Outreach Advice Worker Jobs in Home Based

This is a demanding, yet hugely fulfilling new post in which the post holder will have overall responsibilty for the management and day to day operations of our new mobile support van. A large proportion of the post holder's time will be spent taking the van along with a team of volunteers on outreach to where there are survivors of sex trafficking. Our immediate priority area is Bedford. This will involve driving to sites of sexual exploitation and trafficking, such as the streets, properties used as brothels, airports and other places, as necessary. The post holder will lead outreach sessions supported by trained volunteers, in partnership with Bedfordshire Police and UK Border Force. The post holder will be a part of Azalea's frontline team (we call this Encompass) and will work with them to offer survivors of sex trafficking crisis intervention and ongoing support. The nature of this form of outreach will require the post holder to work flexible and unsociable hours (37.5 hours per week), however Azalea offers a generous annual leave allowance. Since this is a new role, the post holder will be required to use their initiative and draw on the knowledge and experience of the team to develop a full-time operational plan for the support van.

About the Role:  

Kinship is the leading kinship care charity in England and Wales. We work with all kinship carers – the grandparents, siblings, aunts, uncles, and family friends who are caring for children when their parents can’t.  

We’re seeking an experienced Family Worker who is passionate about supporting kinship carers to get the right support at the right time. We are looking for someone who has the interpersonal and partnership skills to perform their role sensitively and creatively while also meeting reporting and impact requirements. The successful candidate will work collaboratively with East Sussex County Council and will share their ambition to support families to keep their children safe through support, advice and guidance.

You’ll be delivering our Kinship Connected programme offering support to special guardians and other kinship carers, providing emotional and practical support to carers in their homes and in the community over a six-month intervention cycle. You’ll also connect them with other kinship carers through peer support groups, as well as supporting to unlock community assets.   

This is a role where you’ll need to have strong boundaries and personal resilience – which we’ll also support through development and reflective practice. 

Key responsibilities include:

• Practical and emotional support to kinship carers virtually or in-person in their home or community.
• Signposting or referring to relevant national and local services.
• Liaising with other professionals and organisations.
• Attending professional meetings when the carer needs extra support (e.g. CIN. CP, family group conference, school meetings).
• Making referrals to other Kinship services such as Advice, Someone Like Me, Peer-to-Peer.

Using a gender and trauma-informed approach, the Outreach & Engagement Worker will deliver 2-4 daytime outreach sessions per week in Manchester alongside partners, colleagues and volunteers. They will also attend the Street Engagement Hub weekly to provide specialist support to women attending this multi-agency hub. About the Service

‘Flourish’ is a highly flexible and responsive outreach service developed to meet the needs of refugees and asylum seekers in Hertfordshire. The aim of the service will be to support refugees and asylum seekers in the community with multiple needs who have mental health support as a primary need. 

Purpose of Post

• To provide advice, information, onward referral and holistic support to clients with mental ill health.
• Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.

• To remain a source of independent support for all clients.

We are looking for a Support Worker to work as part of a service that provides late-night and early-morning outreach for women who are sex working and supports women who are homeless or at risk of homelessness.

Position: Centre Support and Outreach Worker

Salary: £26,800 - 30,800

Hours: Full time, 36 hours per week (Working pattern includes 1 outreach shift per week, which

About the role

The Post Holder will work within Spires’ Women’s Service. This service is comprised of:

A dedicated nighttime outreach service to support women who are involved in street-based sex working. This includes follow up work, casework, and multi-agency working.

Centre-based support for women with complex needs who are homeless or at risk of homelessness. This also includes casework and supporting Spires’ Housing and Homelessness team.

Key responsibilities will include:

· Supporting a caseload of clients

· Completing one nighttime outreach shift a week (this is carried out in the Spires car)

· Supporting in the running of Spires’ women’s drop in services

· Supporting Spires’ Housing and Homelessness service where required

· Representing Spires to external stakeholders at multi-agency meetings

· Ensuring detailed and accurate records are kept of all client work.

About you

You will have previous experience of working with people exhibiting complex needs and people experiencing homelessness. Ideally you will have experiencing supporting women with some of the following support needs: substance addiction, mental health needs, domestic violence, no recourse to public funds and sex working.

You will also bring with you the following skills and experience:

· Experience of challenging appropriately and maintaining clear and appropriate boundaries.

· Networking and effectively liaising with specialist service providers or agencies in order to establish and/or improve access to services for clients.

· Sound understanding of the issues faced by women and other individuals involved in sex work.

· Understanding of the legislation around supporting the victims of Domestic Violence.

· Understanding of the effects of trauma on people and how to work in a trauma informed way.

· Good understanding of safeguarding practices in outreach work, social care and/or day centre environment.

· Good understanding of confidentiality and working with sensitive information.

About the organisation

The organisation was established in response to homelessness, which had become a growing concern in Streatham in the late 1980s. Initially providing a hot meal, clothing and advice to those who came for help, they have developed into an organisation that provides a range of services, including the Housing and Homeless Team and Woman’s Team.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

Desired knowledge:

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.
• Ability to communicate, both verbally and in writing and collate and evidence work and outcomes using case management databases.
• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

What you’ll be doing? 

The Women’s Gambling Link Worker will deliver an awareness-raising and personalised, holistic support programme for women in Lambeth experiencing gambling and gambling addiction-related harm, or those impacted by the gambling of a close family member. You will:

• Deliver an outreach programme by regularly attending community events, visiting local groups/ spaces and organisations frequented by women in Lambeth.

• Develop partnerships and deliver outreach support to primary and secondary care partners and services across Lambeth.

• Speak with women's groups in Lambeth, either 1-2-1 or presenting to groups, to discuss the type of support available and promote the work and support of the Primary Care Gambling Service and Age UK Lambeth services. 

• Provide holistic support to patients referred to the Primary Care Gambling Service, helping to address housing, debt, bereavement, loneliness, mental health and other related social issues, that are both a symptom and/or a cause of their gambling.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me’.

This is an exciting opportunity for a Gateway Assessor or Advice Worker with a minimum of three months experience to be fast tracked through the full Citizens Advice Generalist Adviser training programme. Once having completed an initial four weeks training period you will be allocated to work on one of a range of different projects and services providing direct advice to the public and undertaking casework on social welfare law issues.

• You must be a trained Gateway Assessor with a minimum of three months experience.
• An understanding of the advice needs and issues of the local community in a diverse inner-city area.
