Overall Purpose of Role

In your role as Community Pastor, you will lead, innovate, and add momentum and direction to all of our local Community Outreach work at Saint Paul’s Hammersmith. You will strategically represent the church with external stakeholders, help to create key partnerships within the community, whilst building and empowering a dynamic and vibrant volunteer culture within the Saint Paul’s congregation.

You will oversee our Community Café and lead on all our community outreach programmes across Hammersmith, including our Community Hub, Café Concert and Love Christmas Project.

We are looking for someone with a proven track record in building volunteer teams, supporting key leaders and the ability to develop collaborative relationships with external organisations, charities and grant making bodies. You will enjoy the challenge and opportunity of a fast-paced environment, where teamwork is essential but where individual skills and distributed leadership are also required.

You will also be an ambassador for Love Your Neighbour (LYN) which is part of a national movement of churches collaborating in local communities, alongside people of all faiths and none to support individuals and families in need. This will involve being part of the LYN Hub Accelerator Programme, to help Saint Paul’s refine our vision and impact in this area of social transformation.

Key Responsibilities

• Effective coordination of our City Transformation ministry.
• Ensuring that our projects are meeting the needs of our community.
• Reaching out to those in crisis to help people access places of care, both inside and outside of Saint Paul’s, and helping people access programmes that address the root cause of crisis.
• Co-ordinating and implementing our vision to become a Love Your Neighbour hub that makes a long-term impact through social transformation and helping to implement our 5-year strategy for Social Transformation.
• Oversight of our Saint Paul’s Community Café and vision and strategy on scaling the café.
• Acting as an ambassador for and developing relationships with our partner charities, Spear and Crosslight to ensure that their ministries are well supported and integrated into the life of Saint Paul’s.
• With the support of our Operations Manager and using existing Love Your Neighbour impact measurement systems, collecting data and carrying out regular impact measurement to create reports for church leadership, PCC and funding partners.
• Key Lead and point person for Adult Safeguarding and sitting on the Saint Paul’s Safeguarding Team.
• Termly line-management and connection with our volunteer chaplains, chaplain to St Vincents Care Home, and chaplain to St Paul’s Primary School.
• Recruiting, training, supporting, and investing in a volunteer team to enable wide participation in our City Transformation ministry.
• Networking and building relationships with charitable organisations, churches, public sector organisations and other key stakeholders.
• Maximising opportunities for all ministry areas within the church to be involved in social transformation ministry.
• Responsibility for identifying and following through with grant applications and reporting.

Cafe Concerts

Our monthly Cafe Concerts were established in the Spring of 2023, aimed at creating a uniquely multi-generational open door for the local community to enjoy a morning of world-class music, coffee, cake, and a warm welcome. We now regularly host between 60-70 people, including local asylum seekers, residents from St Vincents care home, all alongside our growing Stay and Play community of Mums and Toddlers who join the Cafe Concert.

Community Hub

The Community Hub was created in 2023 to support those who are experiencing loneliness. There is a hot meal provided, as well as kids activities and lots of space for conversation. There have been seven Community Hubs in 2023, with over 500 guests (including many asylum seekers based in local hotels) and a 30-person volunteer team. We have supported in a variety of ways, including writing references, providing clothes and most importantly, forming friendships with people. 

Love Christmas

In December 2023, we partnered with Love Christmas to pack and deliver more than 720 Bags of Kindness to our local community, with the help of 30 volunteers. The combination of donations raised from within our congregation, and match funding from Love Your Neighbour enabled us to show practical love and care to some of the most vulnerable in our local area. You will be responsible for overseeing and developing this project.

You will also helping to establish new initiatives as the need arises, in collaboration with the Senior Team. This might include a breakfast club for children at our local school, Kids Matter, and Recovery Groups/ courses that will have a missional outworking and community focus.

Working Requirements

NHS England recently announced a new transformation fund to be allocated to pilot sites within 12 Sustainability and Transformation Partnerships/Integrated Care Systems across the country to test new and integrated models of primary and community mental health care. The Surrey Heartlands Health and Care Partnership in Redhill is one of these successful pilot sites, and Richmond Fellowship are proud to be part of that partnership.

