This is an exciting job opportunity to join a dynamic and committed team in a front-line role to support and deliver the activities and services of elop’s LGBTQ+ Children & Young People’s Service.

In this role you will have opportunity to design and deliver innovative youth support programmes, groups & other activities; provide LGBTQ+ affirmative support, guidance and mentoring on a one- to- one basis; work with our wider family work programme, support families with LGBTQ+ children and deliver work in schools to students & teaching staff.

You will have lead responsibility for our LGBTQ+ Young Peoples Service, including promotion, & administration, along with the induction, support & supervision of sessional staff & volunteers. 

Initially you will be working remotely via home-based working whilst elop relocates its offices & centre transitioning back to fulltime in-person working & service provision during 2024.  Some services are in person in East London. 

Full Time: 37 hours per week   

You must be available to work Tuesday evenings & once a month on a Sunday afternoon.  There will be other evenings and occasional other weekend working required.

Night Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community. Night Support Workers are responsible for ensuring continuity of a support service at night and for keeping customers and the building safe.

What you'll do:

Building supportive, trusting relationships with customers and creating a positive atmosphere

Involving customers in the design, development and delivery of the service
Ensuring Look Ahead Health and Safety policies and local protocols are adhered to at all times e.g. fire risk assessments, fire alarm checks etc.
Taking on shift leading responsibilities by coordinating and leading shifts in the absence of a manager
Being responsible for the handover of key information between shifts
Adhering to all other Look Ahead's policies and procedures
Engaging in learning and development activity to increase knowledge and skills
Undertaking any other duties consistent with the grade and nature of the post as assigned by the Service Manager/Deputy Manager

We are looking for a People Operations Manager to join the National Theatre People Team.

The purpose of the role

The People Operations Manager will lead and manage the People Operations function and be responsible for the design, improvement and delivery of all people processes and experiences including transactional HR processes, on-boarding.  A key part of the role is the continued development and delivery of HR functionality of NT People, the NT’s workforce system.  You will ensure a high level of customer service and work closely with the Payroll Team.

About the Project

Our Nightlight Service is proud to provide urgent support, crisis intervention and emotional support to people experiencing a mental health crisis in Hertfordshire as an alternative to statutory pathways.

We provide support to adults across Hertfordshire 7 days a week, 365 days a year through our helpline, crisis centres, crisis cafes and a weekend Crisis House. We aim to provide a safe space for people in crisis to talk and feel supported, be listened to without judgement, access relevant support with problem solving and facilitate access to external services when necessary.

Service Objectives

The objectives of Hertfordshire Mind Network’s (HMN) Nightlight Crisis services are:

• To improve the mental wellbeing of people experiencing a mental health crisis in Hertfordshire
• To increase early access to help for people experiencing a mental health crisis in Hertfordshire, by providing a clear and effective pathway to services provided by HMN and other third sector and statutory providers.
• To contribute to an improvement in an individual’s mental wellbeing.
• To remain a source of independent support for all clients.
• To provide an alternative pathway to people in distress and crisis, to support better outcomes and reduce the demand on statutory crisis services and emergency services.

About the Role:

The Mental Health A&E Link worker will work closely with individuals in a mental health crisis currently in A&E, by using a person-centred approach to ensure that everyone is supported with dignity and respect.

Key Responsibilities

• To provide a consistent presence in A&E for individuals with mental health needs to access, as well as partner agencies, including A&E staff, integrated discharge teams, mental health liaison teams, CGL, Bounce Back, to refer into.
• To facilitate a clear and easily accessible pathway into the wider Nightlight Crisis Alternative services for individuals, to support timely A&E discharge, as an alternative to statutory crisis intervention, or as a supplement to wider care and treatment plans.
• To provide immediate information and advice about managing wellbeing, reducing risk and onward signposting and referral into wider HMN services and VCFSE organisations.
• To undertake assessments of needs and risk, devising crisis and risk management plans, and liaising with services, to facilitate referrals daily.
• To develop sound and extensive knowledge of local resources to support in enabling appropriate referrals where necessary.
• To promote and raise the profile of Nightlight and other HMN services within the hospital to ensure greater community awareness of crisis alternative provision.
• To participate in daily check in meetings with Nightlight at 17:45 and 21:00 through Microsoft Teams, communicating appropriately and sharing information with staff following assessment and onward referral into the service.
• To develop relationships across both Bounce Back and Nightlight teams
• To ensure that a collaborative approach is used, with effective communication links developed with external professional NHS services and to work as an effective member of any multi-disciplinary team. This will require stringent relationships.
• To record accurately and timely tasks done; utilising our client management system to capture the outputs and outcomes achieved, to monitor service impact.
• To represent Herts Mind Network at external meetings, and network locally to develop contacts, services and raise Herts Mind Network’s profile.

