It takes a special kind of person to be a Wellbeing Navigator with our client; someone who has patience and an empathic nature, who is reliable and dedicated and above all, has a desire to help others.

As one of their valuable Bullring Wellbeing Hub Navigators, you will be on-hand to provide advice and signposting and offer an important listening service to those in need. You’ll create and deliver a variety of informative and enjoyable sessions and events that support wellbeing and encourage people to join relevant groups. You’ll love meeting new people, providing a welcoming atmosphere for those who attend the Hub and will be able to identify if there are events / groups that would be beneficial to support visitors. You’ll be part of a small, friendly team who ensure the public have access to vital information and that it reaches the most vulnerable people, many who wouldn’t usually reach out for support or help for mental health support. They’re looking for a communicative and confident individual, who feels at ease liaising with external community partnerships. 

With experience of working in the mental health field, you’ll know how to help people be in control of their lives and will treat everyone with dignity and respect. You’ll have a good understanding of the Equality Act and will be able to demonstrate a positive attitude to those who have experienced mental health difficulties.

We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join our Stroke Recovery Services based in the Dorset East area.

There will be opportunities to hot desk within Bournemouth and Christchurch, as well as Poole NHS sites.

This is an exciting opportunity to work with stroke survivors and their families to provide Key Worker support following a stroke.

Position: S11166 Stroke Association Support Coordinator (x2 posts)

Location: Home-based, Dorset East however, Frequent travel will be required as part of this role (May include team meetings or other work-related meetings)

Hours: Part-time, 2 x 28 hours per week

Salary: Circa £21,391 per annum

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

The Stroke Association Support Coordinator will:

· Support new stroke survivors and their carers from hospital discharge into the community.

· Provide personalised information, advice and support.

· Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.

About You

The post holder will have experience/background in:

· A caring profession ideally with experience of supporting people with disabilities.

The role 

We are looking for Housing Support Workers who can provide intensive housing management & support to service users in our supported housing projects. 

You will work proactively with service users who have been homeless and have complex needs to sustain their tenancies and move on into independent living.

You will also work empathetically to empower services users, actively engaging them with the community and helping them to support each other, to develop resilience, to live independently, and to be an asset to the community. 

The right fit 

• You will have prior experience with service users who have been homeless and/or have complex needs including, Mental health, substance misuse, domestic abuse & chaotic lives etc. 

• You will also have strong ICT skills. 

• You will be competent in facilitating support sessions and devising support plans to achieve successful outcomes. These will be person centred in their approach, identifying individual needs and strengths of each Service User. 

• You will have experience in working in the housing sector 

• You will have experience of liaising with partner agencies to compliment individual support packages

This is a full-time permanent position, working Monday to Friday with additional monthly on-call duties. • Excellent communication skills and an understanding of Customer Relationship Management systems (such as Salesforce).

The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.

South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.

The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.

You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.
