What you'll do:

* Undertake key-working responsibilities for a caseload assigned by the Manager
* Provide support 9am-5pm 7 days a week
* Assess prospective discharges within 3-5 hours of receiving referral information.
* Support the customer to settle in and meet their immediate social care needs.
* Update the Discharge/Patient Flow Team on patient progress on a weekly basis.
* Escalate concerns to the Discharge Team and urgent concerns via the Crisis Line.
* Proactively monitor customer's mental state, ADLs and move-on plans.
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
* Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
* Support customers to access activities and services in order to maintain and improve well-being, access benefits and reduce isolation.
* Maintain regular communication with Hackney Council (Local Authority) to facilitate re-housing.
* Develop and maintain links with all key agencies and service providers in the local community.
* Empower customers to ensure they receive the service and benefits they are entitled to
* Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.

KEY RESPONSIBILITIES

Support and Resident Care

Safety and Security

· Ensure resident safety by implementing visitor procedures and preventing unauthorised access.

· Monitor CCTV, address antisocial behaviour, and complete relevant reports

· Report repairs and any health and safety issues

Incident and Emergency Procedures

Night staff must be familiar with emergency protocols, including fire safety, evacuation, and business continuity plans, ensuring resident safety and compliance with statutory and contractual requirements.

· Update INFORM with relevant details of all resident interactions during shifts.

· Maintain accurate and comprehensive resident records to professional standards.

The main purpose of the role is to prepare individuals to move on to independent living in the community. Service users may step down from the intensive support service to high, medium or lower needs before moving into the community. From the outset of an individual's tenancy the focus of the support is on preparing them for their journey of recovery and independence over an agreed period. The role requires the ability to work both independently and within a team, including inducting new peer staff and volunteers, and collaborating with statutory services like the NHS. Candidates must be able to work flexible shifts, including early mornings, late evenings, weekends, and bank holidays, to meet 24-hour service needs.

Good IT, literacy, and numeracy skills are essential, along with an awareness of health and safety in the workplace.

We are looking for a dynamic and experienced Adults Peer Support Worker to join our busy Peer Support team. You will have your own lived experience of mental health issues and be able to use this experience as well as your professional skills and knowledge to support others.

RB Mind has run effective peer-led services for many years. The role will be supported by RB Mind Senior Peer Support Workers, and the Peer Support Coordinator in South West London St George’s Mental Health Trust (SWLStG). With the expansion of our work in this area we aim to reduce waiting times, reduce the number of people going into crisis, address inequalities and support transitions from youth to adult services. We offer holistic and personalised care and support to service users.

The role has its own caseload and provides a range of support and person-centred interventions to meet these needs, which could include group support, one-to-one interventions and signposting where specialist advice is needed.

We are looking for someone who is passionate about motivating and coaching individuals with mental health issues, and able to support people on their journey to recovery. As well as having an empathy with the core values of RB Mind, the successful applicant will have the skills to provide person-centred support through one-to-one sessions and facilitating groups. You will have an awareness of the boundaries we work within when supporting vulnerable adults. You should be enthusiastic and self-motivated, and must be able to work on your own initiative.

This is a partnership project with South West London St George’s Mental Health Trust (SWLStG), locally led by Richmond Borough Mind.

We are seeking highly motivated Relief Support Workers to support people with autism spectrum conditions and other needs living in a bespoke supported living service in Little Thurrock to enjoy an excellent quality of life, wellbeing and opportunities for personal development. The service will offer an empowering, strengths-based model of support which enables people to lead fulfilling and valued lives, maintain their tenancies, participate in their local community, develop independent living skills and achieve their personal goals and desired outcomes.

Your role will involve providing care and support in a personalised and responsive manner. You will provide respectful personal care and support to enable people to develop their quality of life and independence. This will include providing support to build confidence and skills in all activities of daily living. You will enable people to establish a positive and meaningful lifestyle, build community connections and relationships with others.

Prior relevant paid or voluntary work experience of supporting people with autism, a learning disability and/or mental health needs is essential. We are looking for individuals who have the following personal qualities and attitudes:

This is a Relief Support Worker position which enables you to pick up shifts at a time that suits you, which is ideal for fitting around personal commitments, study or caring responsibilities. The flexible shift pattern includes evenings, weekends and sleep-ins.
