Our Youth Development Leads are the heart of our programme delivery and facilitate high quality and impactful sessions for our young people. Reporting to the Programmes and Impact Manager, you’ll work with autonomy to manage your school partners, develop your team of volunteer mentors and collaborate across our ambitious delivery team with a focus on evidence based continuous improvement.

Designed as a two-year experience for graduates and early-career professionals ready to take on real responsibility from day one. You’ll build the skills, confidence and experience to thrive in leadership roles across charities, education, social impact and beyond. 

Contract: Permanent, part-time (0.8FTE) with a probationary period of 6 months

Salary: £26,227.50 pro rata (£20,982 for 0.8 FTE) in line with the real living wage

Location: Manchester

• In-school project delivery Tuesdays, Wednesdays and Thursday with travel costs covered
• Hybrid working for the remainder of your time – choose to work from home or access co-working spaces across the city

Job Purpose 

The weekend Hospital to Home Project Officer is responsible for responding to referrals received over the weekend from both Royal London Hospital & Homerton Healthcare NHS Foundation Trust. You will work with the manager on Friday to deliver services and plan visits on the weekend for you to attend.   
  

The service supports people returning after a stay in hospital, helping them to settle back at home, regain confidence and support with some practical tasks, e.g. shopping/cleaning. You will manage referrals, carry out assessments, and in liaison with the manager, allocate service users to Hospital to Home Outreach Worker caseloads. You will provide direct frontline support and co-ordinate visits to the service users’ home in preparation for their return home. You will work collaboratively to lead and manage the weekend service to support the smooth transition from hospital to home for service users. The service is time limited, up to 4 or 6 weeks, to adjust, build confidence, and settle back in their homes.    

You will also be required to undertake community-based outreach as part of service delivery, including visiting service users in their homes both pre-and post-discharge. This includes conducting access visits, welfare checks, and providing practical support such as shopping assistance to ensure a safe and sustainable transition from hospital to home. Over the weekend you be based in the hospital, with lone working visits to client’s properties for support.  

Key Tasks 

• Act as the first point of contact for all referrals over the weekend across both hospitals.  

• Meet with patients in the community and/or hospital and carrying out assessments and risk assessments.   

• Enter referrals on to AUKEL’s case management system Salesforce in accordance with GDPR guidelines.  

• Allocate referrals to Hospital to Home Outreach Workers using tech available Field Service App, training will be available.  

• Collect feedback from service users and upload to case management system.  

• Deputise for the manager at discharge planning and other meetings as required.  

• Monitor the case loads of each of the Hospital to Home Outreach Worker (HHOW) to ensure they are evenly and fairly balanced.  

• Work collaboratively with other agencies providing support services.  

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to prevent unnecessary readmission to hospital.  

• Plan and develop person centred interventions to provide short term support for people after hospital discharge.  

• Deliver the front-line support services over the weekend such as:   

• Undertake home visits to service users pre- and post- discharge to support safe and timely hospital discharge.  

• Provide welfare checks to monitor service users’ wellbeing during post-discharge period.  

• Deliver practical, person-centred support during visits, including shopping assistance and other essential tasks to aid independent living.  

• Identify and respond to risks within the home environment, escalating concerns where appropriate  

• Provide occasional cover for the A/L of other Project Officers   

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.  

• Supervise, support and develop HHOWs and volunteers in your service.  

• Liaise with the handyperson service about referrals and jobs for service users.  

• Ensure HHOW are resourced to sign post service users to other services when the home and settle service intervention ends. 

Administration 

• Keep AUKEL’s case management system up to date, input all data, and ensure all referrals, interventions, time spent with service users are recorded.  

• Allocate referrals to HHOW using tech available Salesforce ‘Field Service’ App.  

• Ensure data collected meets the requirements of the service’s contract monitoring and key performance indicators.  

• Ensure service user feedback questionnaires are sent at the end of each intervention and record returns.  

• Ensure admin tasks relating to service users, HHOW or general admin is carried out in a timely and efficient manner. 

Quality  

• Be familiar with and to implement AUKEL policies and procedures in line with our Values.  