The role of Community Connector is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (Primary Care Networks). Indeed, whoever takes on this particular challenge will be instrumental in supporting the ongoing development and mobilisation of integrated primary care mental health services in the future. Along the way, you’ll work jointly with a Mental Health Practitioner to conduct assessments. We’ll also rely on you to help patients identify their socially determined needs and goals, provide self-management tools, and develop personal support plans. Delivering a range of motivational and structured psycho-social interventions, promoting independence through an enabling asset based approach and developing effective relationships with a range of agencies to facilitate a ‘joined up’ approach - these are just some aspects of this vital role.

As well as proven relevant experience, you’ll need an appropriate diploma or degree i.e., in counselling, psychology, social work, probation, mental health, Health and Social Care (level 3), NVQ’s level 3+ and/or community experience of drug, alcohol, mental health work. You’ll also need the ability to work co-operatively and flexible as part of a multidisciplinary team from a service hub in one of three locations and be willing to travel to and from a number of different locations on a daily basis. Comfortable working to the confidentiality, consent, information sharing and safeguarding policies of the integrated service, the ability to manage any challenging behaviour, anger and verbal aggression from patients is important too. And, it goes without saying that you have excellent communication skills and are proficient in the use of office IT.

Job description

 Mind in Tower Hamlets, Newham and Redbridge are working in partnership with Stepney and Wapping Primary Care Network’s Neighbourhood Team to support the Transformation of Mental Health Services across Tower Hamlets.

Tower Hamlets is one of the pilot Boroughs to implement the Community Mental Health Framework which aim is to redesign the offer to support people with serious mental illness (SMI) to not just get back on track with their lives but to be happier and more fulfilled.

The programme is grounded in a population health approach to SMI, delivered through and with Primary Care Networks in Tower Hamlets, with an ambitious vision to support more patients closer to home and reduce the stigma and fragmentation in existing services.

Purpose of the role

 The Community Connector will be responsible for working within the Neighbourhood Team within Stepney and Wapping PCN to ensure that people with SMI are able to live fulfilled lives through offering of one-to-one support to set goals towards their aspirations, needs and to offer hope and choice using a range of skill, knowledge and tools to achieve this. The community connector is seen as valuable asset to our neighbourhoods and act us a local expert, gathering and sharing information about local services.

The post holder will work collaborative and in partnership with the Multi-Disciplinary Teams and local Services and Community Groups to create pathways of support for people and to will act as the ‘glue’, linking people in with experts and local assets, and undertaking support work in partnership with external stakeholders to complement their interventions. The post holder will support people to identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, employment circumstances and unemployment, loneliness, isolation and caring responsibilities.

You must have:

Climate Change Adaptation Officer

As the Intergovernmental Panel on Climate Change (IPCC) sixth assessment report laid out in March 2023, we are now on the brink of irrevocable damage from climate change; this is our ‘final warning’ and the time is now for action to address this crisis.

The report highlighted that restoring nature was one of the key actions needed to address this issue. Somerset Wildlife Trust are at the forefront of this work in Somerset and are fully committed to this path through our Wilder Somerset 2030 Strategy and to delivering a vision whereby we reverse biodiversity loss and tackle the climate crisis.

Developing and further embedding the ‘Act to Adapt’ process, through our ‘Adapting Somerset’ and ‘Somerset’s Wilder Coast’ projects to support communities and to develop plans to adapt their local areas to the impacts of climate change.

Responsibility 1: Providing leadership and technical expertise

Being proactive and nurture strong relationships by:

• Providing leadership on SWT Climate Change Nature Adaptation work.
• Working closely with the Local Nature Partnership Coordinator and Advocacy Manager, and wider project team from partner organisations, to develop and deliver a programme of online and in-person communications, meetings and workshops. This is designed to foster climate change adaptation in the county supporting nature’s recovery and community resilience, and therefore involves working closely with farmers and landowners, all layers of local council, infrastructure experts, academics and the general public, including communities already impacted by flooding.
• Providing line management to the Climate Adaptation Assistant.
• Continuing developing the RAD approach for priority habitats, alongside the Landscape Recovery team, which will contribute to managing land within internationally important wetlands.
• Keeping abreast of national and international climate change developments and relate these to Somerset.

