The Care Navigator will support those with long term conditions to access timely care and community support services. Working closely with Camden GP practices and Integrated Care Teams is required as is accurate record keeping using IT systems. Through timely intervention and care co-ordination, unnecessary hospital admissions, GP appointments and residential placements will be reduced.
The successful candidate will come from an information/advice, social care or health background, must be computer literate, and have excellent people and communication skills, and able to work in a multi-disciplinary team. Experience of person centred planning is required. It is important that a candidate promotes a culture that values, protects and uses information for the success of the organisation and benefit of its clients/patients. For this role it is essential that you to have the ability to travel across Camden as the role involves visiting clients in their own homes. This is a full time role with no job share opportunity and works out of GP practices in Camden. AUC offers a contributory pension, and season ticket or bicycle loan facility.

Role Summary
The Graphic Designer plays a key role in shaping and maintaining Alcohol Change UK’s visual identity. This role is responsible for producing engaging visual content across multiple digital and print channels to inform, engage, and inspire our audiences, and across our flagship campaigns including the Dry January® challenge. Sitting within our Marketing and Communications department, the postholder will work closely with internal teams and external partners to ensure that all creative outputs align with our brand identity and communication goals. We are looking for a proactive self-starter to sit within a high-performing team working to raise awareness of SSAFA’s support in the North Region of England to those in the Armed Forces Community who need it most.
With the ultimate objective being to grow SSAFA’s presence, profile, and identity, the role is responsible for all marcomms activation in the region. The role holder will work to deliver the central Fundraising, Marketing & Communications strategy.
This role reports to the Head of Communications with close alignment with colleagues across the marcomms directorate. The marcomms manager will be responsible for managing marketing campaigns in the North Region of England and promoting SSAFA’s fundraising initiatives and services. You’ll identify the areas of greatest impact and cut through, deliver integrated awareness campaigns and secure media coverage of fundraising events. Crucially, you’ll be able to identify stories which resonate with our target audience and write calls to action.

About the team
The Communications team is a small but dynamic group of individuals with strong skillsets across Public Relations, Storytelling (case studies) and Internal Communications. The team is a mix of ex-forces and civilians, and they are always sharing knowledge and expertise with each other.
You will work closely across all directorates within SSAFA, with a focus in the North Region of England, to garner information and create compelling marcomms campaigns and activities to raise awareness of SSAFA’s work.

As a natural storyteller with strong media instincts, you will excel at securing high-profile coverage and elevating our brand presence. In this role, you’ll be responsible for:
The Lead Coach is primarily responsible for overseeing the training room and group sessions with young people. You will help shape the training environment and will confidently adapt and deliver materials and sessions to work within the group’s development towards work readiness.
Delivery of Spear Programme - Group and 1:1 coaching [60%]
- Preparing and coaching Spear Foundation sessions and 1-1s, ensuring that young people are engaged, retained and work-ready by the end of this initial phase of the programme

- Adapting the Spear coaching material according to the needs of the Trainees and managing behaviour effectively; handling safeguarding

- Liaising with volunteers for key programme engagement events such as mock interviews and external panels. Leading on briefings and communications with these volunteers
Delivery of Spear Career [20%]
- Delivering weekly Spear Career sessions (including running workshops with past Trainees), maintaining records and reporting on statistics, and creatively developing ongoing relationships with Spear Trainees
Training the Graduate Coach [10%]
- Contributing towards the training and developing of the Graduate Coach by modelling excellent coaching and giving regular feedback
Trainee recruitment [10%]
- Developing relationships with Job Centres and local referral agencies to ensure that the maximum number of Spear Trainees are enrolled on each programme. This involves liaising with referral agencies, active face-to-face recruitment and monitoring recruitment statistics

Support Coordinator
We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in Lincolnshire. This is an exciting opportunity to work with stroke survivors and their families to provide stroke recovery support following a stroke.
There are two positions available:
1 x 28 hours per week – This is a fixed term contract until 31 March 2026

Position: S11265 Stroke Support Coordinator
Location: Home-based Lincolnshire, however, frequent travel will be required as part of this role (may include team meetings or other work-related meetings)
Salary: Salary circa £21,948 per annum for 28 hours per week and £16,461 per annum for 21 hours per week (FTE circa £27,435 per annum)
Contract: Services are contracted and there is currently funding for this contract until 31 March 2026.