• Ensure the service is delivered in accordance line with Care Quality Commission principals.  

• Ensure integration with other AUKEL projects, and service users are referred and/or sign-posted to appropriate support services available to them.    

• Respond to safeguarding or other concerns and make appropriate reports and management action 

Liaison  

• Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional and national issues affecting quality and compliance affecting care and home support services.  

• Work in collaboration with hospital colleagues including discharge teams, A&E, ward staff, community teams to support the successful discharge of people and reduce readmission.  

• Liaise with other AUKEL home and settle services across London and work collaboratively to maintain effective services, including sharing staff where appropriate.  

• Work closely with the AUKEL handyperson service to meet contractual obligations where a handyperson service is provided as part of the service.  

• Liaise with contractors to arrange deliveries and works to service user’s homes.  

 General 

• Meet regularly with your line manager for support, supervision, and appraisal.  

• Attend team and staff meetings, and other meetings as required.  

• Undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of AUKEL.

Job Description

The Head of Delivery & Partnerships is a senior leadership role responsible for overseeing the 
successful implementation, quality of outcomes, and retention of DFN Project SEARCH programmes 
across the UK, the Republic of Ireland and Iberia, in line with our franchise agreement from Project 
SEARCH . This role leads the Delivery & Partnerships team, ensuring strategic alignment, excellence 
of delivery, and sustained impactful partnerships.
You will play a pivotal role in driving programme quality, ensuring retention, maintaining model 
fidelity, and ensuring high-quality outcomes for interns. Additionally, you will oversee the growth of 
our number of programmes, in line with our strategic regional and national plans. You will also 
contribute to the strategic direction of the organisation, working closely with cross-functional teams 
to maximise DFN Project SEARCH’s impact.
Regular travel throughout the UK will be required, with occasional travel to other areas of our delivery. 
In addition, occasional international travel may be required, for example to the annual conference in 
the United States.

You will develop evidence-based and impact-led cases for support for various projects and must be able to unlock growth, develop new approaches and help communicate our impact.

Healthwatch

Healthwatch was set up in 2013 to champion the rights of health and social care users, and hold the health and social care system to account for how well it engages with the public.

Healthwatch has a remit which covers all publicly funded health and social care services for adults and children.

The Advocacy Project delivers Healthwatch Brent, Healthwatch Westminster and Healthwatch Kensington & Chelsea – three of 152 local Healthwatch organisations. About the role

In this role you will promote Healthwatch to local residents and stakeholders, and support people to get involved in our work. You’ll be a skilled communicator, comfortable networking and engaging with a diverse range of local residents. This role is primarily based in Westminster, and we’re looking for someone who can understand and relate to the diverse local population.

Your role will involve finding innovative ways to present the information the team gathers to key audiences. This will range from face-to-face conversations with partners and funders through to producing imaginative materials. This includes producing accessible reports and presentations to make sure we can maximise the impact of people’s feedback.

You’ll be responsible for a variety of internal and external communications for the Healthwatch Westminster producing impactful communications across all platforms including our website, social media channels and newsletter. As well as developing our online presence, you’ll own the Westminster engagement plan. This will involve identifying engagement opportunities, working with the Volunteer Coordinator to ensure we have good coverage at events across the borough, and getting out into the local community to promote our service and hear from local residents.

You will lead the coordination, administration, and reporting of activity funded through a significant seven-year partnership with City Bridge Foundation. We are seeking an organised, dynamic and people-focused Volunteer Coordinator to lead volunteering across both our programmes. This role will supporting and coordinate volunteers across

•  Touching Safe Ground – supporting women and children seeking sanctuary.
•  Well Old – supporting older people experiencing loneliness and isolation.

You will support our established volunteer team within Touching Safe Ground, and develop volunteering opportunities within Well Old. This role is responsible for recruiting, coordinating, supporting and developing a diverse team of volunteers who make our sessions and activities possible.

You will help ensure that volunteers have a positive, rewarding and meaningful experience, while enabling Globe Community Project to deliver high-quality support to participants. This is an exciting opportunity for someone who enjoys building relationships and using their great admin skills to create opportunities for others, helping volunteers develop their skills, confidence and sense of belonging.

