Are you passionate about supporting people experiencing homelessness on their journey to recovery in Bristol?

This post is offered as a 12 month fixed term contract or secondment for internal applicants.

St Mungo’s provides support to patients experiencing homelessness in the Bristol Royal Infirmary. We’re looking for someone to join the outreach team as a dedicated worker supporting clients who attend or are being admitted into hospital.

We believe that rough sleeping is intrinsically harmful to health and this opportunity provides support for patients to move away from rough sleeping at the time they may need it most. By providing holistic, trauma informed care, and taking an assertive approach, we can support patients into safe accommodation on discharge from hospital.

You will work with patients, hospital staff, local authorities, housing providers and other organisations to improve communication, understanding, discharge practices and access to services for this often-excluded group.

In this role you will:

  • Work with clients in hospital to assess their individual need, find suitable housing options and improve care via strong advocacy.
  • Build and maintain productive and positive relationships with the hospital teams as well as external stakeholders to develop effective pathways into accommodation from hospital.
  • Deliver a recovery-focused service and achieve positive outcomes for our patients.

About you

This exciting role will suit someone with some experience of working with health services to support people with complex health and social care needs.

  • The ability to deal with sometimes complex behaviours in relation to people and the empathy to provide person centred support to people who may be in a difficult personal situation.
  • Excellent communication skills and the ability to build effective relationships with a variety of people and services to reach positive outcomes.
  • Knowledge of the services and legislative environment relating to people experiencing homelessness.
