We are seeking a dedicated and compassionate Community Wellbeing Support Worker to join our Flintshire Wellbeing service. This role does not provide direct basic care but is focused on motivating and supporting individuals to identify their own goals as well as developing self-management strategies to improve their health and wellbeing and maintain their independence.

This role will ensure that guided conversations are led by the individual and that they are at the centre of everything we do.This is a new service commissioned by Flintshire County Council on behalf of health and social care partners through the Welsh Government Regional Integration Fund. The service has been commissioned to fill a gap identified within existing service provision.

The Flintshire Wellbeing service aims to support individuals for a period of up to 6 weeks following hospital discharge. We work collaboratively with individuals to help them identify their strengths, overcome limiting beliefs, and develop practical strategies for achieving their goals at home and within their community.

A key element of this role will be to work with the individual and co-design a personalised action plan. From this information, meaningful functional goals and outcomes are developed with the individual, to promote wellbeing, autonomy, independence, and choice. This approach aims to ‘enable people to be and to do what they have reason to value’

This is a great opportunity for a Waking Night Support Worker, wanting to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT). We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

You will be based at Hope House. We have two sites: Hope House and Doddsfield Rd. Hope House/ Place is a 10 bed supported housing service, providing support for 16 customers overall. The support hours for Hope House are 24 hours, 7 days a week, with one waking night.

Doddsfield Rd has 6 independent living flats with floating support.

What you'll do:

- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager.
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
- Develop and maintain links with all key agencies and service providers in the local community.
- Empower customers to ensure they receive the service and benefits they are entitled to
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team.
- Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.

Job Purpose: To support young people transitioning to the community. As part of the team, you will provide effective 24-hour support to the young people accessing our services.

ESSENTIAL CRITERIA

• Maths and English GCSE at Grade C OR an equivalent qualification
• IT Skills - ability to use email, internet and databases to electronically record support work (within SCNI we use a client recording system called OSKA but previous use of this is not essential).
• NISCC Registered (or willing to become registered if appointed) and willing to be subject to Access NI checks. (Successful applicants must commence NISCC registration process prior to employment).

• ​​​​Six months’ recent experience (either paid or voluntary) of directly supporting people with complex and multiple needs within a residential or community setting gained within the last three years. Complex needs include the following area - Homelessness Prevention, Substance Misuse, Offending, Mental Health and Young People. 
• ​​​​​​​​Experience of identifying support needs, including client specific risks and putting plans in place to support those needs.

The role 

We are looking for Housing Support Workers who can provide intensive housing management & support to service users in our supported housing projects. 

You will work proactively with service users who have been homeless and have complex needs to sustain their tenancies and move on into independent living.

You will also work empathetically to empower services users, actively engaging them with the community and helping them to support each other, to develop resilience, to live independently, and to be an asset to the community. 

The right fit 

• You will have prior experience with service users who have been homeless and/or have complex needs including, Mental health, substance misuse, domestic abuse & chaotic lives etc. 

• You will also have strong ICT skills. 

• You will be competent in facilitating support sessions and devising support plans to achieve successful outcomes. These will be person centred in their approach, identifying individual needs and strengths of each Service User.

Young Person's Night Porter - Housing

Our client has an exciting new opportunity to join their diverse and values led organisation as a Night Porter at their Young People’s housing service.

It is a busy and dynamic supported housing service for young people aged 16 – 21 years. Commissioned by Nottinghamshire County Council, they offer a high quality, strengths based support service which aims to build the skills, confidence and independence of the young people who come through their doors.

The Role

As their Night Porter, you will undertake a varied range of tasks on the night shift and not one shift is the same as you will be involved in directly responding and helping young people with any issues or concerns they may have, whilst also managing the site, conducting safety checks and operating surveillance of the building. Other duties may include but are not limited to:

• Booking residents and visitors in and out of the building and log all relevant information.
• Logging all complaints, accidents, potential risks, and incidents of anti-social behaviour in accordance with policies and procedures
• Keeping a day-to-day log of events and happenings when on duty and ensure effective handover of information to relevant staff.
• Conducting regular security checks including fire alarm, emergency lighting and fire extinguishers and report any concerns.

About you:

You will be compassionate, resilient and have the skills to de-escalate potential incidents through your fantastic communication skills. You will need to be able to confidently diffuse difficult situations which may arise as a result of tension or disagreements between young people, and be understanding of the needs of people from diverse social, cultural and racial backgrounds.

Although training will be provided, you must be comfortable with lone working and using your own initiative. It is beneficial if you have an understanding of professional boundaries working with a vulnerable client group and/or experience of working with a vulnerable client group. A basic level of literacy and numeracy is essential as well as caring for the safety and wellbeing of young people.

Working times:

The service operates 7 days a week, 365 days a year including bank holidays and weekends. Staff will be required to work 37 hours including weekends across 4 shifts. 8:45pm -7am

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.

What you'll do:

Work proactively with Slough Hostel team to handle the service caseload and support an effective team approach to meeting each customer's identified needs.
Motivate the team and champion a positive local culture within the service.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns.
Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks, such a facilitating group actives
Support customers to undertake domestic tasks, including practical assistance to ensure a high quality living environment that promotes customers' dignity, respect and allows customers to feel safe and welcome in their own home.
Participate in support planning, risk management and assessment of needs in line with support planning.
Work with customers to promote social inclusion to promote independent living, encouraging customers to maximise their skills and choices.
Involve customers in the design, development and delivery of the service through co-production.
Report and react to any observations relating to customers welfare.
Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community.
Competent use of relevant systems, company policies and frameworks.
Adhere to all the policies and procedures of Look Ahead Care & Support plus those specific to the project including any statutory requirements.
Carry out other duties commensurate with the designated role level, as may be deemed reasonable by Look Ahead including working at an alternative patch, if such a need arises.
