Purpose of the Job:
This role is within our crisis alternative service – Safe Space - which is an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm(midday)-11:30pm across all of our boroughs. The shift pattern for this role is 4:30-11:30pm. This will be on a rota basis across support workers.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
- To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To treat service users with respect, dignity and personalised support
- To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
- To increase self-management skills of those accessing the service
- To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
- To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.
Key Responsibilities:
- Providing a person centred and recovery orientated approach in all aspects of the roles and responsibilities.

- Working collaboratively with clients to understand their needs and developing flexible and realistic crisis support packages/person centred plans

You will have:
- Minimum of 1 year working in mental health services and with clients experiencing mental health distress and crisis

About the Role:
As a Specialist Multiple Disadvantage Worker, you will be responsible for delivering targeted, person-centred interventions for individuals within SHP’s Camden Adult Pathway Hostels. Your role will involve holding a designated caseload and working closely with the wider client group through our in-house recovery programme. Our Peer Support Service started 10 years ago, with 2 Wellbeing Centres offering this new service. Since then, we have seen our service grow and develop in ways we could not have imagined. This service is the key service most requested by people and we now offer Peer Support across all of our Wellbeing Centres across Hertfordshire.
Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website). These values form the ethos we work towards within our peer support team. About the Role
Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.
The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.
To meet client need, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres. Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.
The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.
Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

About the role
We have an exciting opportunity for a Peer Support Worker to join our team in the Camden Crisis Teams. We are looking for both full and part time staff, (working hours to be agreed, however, our service runs Monday to Friday, between the hours of 9am-5pm), on a permanent contract. Reporting directly to the Connections Team Leader.
Your role will require you to provide one-to-one support for service users, setting personal goals with the service user for recovery, developing ways with the service user on how to keep well and identifying their own personal sources of support. This will involve work and travel within the Borough of Camden but will be based at in one of the Crisis Team offices either at Daleham Gardens or St Pancras Hospital. The post is a permanent contract with a starting salary between £24,000 - £31,000 pro rata depending on experience.

You will work with the Peer Support Team and draw upon lived experience to assist service users to move forward positively. Acting as a specialist resource to provide a unique perspective to service users.
You will hold a small caseload to allow regular sessions to be held with the service user, maintaining confidential records, utilising VoiceAbility and NHS electronic records system. Planning, implementation of and review of care packages, including parts of the individual care plan.

In this role you will support adults living with mental health difficulties by providing trauma informed relational peer support, nurturing relationships which are mutual, explorative and share power, helping people to overcome the distress they experience and achieve the goals which are important to them.
You will be responsible for a case load of individuals who you will support either face-to-face or via telephone, on a one-to-one or group basis. Service Delivery
· To work effectively as part of a multi-disciplinary team, contribute to team meetings and MDT meetings to ensure continuity of care for people and to ensure their safety.

· To liaise with carers, other health professionals, statutory and voluntary agencies to enable the recovery needs of people using the service.

·To support other members of the multi-disciplinary team in promoting a recovery orientated environment and in identifying recovery focused activities facilitating the sharing of information/education as required.
· Provide referral partners with timely updates about the support provide to ensure the support and care individuals receive is delivered collaboratively and well-coordinated.
· To comply with all training requirements aligned to this role including mandatory training. To actively identify training needs and be prepared to engage in any other training/development opportunities relevant to this role.
· To identify, report and manage any safeguarding concerns, in-line with the organisations policies and procedures.
Service Development
· Support the VCSE Service Lead with the delivery of the VCSE service as a whole, contributing to its ongoing improvement and development.
· Assist management in preparing quarterly monitoring reports outlining progress of the work, assessment of the project impact and outcomes, and key issues raised by members and other stakeholders.
·To motivate and encourage other people who use services and carers to get involved in the Service User and Carer Engagement Scheme and Volunteering Scheme if they show an interest in being involved.