The Role
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A key part of the role will be to support service users and the delivery of the service.
Reporting to the Stroke Service Delivery Coach, the Stroke Support Coordinator will:
• Support new stroke survivors and their carers from hospital discharge into the community.
• Take a person centred approach to goal setting and support to enable stroke survivors and their carers to improve communication
• Provide personalised information, advice and support.
• Support stroke survivors to make informed lifestyle changes, which will help them to prevent further strokes.
• Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
About You
You will have:
• Experience/background in a caring profession, ideally supporting people with disabilities
• Excellent IT skills and an ability to maintain accurate records.
• An affinity with the values of the Association.

This role reports to the Head of Communications with close alignment with colleagues across the marcomms directorate. The marcomms manager will be responsible for managing marketing campaigns in the South West Region of England and promoting SSAFA’s fundraising initiatives and services. You’ll identify the areas of greatest impact and cut through, deliver integrated awareness campaigns and secure media coverage of fundraising events. Crucially, you’ll be able to identify stories which resonate with our target audience and write calls to action.
With a heavy focus on media and PR activity, key to the role is building strong relationships and contacts with relevant stakeholders in your region including media representatives, employees, volunteers, and case study beneficiaries.
About the team
The Communications team is a small but dynamic group of individuals with strong skillsets across Public Relations, Storytelling (case studies) and Internal Communications. The team is a mix of ex-forces and civilians, and they are always sharing knowledge and expertise with each other.
You will work closely across all directorates within SSAFA, with a focus in South West, to garner information and create compelling marcomms campaigns and activities to raise awareness of SSAFA’s work.

Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. In some services, this will include providing physical, domestic, emotional and social care. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices.
The role involves working with clients with complex needs in a rota system that includes early, late sleep in, weekend and bank holidays
What you'll do:
- If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
- Participate in the support planning and risk management, as facilitated by the lead Support Worker
- Carry out security duties to ensure the safety of the customers and premises
- Monitor the CCTV throughout the shift
- Enable customers to make full use of community facilities by providing support as directed
- Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities

Support Coordinator
This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join a Stroke Recovery Service based in East Kent.
Position: S11278 Stroke Support Coordinator
Location: Home based, Thanet, East Kent, however, regular travel will be required as part of this role within the service area (may include other travel in London and nationally to include team meetings or other work-related meetings).
Salary: circa £16,461 per annum
Hours: 21 hours per week

The Role
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A key part of the role will be to support service users and the delivery of the service.

About the role
This role is a great opportunity for someone who is passionate about supporting those who struggle with complex needs, substance use, and dual diagnosis. We are looking for someone who can work flexibly to support residents and participants who struggle to engage with the support available to them through various providers. You will be at the heart of engaging our participants to regain motivation and the strength to step down from acute psychiatric wards and other supported living, by empowering them with the relevant skills and knowledge to support their journey. You will support them towards achieving their personal goals including independent living. You will provide pathways, support, and appropriate interventions that will enable successful resettlements, and deliver to achieve more safer, integrated communities for our stakeholders. The areas of support will include: Prevention, Transition, Activities of Daily Living (ADL), Support, and Social Inclusion.
This role will work alongside South London and Maudsley NHS Foundation Trust (SLAM) who provide the widest range of NHS mental health services in the UK. They also provide substance misuse services for people who are addicted to drugs and alcohol with 4,600 staff serving a local population of 1.3 million people.
- Support a caseload of participants, contribute to the development of support plans, risk assessments and reviews.
- Positively promote the pathways available towards independent living within the community. Ensure participants understand what tools and resources are available to them to regain independence.
- Participate in, and encourage R&Ps to participate in the running and development of projects, initiatives, training and volunteering.
- Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent a crisis.
