery, which is overseen by the Team Leader.

· Apply specialised materials to support delivery to consider diverse needs, working proactively to provide a non-discriminatory service.

· Be responsible and accountable for working with Team Leader to ensure risk is reviewed on a weekly basis.

· Use motivational interviewing techniques and strength-based approaches in building a range of therapeutic skills to engage with offenders to address their behaviour.

· Encourage and facilitate learning with participants on a group, including proactively working to elicit behavioural change and acceptance of their behaviour.

· Work closely with other professionals to ensure appropriate steps are taken to protect where there is an imminent risk to another person. Apply RISE’s escalation procedures.

· Be personally accountable for the completion of the initial needs assessment, support plans, and end of intervention reports, and ensure they are to a high standard.

As an Employment Coach with our Employcare Project you will be supporting unpaid carers living in Manchester; providing support and coaching in order for them to re-enter paid employment or voluntary roles.  The successful candidates will manage a small caseload of carers and provide them with 1:1 support, helping them overcome barriers to employment and training.  You will be responsible for supporting them through every stage of their journey, enabling participants to address their barriers and realise their aspirations.
