A small central social rights and benefits team provides targeted, discharge‑focused advice for financially vulnerable patients. Patient support delivered via the charity is designed to be clear, collaborative and patient‑centred, with charity staff working as one team to deliver high‑quality support tailored to each site.

Coordinate the collection and use of data, learning and lived experience evidence.
Develop an impact framework and reporting systems.
Produce briefings and updates for partners, funders and decision-makers.
