We are looking for a creative, compassionate and enthusiastic leader who can help us not only reach but exceed our goals for this project and be the nurturing force to help Roots to Wellbeing develop and grow. 

Roots to Wellbeing project aims:

• Provide a safe and inclusive space for participants to receive weekly support.
• Help participants to connect with nature and socialise with others to feel happier, less anxious and less isolated.
• Support participants to access other beneficial services, where appropriate, including health, social and financial.
• Support participants to move onto outdoor work or volunteering with friends of groups where appropriate.
• Facilitate a positive journey through the service for participants to ensure as many people as possible can access the service.

The role 

The roles has four main areas of work:

1. Leadership and management:

• Lead the Roots to Wellbeing team to reach its goals. 
• Line manage and support three Mental Health Officers, each delivering Roots to Wellbeing on one site.
• Work with local communities and other team members to explore new opportunities for the project and develop solutions.
• Work with CEO and Head of Fundraising to secure funding for project development.

2. Project management: 

• Oversee the delivery and development of the project to meet key KPIs.
• Collate qualitative and quantitative data and produce impact reports for funders. 
• Support team to ensure administrative records are kept up to date, including but not limited to: maintaining accurate participant records, work programme, incident reporting, risk management, external outreach, website.
• Develop a thorough communications strategy and build on current local referral relationships to further embed the programme within the VCSE & Health sector.
• Ensure service standard and quality is consistent across sites and policies and procedures are followed. 
• Recruit and induct volunteers to support session delivery.
• Work in partnership with stakeholders within and across the organisation to deliver project goals.
• Attend networking meetings and community events to raise awareness of the project and demonstrate its impact. 

3. Session delivery at Hartcliffe Millennium Green: 

• Continually build and maintain relationships with local referral partners to achieve a consistent referral rate into the project. 
• Deliver weekly Roots to Wellbeing sessions by; planning and facilitating delivery of sessions and activities in line with 5 pathways to nature connectedness and that are accessible to all participants, organising logistics of sessions such as travel, food and equipment, acting as safeguarding lead during sessions, liaising with co-facilitator, leading peer-support style check in at the beginning of the sessions and the grounding and meditation at the end.
• Lead on all direct communication with participants at Hartcliffe Millennium Green. Support them to create personalised development plans, signposting and referrals where needed. 

4. Other: 

• Support the work of the wider charity and specifically the South Bristol team as required. 
• Representing the charity externally through media work and presentation where necessary.

Accountable to: Wellbeing Hub Manager and Senior Wellbeing Navigator

Hours worked between: 9.30am - 5.30pm Monday - Friday pro rata

PURPOSE OF THE JOB

The Barnet Wellbeing Hub is an exciting new initiative that utilises a social prescribing model to support adults over 16 years old with common and long term conditions and/or social care needs to become involved in community activities. The Wellbeing Hub is an integral component of the overall Integrated Wellbeing Service Strategy for Barnet. A vacancy available for the right candidate to support the continued growth and development of the Social Prescribing offer in Barnet.

The hub acts as a referral Single Point of Access between statutory providers and community settings. A high volume of people are referred from statutory providers, other community organisations, or could self-refer to the hub. Staff will support individuals to engage with community activities.

The role of the wellbeing navigator team will be to provide a holistic service from referral through to discharge. They will work with partners to accept referrals into the service. They will welcome each person, provide a short triage to ensure that their conditions are appropriate ones for the service to assist, and then make appointments for them to visit for more in-depth assistance where necessary. They will also carry out background administration for this service such as logging individual details onto the service database.

MAIN TASKS

Individual Support

• Working with other team members, to provide first point of contact to all those accessing the service, whether face to face, by telephone or email.
• As part of a team, make contact to all people referred into the service.
• Provide a basic Triage service to people who are referred, either over the telephone or via appointments in the office.
• For those people with appropriate needs, book appointments for them to meet.
• Assist people to engage with community activities.
• Follow up with people who have used the service to ensure that they are engaging successfully.
• Provide a professional service that maintains boundaries, but is warm and respectful.
• Maintain individual confidentiality and respects equal rights and diversity.

Outreach work

Links with other community organisations and relationships with statutory services are integral to the success of the Wellbeing Hub. You will be working as part of the team to formulate an outreach work plan to support the Integrated Wellbeing Service working with our partners,

• Log all individuals accessing the service onto the central database in an accurate and timely fashion.
• Liaise with our partners and encourage collaborative and partnership working
• Build relationships and rapport with external partners
• Seek and generate referrals to the Wellbeing Hub and improve the community access into the Wellbeing Hub service
• Work flexibly at other premises, including GP practices, to deliver Emotional Health Checks to individuals based around the borough where appropriate

Administrative Duties

• Log all individuals accessing the service onto the central database in an accurate and timely fashion.
• Information logged should include people’s demographic information, presenting needs and any other case notes deemed to be appropriate.
• Log appointments for people to meet Wellbeing Navigators.
• Ensure that all paper or computerised records and documentation regarding the service is effectively and securely stored.

