Our Carer Liaison Workers work within both local NHS Trusts (BRI and Southmead Hospitals), where they make a real difference, supporting adult carers – those who care for others who can’t manage without their help, when the person they care for goes into hospital.

You would provide information, advice and support, enabling carers to navigate their way through the intricacies of the hospital journey, whilst liaising with health and social care professionals to ensure the carers voice is heard and their needs are supported, including during discharge planning.

You would also work with hospital staff to improve their knowledge and experience of supporting carers, through development work and training.

You’ll be joining the team at a pivotal time for our charity. Having recently secured our core statutory income for the next 3 to 5 years we’re now looking to use that as a launchpad to grow and develop new and existing services.

An exciting opportunity has arisen in East Sussex for a highly motivated and committed Family Worker to build on a well-established commissioned service.   

About the Role:  

Kinship is the leading kinship care charity in England and Wales. We work with all kinship carers – the grandparents, siblings, aunts, uncles, and family friends who are caring for children when their parents can’t.  

We’re seeking an experienced Family Worker who is passionate about supporting kinship carers to get the right support at the right time. We are looking for someone who has the interpersonal and partnership skills to perform their role sensitively and creatively while also meeting reporting and impact requirements. The successful candidate will work collaboratively with East Sussex County Council and will share their ambition to support families to keep their children safe through support, advice and guidance.

You’ll be delivering our Kinship Connected programme offering support to special guardians and other kinship carers, providing emotional and practical support to carers in their homes and in the community over a six-month intervention cycle. You’ll also connect them with other kinship carers through peer support groups, as well as supporting to unlock community assets.   

This is a role where you’ll need to have strong boundaries and personal resilience – which we’ll also support through development and reflective practice. 

Key responsibilities include:

• Practical and emotional support to kinship carers virtually or in-person in their home or community.
• Signposting or referring to relevant national and local services.
• Liaising with other professionals and organisations.
• Attending professional meetings when the carer needs extra support (e.g. CIN. CP, family group conference, school meetings).
• Making referrals to other Kinship services such as Advice, Someone Like Me, Peer-to-Peer.

We are looking for someone with experience working with young people in a youth work setting and leading a team to open and take responsibility for the KFC Youth Foundation's first youth hub located in Middlesbrough.

The KFC Youth Foundation is the corporate foundation that was setup in 2015 and is financially supported by KFC UK&I. The KFC Youth Foundation is a charity.

Since its inception the KFC Youth Foundation has made donations and grants of more than £8m. In 2021 we became a community grant maker, offering grants of up to £2,500 to grass roots organisations working with young people in their communities. Organisations who share the passion we have for developing young people. Since setting up the programme we have made grants to a value of £351,570, supporting 10,271 young people.

But we aren’t ones to rest on our laurels and over the last 18 months we have been developing a strategy to transform ourselves into a service delivery organisation. It is our ambition to create KFC Youth Foundation Hubs; amazing spaces with awesome youth workers that allow young people who most need it, to feel safe and secure.

We are opening our first Youth Hub in Middlesbrough this summer. It is our intention to deliver open access youth services with a particular focus around food education, employment and food aid, by creating a training kitchen and cafe.

What will you be doing?

Ahead of the KFC Youth Foundation Hub opening you will be feet on the ground in Middlesbrough during the development of the Youth Hub to opening, supporting with the recruitment of youth workers and hub employees, policy writing, creating a youth board for the hub and co-designing opening programming and the opening of the hub.

Service Delivery & Partnership

• Responsible for the management and preparation(s) of programme delivery - play and youth work and early help provision onsite, that meets the needs of children, young people and families including through direct and partnership delivery with third sector providers.
• Develop and maintain the support needed for a varied 7-day programme of term-time and holiday youth provision (open access and targeted); inclusive of sports, arts, cultural and recreational activity that promotes the development of physical and emotional well-being
• You will have a local knowledge and be able to develop a meaningful dialogue with children, young people and families, with the purpose of assessing their needs and creating services to meet them.
• Establish an inclusive culture within the youth hub; where all are welcome and supported to participate, and where respect, honesty, and trust enables children and young people to thrive.
• Develop initiatives and partnerships to build strong links with the local community and support the growth of positive perceptions of children, young people and their families.
• Implementing methods for observable or measurable indicators of success.
• Organise regular outreach to facilitate the involvement of children, young people from across the ward and borough; targeting those who have had little contact with services and may be hard to engage.
• Work with other key agencies to manage risk.

Leadership

• Ensure all staff are aware of practice standards, expectations and timescales, and establishing a culture of responsibility and accountability building trust, good morale and teamwork.
• Manage service area delivery in a manner that promotes equality of opportunity and collaborative working; ensuring that all youth workers and hub employees are aware of the requirement to deliver non-discriminatory services and to promote greater equity for disadvantaged groups.
• Consistently promote and apply the KFCYF policies and standards in managing employee sickness absence, unsatisfactory performance, conduct, discipline, and grievances, and customer complaints according to the Foundation’s policies.
• Support the recruitment and induction of staff and carrying out regular supervision and annual performance appraisals for line reports.
• Take responsibility for staff timetables, rotas, and annual leave; arranging and authorising any necessary staff cover that may be required.
• Take authority for expenditure for events and activities within established KFCYF financial processes and accounting for all budget spending as required.
• Ensure that KFCYF policy and procedures for health and safety are adhered to and that all equipment is maintained, taking action as necessary to ensure compliance.