Responsibility 2: Engagement with Communities and Creating Robust Partnerships

Lead engagement efforts by:

• Contributing to discussions and workstreams developing climate change adaptation pathways at a county level, and in specific local areas relating to flooding, drought and wildfire.
• Developing information for a lay audience regarding the role of nature in reducing the impacts of climate change, including: sea-level rise; predicted changes in land use; warming, etc. and to help people understand what the future may bring especially in Somerset’s more vulnerable nature habitats.
• Supporting people’s wider understanding of the role of nature to mitigate climate change through carbon sequestration and storage.
• Continuing the rollout of the Act to Adapt process in local communities, organising events and workshops and using the Climate Adaptation Toolkit to explain the process of developing community and nature based solutions to the big issues of climate change.

Responsibility 3: Developing sustainable projects and evaluation

Lead and support projects and reporting by:

• Working with the Somerset Local Nature Recovery Strategy team to incorporate climate adaptation and vulnerability principles to these priorities.
• Assisting with work to share information and project developments with project partners in the UK.
• Developing and implement a communications plan that raises awareness of the project and its impact; garners support; and highlights Somerset as a place at the frontline for climate change. This will include engagement through social media and the production of awareness materials in printed and online formats.
• Managing the budget of the Adapting Somerset project and keep track of time contributions to the ELM’s Landscape Recovery and Somerset’s Wilder Coast projects.
• Monitoring and evaluating progress and impact, meeting project milestones and reporting to funders.
• Representing SWT externally in meetings, in the media and by giving presentations to various audiences.

We are hiring an Area Manager to provide management and oversight for services which span across the Royal Borough of Kensington and Chelsea (RBKC). Our services provide 154 units of accommodation to people experiencing homelessness and have complex needs. The services are 24 hour supported accommodation sites. You will have the overall responsibility to deliver the highest standards of service quality, performance and improvements across your services through excellent leadership and embodiment of our SIG values.

As the Area Manager, you will lead the team to deliver exemplar Trauma Informed support to our residents and participants. The current team you will directly manage:

• Deputy Manager x 3
• Activities Coordinator x 1
• Resettlement Manager x 1

Rota: Monday to Friday 9am to 5pm, flexibility required to meet service needs, based across all RBKC services under your area

The in-service leadership team support a team of frontline Support Mentors and Peer Support mentors to deliver dynamic and flexible person centred support aimed at empowering our residents to make and sustain positive change.

• Ensure service responsibilities and requirements are carried out effectively. This includes but is not limited to; welfare checks, risk assessments, support sessions, safeguarding checks, training and development, and other responsibilities required for effective service delivery.

• Work proactively to ensure the service and team meet and exceed Key Performance Indicators, and work in line with professional codes of conduct.

Risk Management

• Follow relevant risk assessment and management procedures, share relevant information with others as necessary.
• Ensure all case recording and information is accurately recorded in a timely manner, ensuring confidentiality and GDPR.
• Champion, and act as a point of reference for safeguarding concerns within the service(s). Ensure all staff attend relevant safeguarding and training and use knowledge to coach and advise the team.

Property and Housing Management

• Ensure the accommodation in which residents live is clean and maintained to a high standard
• Ensure all repairs and maintenance issues are correctly reported and managed through to completion.
• Ensure a provision of high quality housing management service is provided to residents. 

Financial Management

• Manage the service budget and review monthly management accounts: Set budgets and ensure resources are maximised.
• Promote effective cost control mechanisms and other financial activities.
• Maintain financial management within the service.

Other

• Some of what else is included: Contract management and Internal auditing, admin, and general other duties as required.

KEY CRITERIA

What we are looking for:

• Understanding and/or experience of working with people of complex backgrounds, ex-offenders, mental health, substance misuse, challenging behaviours
• Experience of working with and engaging with diverse groups of people from varying backgrounds

• IT Proficiency, including Microsoft Office, and the ability to navigate and learn new case management systems and other types of organisational software.

About the Peer Mentor Service

Shelter is delivering a Peer Mentor Service in partnership with the Forensic Intensive Recovery Support Team (FIRST) within Birmingham and Solihull’s Mental Health Foundation Trust. The FIRST service is a comprehensive recovery focussed multi-disciplinary team addressing the individual needs of our service users through tailored packages of care. It includes Consultant Psychiatrists, Psychologists, Advanced Nurse Practitioners, Community Psychiatric Nurses, Occupational Therapists, Support Workers, Peer Mentors, Substance Use workers and Social Workers. Each service user will have an allocated Care Coordinator and Responsible Clinician who monitor, and review care and support needs and interventions.

