About the role
We are looking for a multiple Support Workers (known internally as Project Workers) to join our Hammersmith & Fulham Pathway.
We have a vacancy in our semi independent team, supporting 22 clients across two projects based in H&F. Shepherds House and Talgarth road focus on providing recovery based support to those in the last stages of recovery before moving into more independent accommodation.
We are also looking for a Support Worker to join our team at The Coninghams, a 31-bed complex needs service, that supports both men and women in Hammersmith.
In the role of Project Worker, you will be supporting our clients on their road to recovery; aiding clients to gain necessary skills to move on from our services and into suitable accommodation. You will work with a case load of clients, aiding them to gain necessary skills to maintain independence in the community. The role is focused on supporting residents around their strengths and aspirations for the future.

Post Title Progression Worker

Purpose of Post To support users of services through an education, training, and employability programme integrated with SCT’s wider services and enterprise offer, as well as partner opportunities in the wider community. Spitalfields Crypt Trust (SCT) is an East London charity embedded in the local community, providing practical support and training to people affected by homelessness and addictions so they can rebuild their lives. We have a Recovery Hub in Shoreditch, where we run an abstinence-based residential recovery service (‘Acorn House’), and a Training and Development service, including tutor-led art, creative writing, digital skills, gardening, literacy, and woodwork. In addition, we run Housing First and Supported Housing services, both in East London. We also have eight charity shops and two social enterprises, which give us a presence in the wider East London community while generating valuable funds.

We are dedicated to supporting those who access our Training and Development service to explore further Education, Training, and Employment (ETE) opportunities as part of SCT’s Progressions Programme, both within and outside of SCT. This includes those in early recovery who are resident in our abstinent-based recovery service (‘Acorn House’), as well as those in our move-on house and the wider community who may be more established in their recovery. SCT’s Progression Worker supports those accessing our services to explore ETE opportunities, offering tailored, individually specific support to help them reach their full potential. Collaborate with SCT tutors and managers, particularly the Therapeutic Programme Manager and Supported Housing Manager, to identify those who may benefit from the support on offer.

The Role
Through sharing wisdom from their own experiences, the Mental Health Support Worker will inspire hope and belief that recovery is possible in others. As an integral and highly valued member of the team, you will provide formalised peer support and practical assistance to people using the service in order for them to regain control over their lives and their own unique recovery journey.
In particular, the Mental Health Support Worker will support the smooth transition of people using the service to other Mental Health services, by helping them to complete recovery and crisis plans for example. Within a relationship of mutuality and information sharing, you will promote choice, self-determination, and opportunities for the fulfilment of socially valued roles and connection to local communities.
Key responsibilities include:
- Work alongside an agreed number of people using the GPIMHS / MHICS service on a 1:1 basis, in person and / or virtually, from a lived experience perspective.

- Use personal, experiential knowledge and to share lived experience when appropriate to benefit others.

- Support people using the service to identify and overcome fears within a relationship of empathy and trust.
- Signpost to various resources, opportunities, and activities within the Organisation and in communities to promote choice and informed decision making.
- Accompany people who use the service to appointments / meetings / activities of their choice and performing a range of practical tasks, aligned to recovery goals.
- Assist in risk assessments with multi-disciplinary staff, highlighting any changes in service users’ presentation relevant to their safety and feedback accordingly.
- Maintain a working knowledge of current trends in mental health, recovery, and peer support by reading books, journals and accessing peer support networks.

What you'll do:
Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.

About the role
Recovery and connect is a project delivered in partnership with Surrey and Borders Partnership NHS Foundation Trust. The Outreach Support Worker will work intensively with a caseload of individuals identified by the HomeFirst alliance, who have a pre-existing mental health concern and to support them to access ongoing community services.
The role will involve traveling across different locations in Surrey Heath and working as part of a team aligned to the shared care forum to support vulnerable service users.

Herts Mind Network (HMN) has formed a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.
This is a new arm of the project focused on serious violence, with one worker based at Watford General Hospital and one worker based at Lister Hospital working in A&E departments and wards facilitating discharge for people aged 16 - 25 who have been admitted to hospital due to being a victim of serious violence.
About the Role:
The Bounce Back Workers will be based at Watford General Hospital, Lister Hospital and HMN Centres, promoting joint working between A&E, creating a seamless pathway into community support for clients recovering from serious violence and mental ill health.
The aims of the Bounce Back service are; to provide up to 16 sessions of emotional and practical support, advice and information to patients. We will visit patients on the ward and continue the sessions in the local community and in patients homes once discharged. We will facilitate a smooth and timely hospital discharge and focus on re-integration back into the local community.
The Bounce Back Worker will work with the HMN Community Outreach Teams and the hospital staff to ensure patients are offered advice, information, and holistic support. The Bounce Back worker will use a recovery orientated approach working with patients on the ward, and in the local community, including in people’s homes.
The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.
Key Responsibilities
- Provide practical and emotional support to encourage patients to develop their independence within their local community.
- Facilitate a smooth discharge from hospital back to the community.
- Have a solid understanding on the impact of serious violence on an individual’s mental health.
- Have a working knowledge on the areas of serious violence including knife crime, cuckooing and county lines.
- Monitor outcomes and evaluate the service.
- Ensure that the safety and wellbeing of patients using the service is monitored and reviewed regularly.