Youth Outreach and Engagement Worker

Streets of Growth is seeking to employ an experienced, motivated, and personable individual to work as part of the frontline team to deliver targeted street work to engage and build relationships of trust with high-risk vulnerable young people (15-25 years), in hotspot areas known for Anti-Social Behaviour (ASB), exploitation, serious youth violence and young people that are furthest away from engaging in services.

You should expect to be appointed at the starting point of the salary scale and a higher starting salary band is dependent on you having extensive experience and skills in street work and detached outreach work as well as relevant frontline experience of working with at-risk young people.

These working hours will vary and will be worked according to business needs and notified to you in advance. You will be required to work late afternoons, evenings, and weekends as part of your normal working week.

The role:

Delivering targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.

You will be working alongside Intervention Coaches to progress street-engaged young people into Streets of Growth’s staged-based Appropriate Intervention Bridging Programme delivered from our flagship Centre based in Aldgate East and delivering appropriate targeted intervention work at the street/estate level during critical incident situations in the community.

Who we are looking for:

Along with a relevant qualification up to at least Level 2, you will have robust frontline experience working with young people, including experience of detached outreach work with young people. You will also have good knowledge of safeguarding risk assessment and contextual safeguarding. A key component of the job role is the ability to engage and build transitional relationships with young people, most particularly hard-to-reach individuals.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
• To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
• To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.
• To at all times undertake your role in a professional manner maintaining a high quality standard of work, and to always work in accordance with the aims, values and ethos of The Passage.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

Desired knowledge:

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.
• Ability to communicate, both verbally and in writing and collate and evidence work and outcomes using case management databases.
• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

About the Youth Crisis Service

Our Young Person’s Crisis Service offers a safe, welcoming, and non-judgmental environment outside of normal school hours for those aged 14 – 17 who feel in mental distress or crisis.

About the Role

The Youth Mental Health Worker will support young people aged 14-18 to deescalate crisis and improve their mental health and wellbeing, through focussed 1:1s and structured crisis interventions. The role will involve assessing needs and offering one-to-one support to service users to enable them to begin the process of improving their mental health and well-being and to provide clarity on services and pathways available in the area. The role will be part of a larger Young Person's Crisis team made up of Youth Mental Health Workers, Managers and volunteers.

The successful candidate will have experience of working with young people (under 18) with mental health issues, experience of assessing level of need and experience working in a team or small service.

Service Delivery

• Deliver crisis focussed 1:1s and ensure outcomes which supports young people experiencing a mental health crisis in line with Mind BLMK’s agreed crisis service and requirements as well as contract requirements.
• Assessing need and suitability for the service in line with the access to services process, and undertaking 1:1s.
• Contribute to maintaining an effective volunteer workforce for the service in line with Mind BLMK’s HR policies, procedures and guidance (workload planning and support).
• Carry out health and safety and cash handling in line with Mind BLMK’s policies, procedures and guidance.
• Carry out health and safety responsibilities as directed by the Team Leader in line with Mind BLMK’s H&S policies, procedures and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
• Carry out responsibilities as directed by the Team Leader for the collection, updating, monitoring and reporting of service data in line with Mind BLMK’s contract and systems requirements and procedures.

Senior Youth Outreach and Engagement Worker

Streets of Growth is seeking to employ an experienced, motivated, and personable individual to work as part of the frontline team to deliver targeted street work to engage and build relationships of trust with high-risk vulnerable young people (15-25 years), in hotspot areas known for Anti-Social Behaviour (ASB), exploitation, serious youth violence and young people that are furthest away from engaging in services.

You should expect to be appointed at the starting point of the salary scale and a higher starting salary band is dependent on you having extensive experience and skills in street work and detached outreach work as well as relevant frontline experience of working with at-risk young people.

These working hours will vary and will be worked according to business needs and notified to you in advance. You will be required to work late afternoons, evenings, and weekends as part of your normal working week

The role:

Taking on a lead role to deliver targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.

You will have senior responsibility with the Frontline Director to plan and execute street work and outreach delivery in identified hotspot areas or risk based on street/community and partner intelligence. As well as holding a small street-engaged caseload of young people and providing a lead responsibility to deliver appropriate targeted intervention work during critical incidents in the community.

Who we are looking for:

Along with a relevant professional qualification at level 3 or degree level, you will have significant frontline experience working with young people, including experience in detached outreach work with young people and experience working with at-risk, vulnerable young people. You will also have good knowledge of safeguarding risk assessment and contextual safeguarding in addition to good practice in engaging young people.
