- To run an active caseload within the KPI requirements of the Bromley Well contract.
- To input cases on to the Charity log system accurately
- To ensure that services are accessible to carers.
- To participate in the provision of news and the production of the Carers newsletter
- To collaborate with other Bromley Well partners, including Mencap/MIND etc. to ensure effective appropriate referral pathways for quality service provision.
- To abide by and take part in the development and review of policy and procedure for the Carers pathway.
- At all times to maintain the professional integrity and reputation of the Charity and represent their main interests in any dealings with other bodies, groups and individuals.
- Attend staff meetings, supervision and organisational events as required.
- Collect case studies to help demonstrate AGE UK Bromley and Greenwich’s Adult Carers Support service’s impact.
- To undertake any other duties commensurate with the purpose and remit of the post.

This role offers an exciting opportunity to grow and develop a Dementia Service that was launched at Age UK Westminster in 2022. Offering a weekly community-based Maintenance Cognitive Stimulation Therapy group that provides a vital contribution to living well with dementia and a monthly Memory Cafe, we are looking for someone with the knowledge, skills, experience and passion to ensure that more residents can benefit from this Service.
Summary of role Age UK Westminster launched its Dementia Service in 2022 supporting older people (aged 50+) living in the City of Westminster who are experiencing memory and cognitive impairment and are experiencing confusion and dementia resulting in severe social isolation. The service also supports those who are caring for people with these issues.
In 2024 the ambition is to grow the reach of the successful monthly ‘Heart of Westminster' Cafe delivered in partnership with the Salvation Army’s Regent Hall and a weekly community-based MCST group and provide expertise to the Befriending team to manage some befriending matches. Demonstrating the value of these community-based activities and ensuring they are supported by a trained and supported volunteer team will contribute to securing continuation funding.
The role will involve close liaison with Westminster’s Memory Service and other community organisations that support older people living with memory loss and dementia.
Key responsibilities
● Co-ordinate the successful running of the monthly ‘Heart of Westminster’ Cafe in the Soho ward of the City of Westminster.
● Co-ordinate the successful running of up to two weekly MCST groups at Marylebone Church in Westminster including the co-ordination and triage of referrals for MCST groups
● Ensure the design and provision of activities and information that will enhance the lives of those attending and their carers. There is a significant resource bank of materials and activities
● Facilitate an environment where acceptance of dementia will bring about positive feelings for anyone affected by it
● Promote the service across Westminster to ensure that all those who would benefit are able to attend. This will include working with other professionals and ensuring that publicity materials are available
● Working with colleagues at Age UK Westminster and other professionals to identify and assess individuals who would benefit from being matched with a trained dementia volunteer. Home-visiting may be required.
● Deliver dementia awareness training to staff and volunteers
● Working with Age UK Westminster’s Volunteer Coordinator grow the volunteer resource
● Provide ongoing support and supervision to the team of Dementia volunteers
● Ensure that all records are kept up to date for the service and be responsible for the implementation of monitoring, evaluation and quality assurance procedures

About the role
Contract: 6 Month fixed term contract.
Location: Based in Darlington with homeworking elements.
As a Dementia Adviser, you will provide person-centred and outcomes focused information, advice and support services to people affected by dementia and memory loss symptoms in Darlington. You will identify their needs and the services they may wish to access, support the improvement of their sense of wellbeing, putting them in more control of their lives.
You may need to work with a variety of other professionals, such as GP’s, memory clinic staff and other health and social care partners, and support may be provided in person, over the telephone, via virtual options or in writing. Alzheimer’s Society have an exciting opportunity to join our talented team as a Dementia Adviser in the East Yorkshire area. We are working hard to improve diagnosis rates for dementia and this exciting role involves organising and attending ‘Brain Health Pop Ups’ around the whole East Riding to support people worried about their own brain health. Giving advice about how to approach their GP to gain a diagnosis and supporting people in a relaxed and supportive environment (usually libraries, leisure centres and garden centres). Working closely with the East Riding team and signposting for referrals is a key part of the role also.

About the role
Location: community-based in Bridgend with some office and home-based working.
We have an exciting opportunity to join a multi-disciplinary team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance and support to people affected by dementia in Bridgend. You will help them in identifying their needs, maintain independence, improve their sense of well-being, and putting them in more control of their lives. This post forms part of the Team working around individuals and their families who are experiencing difficulties with changes to cognitive, emotional, or physical health needs.
This new post will work as part of the Integrated Community Network teams in Bridgend. The Network Teams consist of staff across several professions. This includes social work, district nursing, community psychiatric nursing, occupational therapy, physiotherapy, speech and language therapy, dietetics and pharmacy. It aims to provide a seamless and holistic approach to an individual’s care or support needs during times of change or challenge.
As a Dementia Adviser, you will:
- Work closely with health, adult social care and third sector colleagues in assessing need and providing information and guidance to support people affected by dementia.
- Help meet the health and wellbeing needs of an individual by looking at “what matters” to them by providing timely and accurate information, advice, and assistance so that they may benefit from what is available to them.
- Work within the Integrated Community Network Teams in supporting the provision of support to people with dementia and their carers and wider support networks.

About the role

This is a home-based role, with local community engagement and home visits to service users. Job Purpose: To develop and manage, to a high standard, Age UK Bromley & Greenwich’s Information and Advice Service. To be responsible for the effective delivery of the I&A Service which will be delivered through a range of channels, including face to face, telephone, email, digital, and other means, across the Bromley and Greenwich boroughs.
Key Responsibilities:
Line management and supervision
- Provide line management and supervision to advisers and support staff.
- Supervising team advice sessions and casework
- Manage the practicalities of the advice session and ensure adequate staffing and resources.

- Ensure referrals are actioned and clients are supported in a timely manner.

Supporter Engagement is one of the most important teams that is central to the success of Alzheimer’s Society. The function of the team is to manage multi-channel communications with our supporters, both inbound and outbound.