The Peer Mentor role will engage with clients on the ward and within the community - with a primary focus on supporting clients with their transition away from staying on the ward to living in the community. The Peer Mentor service’s aim is to help people with multiple and complex needs to enhance the support available through the FIRST Pathway.

You’ll need to have been stable for at least 18 months and be willing to share details of your successful journey with others to help them. You will be looking to go into a career supporting vulnerable adults and this will be a great opportunity to gain experience. Basic computer skills will be needed, as well as the interest to develop and learn new skills.

About the Role:

The primary purpose of this role is to provide debt advice and money education on a case management basis and to support the provision of advice by volunteers. The post holder will be expected to manage their own cases and lead appointment sessions.

The job holder will play their part in bringing this vision to reality, supporting clients through what at times can be emotionally difficult situations, to get back on their feet and realise their full potential.

The Role

The project funded by the Elwyn Thomas Memorial Fund will deliver an Emotional Support Service as well as through collaborative community working, will result in valuable assets for the community. In the shape of self-sustaining peer and volunteer led support through groups, partnerships and activities.

Reporting to the Emotional Support Service Manager, the team will deliver a professional counselling service to stroke survivors, carers and relatives.  Counselling sessions are designed to help clients to develop emotional resilience, readjust to life after stroke and prepare to reintegrate into community life.

Key responsibilities will include:

· Providing one to one counselling to stroke survivors, carers and relatives including those with communication or cognitive difficulties

· Providing counselling sessions in person, on-line or by telephone, throughout Cornwall

· Assessing the appropriateness of counselling for service users by providing emotional needs assessment sessions as well as using appropriate screening/evaluation tools (e.g. PHQ and GAD)

· Managing risk and safeguarding issues in accordance with appropriate legal, ethical and organisational requirements.

About You

You will have:

· An advanced diploma in integrative, humanistic or person-centred counselling/psychotherapy; for example, BACP recognised course or equivalent (i.e. 450 hours skills and theory, 100 hours of supervised practice, 20 hours of personal therapy)

· A minimum of 1 year post qualification supervised clinical practice

· BACP registration (or equivalent) and actively working towards accreditation

· Experience of delivering short-term counselling

· Experience of working with people with cognitive impairment

SIG Safe Ground is an award-winning charity which delivers high-quality, group work interventions to people in prison and community settings. 

Our arts-based therapeutic group work focuses on relationships and identity, particularly in reclaiming and reframing imposed identities while offering access to new perspectives, creating opportunities for discussion and debate, and inviting participants to take risks, experiencing themselves and each other in new ways.

You will report to the Head of Criminal Justice Services and line manage the programmes coordinator, as well as work closely with our services based within HMP Lowdham Grange and HMP Altcourse. You will lead on the development of new creative work, identifying gaps and opportunities, working alongside various teams to open new business opportunities to secure funding, and facilitate the programmes we deliver, mainly in prisons and community settings. We are looking for someone to bring a new creative outlook, able to be agile and think of new ways to promote Safe Ground, and develop our resources, this could include delivering theatre productions, films, and podcasts. 

Working Pattern: Monday to Friday 9 - 5 although you may need to work outside these hours as required. The role will be hybrid, working from home, our central office in Islington, and able to visit various sites across London, Nottingham, Liverpool, and additional locations as required.

• Ability to create, develop, and deliver new programmes and/or creative content in various settings such as prisons, communities, and wider criminal justice settings
• Ability to promote the service and provide outreach-based provision, with an ability to liaise and work effectively in partnership with stakeholders
• Ability and willingness to travel to various services and work within the prison setting

What we would like, but not essential:

• Relevant professional qualification in applied theatre, drama, or a field related to programme delivery or health and social care in criminal justice system
• Previous experience in developing, producing, and/or performing professional productions for example film, television, or theatre productions
• Previous experience with commissioners, supporting or contributing to research and evaluation projects
• Previous experience in supporting winning new contracts for example in bid writing or fundraising activities
• Financial Management experience: Budget setting, analysis, and cost reduction