We have a vacancy for a Hospital Mental Health Outreach Worker to join our team. This is a full-time position.

About the Project

Herts Mind Network (HMN) has formed a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.

We have two existing arms of the project – one based at Kingfisher Court and Albany Lodge facilitating discharge to the community from inpatient settings, and the other based at Watford General Hospital & Lister Hospital supporting those who have attended A&E in mental health crisis and have a dual diagnosis.

This is a new arm of the project focused on serious violence, with one worker based at Watford General Hospital and one worker based at Lister Hospital working in A&E departments and wards facilitating discharge for people aged 16 - 25 who have been admitted to hospital due to being a victim of serious violence.

About the Role:

The aims of the Bounce Back service are: to provide up to 16 sessions of emotional and practical support, advice and information to patients. We will visit patients on the ward and continue the sessions in the local community and in patients homes once discharged. We will facilitate a smooth and timely hospital discharge and focus on re-integration back into the local community.

The Bounce Back Worker will work with the HMN Community Outreach Teams and the hospital staff to ensure patients are offered advice, information, and holistic support. The Bounce Back worker will use a recovery orientated approach working with patients on the ward, and in the local community, including in people’s homes.

The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.

Key Responsibilities

• Provide practical and emotional support to encourage patients to develop their independence within their local community.
• Facilitate a smooth discharge from hospital back to the community.
• Have a solid understanding on the impact of serious violence on an individual’s mental health.
• Have a working knowledge on the areas of serious violence including knife crime, cuckooing and county lines.
• Monitor outcomes and evaluate the service.
• Ensure that the safety and wellbeing of patients using the service is monitored and reviewed regularly.
• Embed a person centred and recovery orientated approach in all aspects of the roles and responsibilities.

We currently have an immediate excellent opportunity for flexible and proactive individual to join our Nightingale Home from Hospital service, which provides support to older people over the age of 65 living in the London Borough of Richmond upon Thames on their return home from hospital or when they have become unwell in the community.

The team is involved in organising and delivering tasks which facilitate a speedy hospital discharge / make the home safer to avoid readmission and avoid admission in the first place. This includes preparing the home for discharge; fitting keysafes; moving furniture; shopping; arranging travel and providing advice.

The role will involve:

- Managing a caseload of clients.

- Developing a holistic plan of support, and directly delivering aspects of it.

- Liaising with hospitals, social workers, volunteers, family members, carers and other voluntary sector organisations and services.

- Administration related to the delivery of the service.

We are looking for an individual with experience in a health or social care related field, has an understanding of the challenges facing older people upon leaving hospital, has access to their own transport and is able to undertake physical tasks associated with preparing the home for discharge. East London Apartments provides 24 hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).

6 week rolling rota with early, late and some weekend shifts.

Role responsibilities 

As the Initial Support Lead, you will: 

• work innovatively to shape the concept of an Initial Support Pathway, with a view to assisting agencies in identifying, safeguarding, and supporting potential adult victims. This may require the delivery of awareness raising sessions, whilst also working closely with First Responders to tactically advise in a manner that secures the best outcome for victims. 

• lead on the provision of direct support to potential victims of trafficking, ensuring they receive independent advocacy, helping them navigate services, benefits, and the advice they need.

The Initial Support Pathway will provide short-term support to adults who have experienced exploitation. The pathway initially focuses on the individual’s basic needs such as safety, food, clothing, accommodation, subsistence, and access to medical services. The project then considers each individual’s rights and entitlement to support as a potential victim of modern slavery. The key principle of the project is informed choice for the individual. Although the initial support is short and temporary, the long-term impacts for victims can be significant. 

Essential role requirements 

• Experience in health, social work, or working with vulnerable adults. 

