About The Role
We are searching for an approachable, and compassionate Individual to join us as a community-based Dementia Adviser covering Central / West - North Wales. The successful individual will join our supportive and compassionate team, focusing on supporting people affected by dementia, as part of the North Wales Memory Support Pathway.
 
Hybrid with travel covering - Central and West North Wales

We provide advice and guidance to people affected by dementia via face to face, online and telephone appointments; passionately promoting their independence, improving sense of well-being, putting people in more control of their lives. This role also involves building connections with other local services to help people find all of the support and activity available to them.

What you will do:

- Offer a vital, compassionate advice service to support and guide those affected by dementia - in person and by phone.
- Manage referrals, assist clients in assessing their information and support needs in a person-centred manner.
- Problem solve – helping people find ways to better live around symptoms and challenges that come from their dementia.
- Provide knowledgeable and trusted information regarding services and benefits available, assisting people in accessing them.
- Develop a proactive approach in reaching people with dementia and carers who may not otherwise access our services.

About you:

- Ability to update patient or client databased and IT skills to use Teams, Zoom emails
- Have a strong understanding of supporting people who have additional support requirements
- Be able to manage time and workload effectively 
- Possess varied communication approaches and adapt language and style to a wide variety of audiences
- Have resilience to manage and support challenging and potentially emotional situations.
- Live within, or in commutable distance from Central and West North Wales
