Based at the 999 club in Deptford, a safe and welcoming space for those experiencing homelessness Move+ is the latest of the 999 club’s service offerings which also include a drop-in day centre, employment & training support, advocacy & advice, a women’s space and the Bridge, our evening service for people in mental health crisis.

This is an exciting opportunity to be part of a cutting-edge service within the sector. The overall aim of Move+ is to enable people who have been homeless to draw upon their shared experiences and support one another to escape homelessness for good. Move+ empowers people to live independently, to set up and maintain a stable home, while working towards long-term goals and aspirations enabling them to move forwards with their lives. This is achieved through a combination of peer coaching, personalised support and an exclusive program of workshops and activities.

Working closely with the Coproduction Manager as part of the Specialist Services team, you will oversee all aspects of the day-to-day delivery of Move+. This includes line management of a team of Peer Coaches, focusing on support, personal and professional development. You will conduct assessments and manage referrals, build and maintain relationships, as well as promote the service, both internally and externally. Overall, you’ll ensure a high quality of service delivery that meets or exceeds targets and, most importantly, supports and empowers all participants.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
• To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
• To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.
• To at all times undertake your role in a professional manner maintaining a high quality standard of work, and to always work in accordance with the aims, values and ethos of The Passage.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.

• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.

Night Reconnection Assistant - Rough Sleeping Prevention Service

London, Islington, Lambeth and Kensington and Chelsea

Are you looking to start your career supporting vulnerable adults; working for a charity who makes a real difference to the lives of people who are homeless every day?

Our client's exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.

Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.

The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected to work across all 3 services . 

• 7 day rolling rota including early, mid and late shifts
• (K&C): medium – high support accommodation.
•  (Lambeth): medium support accommodation.
•  (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.

Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.

In these roles you will: 

• Work as part of a busy team environment working closely with those affected by rough sleeping. 

• Provide a safe and supportive environment at night to clients; and support a team of Assessment and Reconnection Workers, with key tasks; such as implementing reconnection or assessment plans and supporting clients into appropriate accommodation.  

About you

• These crucial roles will suit people with an interest in in developing key skills to build a career working in a support role, as well as:

• Some experience of dealing directly with the public and / or customers in a busy service environment. 

• Good communication skills and the ability to work with others effectively to get things done.

• Some understanding of the issues faced by homeless or vulnerably housed people and the difficulties they experience in accessing services.

• You don’t need to have direct experience, above all we are looking for inspirational, committed individuals who have a genuine desire to support people to transform their lives.

As an Independent Domestic Violence Advocate you will:

• Implement effective ways of working with victims and those supporting them to increase safety and reduce harm.
• Provide a high-quality, front-line service to victims of domestic abuse, delivering a premium service prioritised according to risk, primarily focusing on victim/survivors aged 16+
• Work within a multi-agency framework consisting of the MARAC and local partnership responses to domestic abuse to keep safety central to all services for victims of domestic abuse.

• Have a knowledge of help-seeking barriers and support needs of victims of domestic abuse (in heterosexual or same-sex relationships).
• Have a good understanding of domestic abuse including the impact of domestic abuse on victims and their children.
• Understand the principles of risk assessment, safety planning and risk management for victims of domestic abuse and their children
• Understand safeguarding issues, and the legal responsibilities surrounding these issues.
• Direct service delivery to victims of domestic abuse or other vulnerable people.

What the job involves

This is an exciting new role in Prostate Cancer UK’s Health Information and Clinical Support team. The team delivers a wide range of high-quality services directly to anyone affected by prostate cancer or prostate disease, as well as supporting a range of activity charity-wide that requires clinical expertise.

Many men, and their friends and family, don’t know the risk factors of prostate cancer – and when they do, they don’t know what to do next. We want a world where every man over 45 in the UK understands their risk of prostate cancer and has the tools and information to navigate it. Our online risk checker is a good place to start, but this role will be a crucial additional source of information for men and their loved ones. 

As a Risk Information Officer, you’ll deliver the charity’s pilot Risk Information Service, providing information and support to men at risk of prostate cancer, and their loved ones, via the telephone. This is a non-clinical role, but you’ll discuss callers’ risk of prostate cancer along the lines of the predominant risk factors (age, ethnicity, family history and BRCA gene status). If necessary, you’ll also support them to seek a PSA blood test from their GP and on occasion routing callers who have more complex clinical questions back to the Specialist Nurse service.

You’ll play a key role in developing the pilot – contributing to conversations and decisions around how to adjust and improve the service in its early months. You’ll also report back on your calls, to help the charity develop its broader understanding of information and support needs when it comes to prostate cancer risk and the PSA blood test.

Reporting into the Clinical Services Senior Officer, you’ll have close and regular contact with assigned Specialist Nurses so that you feel at ease in sharing prostate cancer related information with callers, and in maintaining the boundaries and scope of the service day-to-day.