• Knowledge of the impact of modern slavery and human trafficking on an individual. 

• A good understanding and awareness of the rights and entitlements (including immigration, housing, and benefits legislation) of migrants, trafficked persons, asylum seekers and refugees. 

• Able to work autonomously and take initiative working under pressure. 

• To provide trauma informed care.

• Experience of developing service delivery including partnership engagement. 

• Take a lead approach in multi-disciplinary settings ensuring best outcomes for individuals. 

• Proven experience of carrying out needs and/or risk assessments and the support planning process. 

• An empathetic and compassionate outlook, whilst being able to maintain professional boundaries with those you work with.

• Strong communication skills with the ability to communicate with a range of audiences. 

• Travel across Derby and Derbyshire is key to this role. The candidate will need to have a vehicle and valid driving licence. Mileage costs to be reimbursed.   

• Good people skills especially with those whom English is not the first language. 

• Ability to communicate with individuals via interpreters.

• Discretion and work in accordance with the Data Protection Act. 

• Some occasional weekend and evening work may be required.

Desirable role requirements 

• Experience of adult safeguarding would be advantageous. 

• Good understanding of the trafficking care standards and how they underpin practice. 

• Awareness of emerging themes in the Modern Slavery sector

We have an exciting opportunity here at Barnardo's for experienced, enthusiastic and assertive Family Support Workers to join a brand-new team who will deliver the In Reach Hospital Emergency Department Family Support Programme.

The role is full time , 37 hours per week, permanent fixed term funded until March 2025 and will be based at the North Middlesex University Hospital, The entrance to the A&E department is in Bridport Road, Edmonton, London, N18 1QX.

Successful candidates will work in partnership with the clinical team to facilitate discharge of children aged 0-12 years from the Emergency Department where the clinical presentation is not acute and can be managed at home by local services.  

“This role requires workers to have experience of holistic ways of working with families and young people. Including assessment, support, signposting and making referrals to appropriate services where needed. The successful applicant will be able to work as part of a team as well as on their own initiative and manage the pressures of working in both a busy emergency department and the community”

Some but not all of the key responsibilities are:

• Initial face-to face conversation or triage call to identify presenting issues and the level of intervention required.
• Delivering support flexibly and tailored to the needs of the family unit including telephone, virtual and face-to-face support and home visits where necessary. Contact with families could be a one-off or over weekly sessions based on level of need targeted. 
• Signposting to partner agencies, based on needs identified.
• Supporting families to navigate their way through referral pathways and enable them to access universal & targeted services.
• Where required advocating on behalf of families, supporting parents and carers to communicate with professional/Services.
• Supporting families to access other local services.

Ideal Candidates must have's

• Experience of working with families with complex needs and the ability to build and maintain rapport through effective communication and listening skills, using a whole family approach.
• Ability to be clear and realistic about what support the service and you can offer to families.
• Ability to maintain a visible presence and sustain relationships with clinical staff, being mindful of the various working patterns/ professionals within the department.  
• Ability to navigate a high pressure, fast paced acute healthcare setting. 
• Competent IT skills to record and maintain accurate data. 
• A basic understanding of Public Health, health inequalities and wider determinants that underpin health and wellbeing.

More about the Programme

The In Reach Hospital Emergency Department Family Support Programme model offers a robust way of supporting often vulnerable families to uncover and address unmet need. Through identifying the underlying factors causing the clinical presentation and attendance, and addressing holistic needs of the family unit, we hope activity to unscheduled, primary and emergency care is dramatically and sustainably reduced.

The model is based on Family Support Workers based within Children's Emergency Departments, offering holistic, family-oriented support in a flexible manner either in-person, telephone- based and home visits to be able to address the underlying causes of repeated / avoidable ED attendances.

*Although this contract has a permanent status, please be aware that this post is subject to funding currently to March 2025 and therefore should this funding not be extended further, you may be subject to a redundancy consultation or a TUPE arrangement.

** Please note the hours for this role will be required over a 5-day week and may be done in longer day shifts or with set times i.e. 8am to 4pm (to suit Emergency Department and community/family needs). The role can adopt flexible working arrangements to meet service and personal need but will NOT provide 24 hour provision in the ED settings. This can be discussed further at interview.