General duties

• To maintain accurate records to fulfil reporting requirements

We are seeking a dedicated and compassionate Community Wellbeing Support Worker to join our Flintshire Wellbeing service. This role does not provide direct basic care but is focused on motivating and supporting individuals to identify their own goals as well as developing self-management strategies to improve their health and wellbeing and maintain their independence.

This role will ensure that guided conversations are led by the individual and that they are at the centre of everything we do.This is a new service commissioned by Flintshire County Council on behalf of health and social care partners through the Welsh Government Regional Integration Fund. The service has been commissioned to fill a gap identified within existing service provision.

If you have an empathetic and compassionate nature with a genuine desire to help others improve their health and wellbeing, this role could be for you. The successful candidate will play a vital role in helping individuals improve their overall well-being and make positive lifestyle changes following a recent hospital admission.

The Flintshire Wellbeing service aims to support individuals for a period of up to 6 weeks following hospital discharge. We work collaboratively with individuals to help them identify their strengths, overcome limiting beliefs, and develop practical strategies for achieving their goals at home and within their community.

A key element of this role will be to work with the individual and co-design a personalised action plan. From this information, meaningful functional goals and outcomes are developed with the individual, to promote wellbeing, autonomy, independence, and choice. This approach aims to ‘enable people to be and to do what they have reason to value’

You’ll provide expert nursing advice, support and information on Parkinson’s and related conditions to people contacting Parkinson’s UK national helpline. 

You’ll empower people affected by Parkinson’s, their families, friends and carers to live lives that are as fulfilling as possible whatever that means to them; and that they feel able to take an active role in their treatment and become their own advocate in health and life, wherever possible.

What you’ll do:

• Provide expert telephone nursing advice, support and information on Parkinson’s, signposting clients to appropriate internal and external resources as necessary.

• Develop and update professional knowledge of all aspects of Parkinson’s and Parkinsonism to ensure that the information and advice provided is accurate and based on best available evidence.

• Undertake national helpline work to ensure cover across the helpline’s working hours and in line with guidance and as part of a dedicated team.

• Maintain your own knowledge of the work of the charity and other health and social care providers.

What you’ll bring:

• Ability to provide information on Parkinson’s medications without prescribing or recommending specific drugs. 

• Knowledge of the importance of raising safeguarding concerns.

• Experience of providing telephone support, including use of active listening and questioning skills, and demonstrable digital competence, including online case management systems

• Able to work collaboratively and in partnership with others.

About the role:

The Foundation/Domestic Abuse Prevention Practitioner (FDAP) will play a key role in working directly with individuals using harmful/abusive behaviour within their intimate relationships. The role will include suitability and risk assessments, delivery of interventions, facilitation of group programmes, risk identification, information sharing and preparation of reports for courts and other agencies.

The FDAP Practitioner will be co-located into Priority Crime Teams situated in Newport Police Station to assist with the identification, screening, and engagement of individuals using harmful behaviour in their relationships, working closely with police to provide a timely and streamlined process for earlier identification, referral, and engagement/disruption of high harm perpetrators of domestic abuse.

For this role is it essential you have excellent understanding of domestic abuse, including the impact on victims and their children. Experience of working with individuals experiencing domestic abuse/ and or causing harm. Experience of group work delivery is desirable; however, we will consider applications from candidates who demonstrate an aptitude for the role.

You will need to be able to build and maintain effective working relationships with statutory and voluntary agencies to deliver the best possible outcomes within a multi-agency setting. As well as being tenacious in support of your clients and adept at problem solving.

IT literacy is a must and candidates must evidence both the ability to use bespoke packages and excellence in their recording practice.

Through the partnership of Macmillan Cancer Support, Self Help UK and the Nottinghamshire Integrated Care System, the Macmillan Beyond Dagnosis service delivers support for people affected by cancer, as well as facilitated access to voluntary and community sector cancer specific support and wellbeing interventions across Nottingham and Nottinghamshire. Support and interventions will be identified using electronic health needs assessments (eHNA’s) and personalised care and support planning. This will provide a structured and clear ongoing support plan for those living with and beyond cancer which are integrated with primary, secondary and community clinical care pathways.

The aim of the service is to provide an improved experience for patients beyond their interactions with clinical services by contributing to the overall holistic approach to supporting people living with and beyond cancer. This will integrate the ‘beyond medicine’ approaches offered by the Voluntary and Community Sector (VCS) into existing and developing cancer pathways.

The Macmillan Beyond Diagnosis Service also includes the NUH Cancer Prehabilitation service, where we provide the Beyond Diagnosis model through the Social Prescriber Link Worker role.