What you’ll do

• Coordinate volunteer availability for both of our projects, including weekly Touching Safe Ground sessions. 
• Recruit new volunteers through social media, volunteering websites, local university liaison and community outreach.
• Act as the first point of contact for volunteering enquiries, manage applications, conduct interviews, and manage references and onboarding requirements
• Build and maintain effective partnerships and referral pathways with organisations, universities and community groups, including including universities, faith organisations and community groups across East London.
•  Ensure all volunteers complete appropriate DBS checks, Volunteer Agreements and any other required safeguarding processes.
•  Deliver volunteer inductions and provide practical training, supporting the learning, development and wellbeing of volunteers, and identifying any further training needs as they arise.
•  Network and conduct desk research to identify wellbeing, learning and CPD opportunities for volunteers, particularly those with lived experience of migration or seeking sanctuary.
• Administer volunteer travel expenses and maintain accurate records on shared systems.
• Attend sessions regularly alongside volunteers to build relationships, provide support and understand their experiences.
•  Maintain regular communication with volunteers and respond promptly to questions and concerns.
•  Organise volunteer thank-you events and other opportunities to recognise and celebrate their contributions.
•  Monitor and report on volunteer numbers, recruitment, retention, successes and challenges.
•  Attend relevant staff meetings as required.
• Report safeguarding concerns immediately and follow organisational safeguarding procedures.

Villagers Connect Project – a great opportunity to join our team at Action in rural Sussex

Villagers Connect is a community development project which is working to sustain a strong and vibrant community in the villages of Colgate, Faygate and Rusper. It aims to build connections and relationships which increase confidence, independence and empowerment, particularly for the over 65’s.

We are seeking a part-time community development worker to work as part of a small team that is active in this rural location in the Horsham district. This role involves a good deal of community-based activity, engaging with older people and the wider community. This does therefore require someone who is understanding of older people and their interests, abilities and needs.

We champion an asset based approach to community development, which is about building on existing strengths and supporting local people to flourish; undertaking community led projects that create community cohesion and have lasting impact.

You will identify, develop, and manage partnerships with charitable and not-for-profit organisations across Aberdeenshire, building an effective presence within the third sector locally.You will create diverse, high-quality, structured volunteering opportunities with charity partner organisations, ensuring that volunteering roles reflect the needs and interests of those we support.You will also support the creation of strong referral pathways into our service, and beyond it, building on a knowledge of existing provision. Key Duties Responsibilities 

To engage directly with people 16+ to understand their wants, needs and aspirations and determine the best opportunity for each person and provide the support they need in advance of volunteering. 

To support on a 1:1 basis, in person or digitally with the volunteer to create an action plan. 

To work in partnership with All-in-Aberdeenshire consortium to achieve outcomes. 

To develop and maintain good relationships with referral partners such as The Council, DWP, SDS etc to ensure a steady stream of referrals are received. 

To Liaise with referral sources, regarding new and existing referrals. 

To identify, create and develop relationships with new charity partners or agencies to provide bespoke and tailored volunteering opportunities.These opportunities will help the individuals supported to gain experience, confidence and support them on route to a positive destination. 

To manage the risk assessment, quality assurance, and audit processes as appropriate. 

Ensure monitoring, impact and evaluation of information and data is collected and that the project meets its agreed targets, reporting format and schedule.Assist the Delivery Leader by preparing reports to funders. 

To ensure all the above is done with excellent record keeping, in accordance with compliance requirements.

What You’ll Be Doing 

• Take the lead in managing and delivering NCN infrastructure projects. This includes planning, design, construction, and reviewing how well the project was delivered once completed. 

• Build and maintain positive working relationships with external partners and local authorities. Work together with them to develop active travel infrastructure projects collaboratively. 

• Plan, organise, and take part in community engagement activities. This helps make sure projects are developed in a collaborative and inclusive way. 

• Review and analyse information from policies, audits, surveys, and site visits. Use this information to create technical reports, feasibility studies, and progress updates.