This is a service pilot with the Risk Information Officer role being a fixed term for the duration, with the potential to make the role permanent based on the pilot’s success.

You’ll be a compassionate and resilient communicator; you may be speaking to a caller who is anxious about their risk of prostate cancer. You’ll be a great listener, hearing callers concerns and offering a supportive and empathetic response. Although this is a non-clinical role, with your existing understanding of the healthcare system in the UK, you’ll be willing to learn the basic knowledge of prostate cancer and use the appropriate associated terminology.

You’ll be comfortable in handling more complex conversations, but with self-awareness to recognise your limitations in offering support and recognising clear boundaries of guidance being provided and be able to direct to our Specialist Nurses when needed.

Experience in using a CRM system will be advantageous in this role and any experience with a helpline or providing support or information over the phone will be beneficial in your role.

Aside from the 50+ young people we see every day at our day centre, New Horizon’s Outreach service also works with young people on the streets and in the community. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. The Outreach Worker will work as part of Rough Sleeping Team to deliver at least two street outreach shifts per week and transitional casework to young people facing rough sleeping. You will need to be willing to work early mornings and late evenings, to travel across London to identify young people who need our support, and to work flexibly to respond to evolving needs. Job Purpose

Money Minded is a new service designed to support adults (18 years and over) in Ealing, Hammersmith & Fulham, Hounslow. This role offers an exciting opportunity to collaborate closely with the West London NHS Mental Health Teams, operating within the tri-borough area.

The service provides support with welfare benefits, accessing debt support, housing and well-being. The role of the Mental Health Information and Advice Officer will be to provide advice, guidance and customer representation to tri-borough residents living with Mental Health difficulties. Advice issues may include housing, welfare benefits, council tax, accessing debt support, signposting etc.

The successful candidate will deliver support to enquirers and manage a caseload of individuals, as agreed with the Service Manager. In addition to advice casework, the Mental Health Information and Advice Officer will also be expected to assist onboarding referrals to the service, as and when needed.

The successful candidate will work closely with the West London NHS Mental Health teams and project volunteers, ensuring adherence to the Advice Quality Standard framework (AQS) and Mind's policies and procedures.

Due to the nature of the work, if successful, you will need to have an up-to-date DBS check completed, prior to undertaking unsupervised client work.

The Mental Health Information and Advice Officer will be required to work within the West London NHS Mental Health team offices.

About the role:

This role will be part of our IDVA team in the London Victim and Witness Service. As a DRIVE Independent Domestic Violence Advocate you will:

• Address the safety of victims at high risk of harm
• Serve as a victim's primary point of contact by working with their clients from the point of crisis to assess the level of risk, discuss the range of suitable options and develop safety plans.
• Be pro-active in implementing the plans, including actions from MARAC, which address immediate safety and enable victims to protect themselves and their children, as well as longer term solutions, using sanctions and services via criminal and civil courts, housing options and other organisations.

You will need:

• Have a knowledge of help-seeking barriers and support needs of victims of domestic abuse (in heterosexual or same-sex relationships).
• Have a good understanding of domestic abuse including the impact of domestic abuse on victims and their children.
• Understand the principles of risk assessment, safety planning and risk management for victims of domestic abuse and their children
• Understand safeguarding issues, and the legal responsibilities surrounding these issues.
• Direct service delivery to victims of domestic abuse or other vulnerable people.
• Have excellent communication, negotiation and advocacy skills, both written and verbal when interacting with a range of agencies and individuals. Understand and be committed to equal opportunities and diversity issues in policy and practice.
• Have a strong crisis management skills and the ability to deal with stressful and difficult situations.

Assessment & Reconnection Worker - Rough Sleeping Prevention Service 

London,  Islington, Lambeth, Kensington and Chelsea

Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?

Our client's exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.

Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.

The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected work across all three sites.

• 7 day rolling rota including early, mid and late shifts.
• (K&C): medium – high support accommodation.
•  (Lambeth): medium support accommodation.
•  (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.

Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.

The service will work in partnership with the local authority to secure onward move-on options e.g. access the council PRS scheme, advocating for access to supported housing.

As an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with those new to, or at risk of rough sleeping. 

• Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.

• Build and maintain effective relationships to support clients throughout their recovery. 

About you

These roles will suit people with experience of working at pace in a public facing environment with the ability to adapt to new situations; as well as:

• An understanding of the issues faced by homeless or vulnerable people and the difficulties they experience in accessing services.

• Good communication skills and the ability to work with others whilst managing your own time.

• The ability to manage complex behaviours and to deal with people who may be in a difficult personal or emotional situation. 

• Above all we are looking for inspirational, committed individuals who have a genuine desire to support people to rebuild their lives in these key roles; many of their managers began their careers as an Assessment and Reconnection Worker.
