Age UK East London are recruiting a Community Engagement Officer to join our growing Community Outreach team. We support hundreds of people a year to maintain their health and wellbeing, combat social isolation and digital inclusion through the varied activities and by signposting to relative local support services.

Job description 

Job Purpose 

• To provide a high-quality outreach support service that enables older adults with low level support needs to live independently within the community. 

• To a promote network services across the Boroughs to maximise their take-up. 

• To work closely with statutory and voluntary sector agencies to ensure appropriate services are accessed. 

• To ensure that service users have maximum choice and control over the planning and management of the service they receive, including initiating individual support plans and connecting them with culturally appropriate services. 

Key Tasks 

• To signpost people to appropriate services as required through outreach sessions in the Hub, Satellite centres, GP practices and pharmacies. 

• To develop shared working practices across the service network. 

• To work closely with providers of similar services 

• To work to agreed individual, team, and organisational objectives. 

• To work with existing activity groups at the hub and satellite centres and support the setting up of activities where appropriate. 

• To ensure all contacts with service users are recorded appropriately and to maintain an adequate filing system. 

• To use word-processing software to produce own correspondence and reports in accordance with relevant policy on confidentiality and data protection. 

• To collect monitoring information to enable the service to be evaluated. 

• To provide support to services outside of normal working hours and occasionally attend sessions when required. 

Administration  

• To ensure all relevant administration is organised and efficiently completed. 

• To assist co-ordinating service user client surveys/questionnaires as appropriate, collate feedback and evaluate the service provided. 

• To assist with the collection of data necessary to provide specific information needed for KPI’s and external requirements and outcomes. 

• To ensure all data is collected through AUKEL organisational customer relationship management data base systems. 

Quality 

• To be familiar with and to implement Age UK East London’s policies and procedures. 

• Ensure that confidential data is managed within AGE UK’s policies and procedures.

 General 

• To meet regularly with line manager for support, supervision, and appraisal. 

• To attend team and staff meetings, (and other meetings) as required.

The role and your key responsibilities

As a Partner Success Lead your role is to provide world class customer service to our partner base of over 2,500 schools and colleges. You will be responsible for onboarding our new schools and supporting our existing schools to make the most of the Unifrog platform. Solutions and customer service focused, you will make sure that all interactions with Unifrog are prompt, positive and successful. 

Your key responsibilities will include:

• Partner management

• Being the first point of contact for enquiries from our existing 2,500+ partners.

• Onboarding our new schools by setting them up on our platform and making sure teachers and students are ready to get the most out of Unifrog.

• Continually improving our onboarding materials and processes using feedback from our partners.

• Partner training and support

• Training staff remotely on how to use the platform.

• Troubleshooting issues and providing on-the-spot support via phone, email and video calls.

• Ad hoc tasks

• Leading on and supporting a variety of other projects across other parts of the business during quieter periods.

• Proactively working to foster a sense of community amongst our Unifrog partners and always thinking about how we can promote the sharing and embedding of Unifrog best practice across our partner schools.

About the role:

The SHP Somerset Cluster is based in the heart of the London’s theatre and shopping districts – the main hub is just off Drury Lane, with the other sites near Oxford Street and Tottenham Court Road. The aim of the service is to support people over the age of 18 to navigate various support needs and develop the skills required to maintain independent accommodation long term.   

As a multiple disadvantage (MD) referral and move on coordinator, you will work as part of a larger support team to ensure that consideration is given to the individual experiences of multiple disadvantage when it comes to accessing and moving on from our service.   

You will work with people from their first interaction with the service: at the point of referral, ensuring that all aspects of someone’s experience is considered by undertaking a trauma informed, culturally sensitive assessment and supporting your colleagues to develop their awareness of these experiences. 

From here you will then support their journey through the service by working alongside other support staff to foster a culture of move on, supporting the development of holistic and realistic move on goals.   

This work will be underpinned and supported through the development of a move on programme. This programme, developed and overseen by the MD Referral and Move on Coordinators in partnership with the Recovery and Opportunity Coordinators, will be not only look to develop confidence and ability to navigate social systems, but also be accredited through SHP’s AQA Registered Learning Centre.   

In addition, you will carry your own caseload for people living in the step-down services to ensure that they are prepared, equipped, and confident about moving on. This may include identifying current barriers within the system that can prevent someone from moving on successfully or within their set timeframe. 

About you:

• Experience of coordinating projects across multiple teams or people with varying roles to achieve successful outcomes for vulnerable people
• Knowledge and awareness of the intersection of needs in people facing multiple disadvantage & strong understanding of how the intersection & co-occurrence of these issues can make it difficult for people to engage with support or to move on from services.

Here at One Million Mentors, our aim is to ensure that every young person in the country has access to a trained mentor as they transition into adulthood. We believe that investing in mentors will help to address the skills gap agenda and improve social cohesion.

The Regional Manager for Programme Delivery focuses on mentee demand, and managing the end-to-end coordination of multiple programmes cross London. The role is largely responsible for delivery of 1MM Mentoring Programmes. This includes partnership development, matching mentors/mentees, and oversight of mentoring relationships. Also to consider safeguarding measures, fundraising, and meeting growth targets. The role will help to establish a pipeline in line with 1MM’s aspirations for 2024 and beyond.  This role reports to the Chief Commercial Officer (CCO).

1MM Regional Manager for Programme Delivery’s main tasks include:

• Helping 1MM to scale by helping establish over 150 new, high quality mentoring matches each quarter. (A match means that the mentor and the mentee have completed 1MM’s training and have had their first meeting)

• Meeting all quality assurance KPIs.

• Maximising conversion, retention, and mitigate drop-off

• Ongoing support for key stakeholder partners, including Youth Partners, and maintaining strong relationships.

• Securing at least one new partner per quarter.

• Becoming familiar with the 1MM Platform, using it comfortably for ongoing administration and providing support to users.

• Providing regular reports on progress related to the role.

• Potential for line-management responsibilities as the team grows.

Terms and Conditions: This is a full time role offered in London, for 6 months (continuing subject to funding). This is a hybrid role with two days working at home, two days in the London office, and one day where it is up to you.

We are looking to recruit two additional project officers mainly to help deliver our Live Love Later Life and Healthsmart services whilst providing organisational and administrative support to help these run smoothly.

An exercise qualification and experience of delivering health checks are highly desirable. However, if you do not hold these qualifications you will need to undergo a chair-based exercise qualification and blood pressure training. Applicants need to be willing to undergo this training. They also need to have the confidence and enthusiasm to deliver exercise to groups of older people whilst motivating participants that may have a variety of physical and cognitive challenges. These attributes will be assessed through a task during the interview process.
