Purpose of the role:
Croydon Health & Wellbeing Space (CHWS) is an exciting and dynamic service, run in partnership with Mind in Croydon and Croydon BME Forum and commissioned by South London and Maudsley Mental Health Service (SLaM). The CHWS opened in 2022 with the aim of providing advice, information, and specialist support to Croydon residents with mental health concerns, who drop-in, or have been referred to the via the local Integrated Care Network Plus multi-disciplinary clinical and community mental health support teams.
The Advice Worker will be part of a friendly team, including support workers, crisis workers and clinical mental health professionals and function as the first point of access for visitors to the CHWS. Following an initial conversation to ascertain the clients’ needs, the Advice Worker will provide information and advice, as well as signposting and referrals to specialist services, such as the Mental Health Personal Independence Co-ordinators, Social Prescribers and Community Link Workers, in addition to the wider voluntary and statutory services in the borough. The role will be fast paced and vary each day offering a fantastic opportunity for the postholder to develop their knowledge and skills and make a difference to the users of this service.

East London Apartments provides 24 hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).
6 week rolling rota with early, late and some weekend shifts.

YMCA West Sussex Transitional Housing provides safe, secure accommodation for young people aged 18-25, who have been, or are at risk of homelessness. Our staff team comprises of Supported Housing Support Workers, Housing Mobile Night Workers and two Supported Housing Managers. We offer a support package alongside the accommodation to actively encourage residents into education, training, employment or voluntary work.
In delivering the role, you will work with the Deputy Supported Housing Manager and the wider housing team to provide effective day to day management of our transitional accommodation service. You will have your own caseload of residents, working creatively and effectively alongside young people to develop their assets and talents and to acquire the skills they need to live independent and fulfilling lives. About you

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.
As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.
You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.
There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. About the Role
- Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accmmodation for 15 women who have experienced homelessness and multiple disadvantage.
- The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation. The accommodation also includes overnight safe spaces.
- In this role, you will provide direct support to all the women, as well as holding a small case load of residents who you will keywork.
- You will work closely with support workers and the in-house psychologist.
- You will model person-led, psychological, gender, and culturally informed support, will be skilled at building trust and you will be a great team player.
- The post holder will undertake 8-hour shifts, normally Monday to Friday but with flexibility for weekend working, covering the hours 8am to 8pm. This is a 1 year fixed term contract.

Work proactively with the team to handle the service caseload and support an effective team approach to meeting each customer's identified needs in a recovery-focussed way
Assessing referrals received by the service, within the 4-hour target, and providing a rapid response to these - supporting the team to identify and promote positive risk management.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns
Inducting new customers, and helping orient them to the community and local area
Completing/supporting housing referrals as required, supporting customers with budgeting tools and providing support to customers in accessing any benefits or addressing any issues such as benefits.
Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.
Support planning, including ward management liaison, medication profiles and medication reviews as required, identifying the goals and steps to support customers to manage their own support and risk and ensuring these are correctly documented within Look Ahead's paperwork.

Hours
Average 40 hours a week on a five-week rota. Normal working pattern will be working across early and late shifts & working two weekends. Pension

Felstead Street is a CQC registered care home, which has a heavy focus on supporting customers with enduring mental health care and elderly needs. We provide 24hr high support to 24 customers both female and male. Our building is a purpose-built home on two floors, offering customers individual bedrooms and the use of shared communal facilities and gardens. The service is commissioned by East London Foundation Trust with the care contribution from London Borough of Hackney.
As a Support Worker at Felstead Street you will play an integral role in supporting our customers to making their lives interesting and enabling to manage their social, physical and emotional well being. You will support our customers with a range of activities and health support throughout the day and to live in a safe living environment. You will be part of a team which is very supportive of one another.
The successful candidate must be able to work some weekends and bank holidays as allocated. This role involves working 5 days a week. Each shift is 8 hours, 8am-4pm or 2.30pm-10.30pm, as allocated on the rota. You must be able to work 9am-5pm for 2 weeks every several weeks as required by the rota.
