Our Carer Liaison Workers work within both local NHS Trusts (BRI and Southmead Hospitals), where they make a real difference, supporting adult carers – those who care for others who can’t manage without their help, when the person they care for goes into hospital.

You would provide information, advice and support, enabling carers to navigate their way through the intricacies of the hospital journey, whilst liaising with health and social care professionals to ensure the carers voice is heard and their needs are supported, including during discharge planning.

You would also work with hospital staff to improve their knowledge and experience of supporting carers, through development work and training.

You’ll be joining the team at a pivotal time for our charity. Having recently secured our core statutory income for the next 3 to 5 years we’re now looking to use that as a launchpad to grow and develop new and existing services.

The vacancy

Could you be a Caseworker for vulnerable young people? Could you give them the support and guidance they need to get back on their feet and live independently?

The young people we work with have become homeless for a number of reasons – isolation or mental ill health, abusive relationships, substance misuse, involvement in gangs/county lines - their issues are complex so you will need to be able to build strong, trusting relationships so that our customers feel safe and supported. 

We are opening two new supported housing schemes in Lewisham.  Both schemes are part of our well-established Young Persons and Complex Needs service in London and will provide temporary housing for young people aged 16-17 who are experiencing homelessness.

A bit about the role:

As a Caseworker, you will be working with a team of highly motivated, compassionate, and dedicated colleagues supporting a caseload of young customers with complex needs. You’ll need to meet each customer weekly to help them with their goals and discuss their progress. And, when someone’s ready to move on, we'll rely on you to help them access the housing register or look for suitable rented accommodation.

What we’re looking for:

You may have worked with vulnerable young people in a paid or voluntary capacity, maybe as a Youth Worker or in an education setting providing pastoral care. You’re a great relationship builder, who can create trust so that our young people feel comfortable sharing their issues with you, you listen but in a non-judgemental way and you are able to spot signs for concern and act on them.

You’ll also need to be able to work on your own, sometimes dealing with challenging situations and diffusing them to avoid escalation. Is this you?

• work with the wider team to ensure an effective, trauma informed service is given to those affected by crime

You will be a confident, compassionate and empathic person who can work well under pressure. You will be speaking to people who may be upset, angry or in emotional difficulties and will need to be composed, empathic and be able to quickly assess situations.

You will be confident in using your IT skills to work on a case management system, access phone and online systems, email and Microsoft Office programmes.

About the Role

This role will provide individualised, one to one support to young people, aged 14-17 who are currently experiencing difficulty with their mental health and wellbeing.

This post holder will be working closely with clinical colleagues in Child and Adolescent Mental Health Services (CAMHS) team offering hybrid in person and online support. The post holder will also be responsible for the development of pathways which enables young people to access services offered by the Voluntary and Community sector.

The post holder will ensure they have a good understanding of the services on offer by the Voluntary and Community sector, and other agencies, and will work with individuals to help them access services of their choice.

About the Role

Chance UK has been delivering early-intervention services across London since 1995. Our longest running programme is our mentoring service where Mentors pick the child up from home and run a 1:1 session in the community, once a week for 2 hours. They use the solution-focused approach to work on building the child’s self-esteem and confidence, set goals and explore new places.

Aside from the 50+ young people we see every day at our day centre, New Horizon’s Outreach service also works with young people on the streets and in the community. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. The Outreach Worker will work as part of Rough Sleeping Team to deliver at least two street outreach shifts per week and transitional casework to young people facing rough sleeping. You will need to be willing to work early mornings and late evenings, to travel across London to identify young people who need our support, and to work flexibly to respond to evolving needs. Our mission is to help children and young people have a fair chance to be who they want to be. We do this by providing a safe home, increasing life skills and self-confidence, and improving emotional wellbeing and mental health.

We are looking for a Housing Mobile Night Worker to provide floating support to our teams in high support 24-hour projects as well as medium support and transitional housing units to ensure that our properties are safe, secure, and welcoming through the night. You will be working across Brighton and Hove as well as Worthing to support the static night worker during the night as well as undertaking your mobile duties.

Working on site you will be responsible for ensuring the service is safe by keeping clear records of night time activity within the building. You will regularly complete building checks and monitor CCTV upkeeping health and safety regulations in accordance with YMCA DLG's policies and procedures.  You will work to prevent any anti-social behaviour from impacting our neighbours and other residents. You will work alongside other support staff to maximise the wellbeing of residents by completing one to one interventions with young people who may be experiencing mental health crisis and/or who may need medical assistance.

This is a mobile role, requiring all applicants to have a full valid, UK Driving Licence. You will also be lone working as part of the role, so you will need to be comfortable with decision making and following procedures.

Our Mobile Night Workers work on four nights on, four nights off rota, working 8pm-7am including working over weekends and bank holidays. Although these are long shifts, you will get four days off after, which can provide a great work life balance.

You will already have clear verbal and written communication skills, good IT and keyboard skills and the ability to self-motivate. You will be an effective communicator who is able to deliver robust, clear handovers and build strong relationships with other members of your team.