In October 2022 Salford City Council commissioned Salford CVS to undertake research into the strengths of the Voluntary, Community and Social Enterprise (VCSE) sector as part of their Joint Needs and Strengths Assessment.

Every three years since 2008, Salford CVS leads on State of the VCSE Sector research in Salford and also across Greater Manchester. Working with other local infrastructure organisations, we aim to provide a comprehensive overview of the VCSE sector. Our last research report was published in 2021 and we have started planning for the research to be conducted this year.

In August 2023 we further developed our VCSE sector research and insight work with the commencement of our Salford 99 longitudinal study. This research project is designed to understand the issues affecting the VCSE sector in Salford and aims to track 99 VCSE organisations every six months for a three-year period, so that we gain ‘live’ insight in between our three-yearly State of the VCSE sector large research project. Alongside this work we conduct a Pulse Check survey with a variety of VCSE organisations to get a snapshot of what is changing for them each quarter.

This exciting VCS Research and Insight role involves a combination of VCSE engagement and insight work across Salford.

Job Title: Caring Dads Practitioner

Hours: 22.5 hours per week across 3 days.

Caring Dads exists to change current practice to better include fathers in efforts to enhance the safety and well-being of their children. The practitioner will work directly with fathers with an aim to improve relationships with their children, families, and co-parents/carers. This will include promoting the service to a range of teams across Hampshire and Southampton, to generate programme referrals. You will work closely with Hampton Trust domestic abuse perpetrator service to identify client suitability for the Caring Dads programme.

As the Caring Dads Practitioner, you will be required to deliver the Caring Dads and Accredited Domestic Abuse Prevention Training (ADAPT) Programme. This includes one to one support and group work. Groups are delivered on weekday evenings; you will be required to deliver one group work sessions per week.

You will undertake treatment and risk assessments of individuals causing harm in their relationships; this includes active case/risk management of families living with domestic abuse. Although this position is based in Ashurst, you will be co-located into social work teams across Hampshire three days per week, with an aim to providing support and advice in identifying suitable families for the Caring Dads and ADAPT programme.

This post is an exciting opportunity to work with Nottingham City’s PCN 8 (Clifton and Meadows).. Using the Macmillan Electronic Holistic Needs Assessment to support Cancer Care Reviews will ensure patients have access to the right support for all their non-clinical needs. The post will also work on the preventative agenda, supporting uptake of cancer screening across the PCNs and supporting the practices within the PCNs to deliver their improvement plan around early diagnosis of cancer. (in line with NHS England and NHS Improvement -  QOF Quality Improvement domain 20/21 – Early diagnosis of cancer).

 You will work in partnership with GP surgeries to give non-medical support that maximises patients’ wellbeing through their Cancer journey. The primary role is to do Cancer Care Reviews, with access to each surgery’s IT systems to track patient lists, document, and communicate with practice staff. This role will involve liaising with services from within the NHS structure as well as external services, so you will need to be skilled in navigating intricate processes and organisational structures from both the inside and the outside. You will also help with improving screening uptake and community engagement, building relationships with local services and groups to further enhance the support.

You will be working with children and families to provide trauma informed support with the aim of reducing trauma symptoms and rebuild relationships.

KEY RESPONSIBILITY

To work with children (aged 5 – 16) and their families to provide trauma informed support with the aim of reducing trauma symptoms, rebuilding relationships fractured by domestic abuse and helping the non-abusive parent to understand the impact of domestic abuse on their children so they are better able to support them.

DIRECT WORK WITH CHILDREN AND THEIR FAMILIES

• To understand and assess the needs of children and families and devise a child-focused support plan
• To utilise evidence-based approaches to strengthen attachments and rebuilt relationships with the non-abusive parent that have been fractured as a result of domestic abuse
• To utilise trauma informed ways of working to reduce trauma symptoms and distress in children
• To work with families in order increase stability and reduce risk 5
• To assist the non-abusive parent / carer in their understanding of the impact of domestic abuse so that they can respond appropriately to the emotional needs of the children
• To attend and initiate mutli-agency meetings where required
• To ensure that children and families receive holistic support that is appropriate to their needs, including advocacy and referring to other services
• To deliver programmes of child-focused group work
• To understand risk and safeguarding issues and act appropriately in relation to risk at all times.
• To work within national, county and ESDAS guidelines, particularly in relation to good practice regarding children
• To provide written reports and case studies to senior management as and when required.

You will receive support and training and a competitive starting salary of £20,000 for a 28 hour week. Reduced hours may be possible by agreement. This is a two-year fixed term post with the possibility of becoming permanent after two years.

We are looking for someone who will work with professionalism, empathy and kindness, have knowledge of working with children and families who have experienced trauma due to abuse and have a trauma informed, child focused approach. Experience of working with complex family situations where there are safeguarding concerns with the ability to assess risk is required.

You will be based at our office in Redhill and you must be able to reliably commute to the office.
