The Role
Reporting to the Service Delivery Coach in the East of England, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
· Supporting new stroke survivors and their carers to adjust to life after stroke following hospital discharge into the community.
· Providing personalised information, advice and support.
· Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
· Working with other health and social care professionals across the stroke pathway to ensure high quality support.
About You
You will have experience in:
· Experience/ background of providing person centred support to people with a disability or long-term health condition and their carers.
· Excellent IT skills and an ability to maintain accurate records.

· A flexible approach and an ability to effectively manage a caseload.

About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation
- Accurately record all work completed within given timeframes
To succeed as the Lead Worker, you will have:
- A strong commitment to providing a high standard of service
- Experience working with people who have rough slept or vulnerable adults and supporting them to access services
- Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently
- Good communication skills and confidence in public speaking when representing Thames Reach externally
- Experience in forming strong working relationships with partner agencies, being able to challenge them when required
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone

About the role
You will have your own caseload of families, acting as their key worker and supporting them to either resettle or prevent homelessness. Your primary focus will be on households where some tailored interventions are required around the needs of the children to stabilise their housing situation and enable the family to thrive. You’ll need to provide intensive, tailored support as part of a whole family approach both in a 1-2-1 setting and through group work. You’ll also need to regularly visits clients’ homes and schools across the city liaising with other agencies in the community to provide a co-ordinated service.

About you

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. We are looking for a Housing Mobile Night Worker to provide floating support to our teams in high support 24-hour projects as well as medium support and transitional housing units to ensure that our properties are safe, secure, and welcoming through the night. You will be working across Brighton and Hove as well as Worthing to support the static night worker during the night as well as undertaking your mobile duties.
Working on site you will be responsible for ensuring the service is safe by keeping clear records of night time activity within the building. You will regularly complete building checks and monitor CCTV upkeeping health and safety regulations in accordance with YMCA DLG's policies and procedures. You will work to prevent any anti-social behaviour from impacting our neighbours and other residents.
