Support those who Care
This is an exciting opportunity to join our service supporting Carers across City of London to improve their health and wellbeing, and access support that meets their needs. We are working with Carers from a range of cultures, and it would be an advantage to speak Bengali.
The team work holistically with Carers to understand their individual circumstances and level of need. You’ll carry out home visits and community-based meetings, co-produce support plans, and develop effective relationships with Carers, families, and professionals. You will also support Carers to complete written forms and applications, draft letters, and advocate with third parties.
You’ll build on your local knowledge to signpost and facilitate access for Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Together with colleagues, you will plan and deliver the service’s activity package, including coffee mornings, drop-in sessions, activities, online and face to face groups, and peer support forums. There will also be opportunities to promote the service by attending meetings and forums, and to deliver training sessions to professionals and community organisations so that they have the skills to identify Carers.
Applicants should have relevant personal or professional experience of working with Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector.
This is a full-time position, and you should be an excellent communicator and able to motivate and empower others. You’ll need a practical understanding of barriers faced by Carers, and an awareness of cultural differences and access to services.

Are you passionate about supporting vulnerable adults during critical moments in their care journey? As a Hospital Care Navigator, you’ll be a vital part of ensuring patients leave the hospital safely and confidently, with the right support in place.
In this rewarding role, you’ll be the single point of contact for adult patients preparing for discharge from the hospital. You’ll work closely with the Adult Social Care team, health professionals, hospital practitioners, and community services to:
- Identify needs and barriers to safe discharge
- Achieve safe and timely patient discharge
- Coordinate outpatient appointments and follow-up services
- Provide clear, compassionate advice and support to patients and their families
- Ensure patients understand their discharge plan every step of the way.
You’ll meet with patients, their families, friends, or carers to assess ongoing needs, ensuring a safe plan is in place for their return home. This could include ensuring that medication is arranged from the hospital pharmacy, someone is at home to greet them, food and heating are available, and the condition of the home is safe to return to. You may be required to liaise with Adult Social Care and other home providers to coordinate home support. You’ll follow up on calls or visits to ensure the patient is managing well at home. By providing the proper support, patients can be discharged sooner, and the risk of readmission is significantly reduced.
We’re seeking someone who:
- Has experience working with vulnerable adults in social care, healthcare, or the community/voluntary sector
- Communicates with empathy and confidence across different teams at different levels, and with patients
- Is highly organised, with excellent time management skills
- Has awareness and respect for cultural differences
- (Bonus) Experience working in a hospital or clinical setting.
The role is based at our Southwark office, and you will be required to travel to hospitals and across the City of London, as well as to Tower Hamlets, Camden, Hackney, and Southwark.

About the Role
We are seeking an experienced and highly organised Team Leader for our Young Carers and Young Adult Carers team. You will play a key role in shaping and guiding the service, managing a dedicated team and overseeing day to day delivery while working with the Head of Services to deliver our strategy. Essential to the role is ensuring the service is inclusive, responsive and shaped by the voices of the young people we support. This is a hands-on, people-focused role, combining team management, safeguarding oversight, caseload guidance and partnership working.
Key Responsibilities
1. Operational Coordination and Planning
- Coordinate the day-to-day delivery of the Young Carer Service in line with agreed workplans and available resources.
- Support the Head of Services in implementing strategic priorities and translating them into clear team actions.
- Plan and organise delivery schedules, ensuring one to one work, groups, clubs and school sessions are appropriately staffed and resourced.
- Manage delegated budgets and approve individual expenses in line with organisational procedures.
- Contribute to service planning and improvement by sharing insights from young carers, families and frontline delivery.
2. Service Delivery and Quality
- Oversee the daily flow of referrals, assessments and casework, ensuring timely allocation and consistent quality standards. Provide guidance to the team with their individual caseloads.
- Coordinate and monitor the delivery of group sessions, clubs and school-based activities to ensure they are safe, engaging and age-appropriate.
- Ensure there is adequate staff and volunteer cover to enable the schedule of sessions and activities.
- Maintain up-to-date and accurate information for families, schools and professionals, including content for the website and promotional materials.
- Support effective volunteer participation in groups, events and activities, working with the Volunteer Manager to ensure volunteers are appropriately trained, supervised and valued.
3. People and Team Development
- Provide day-to-day supervision, guidance and coaching to staff, ensuring they are supported, motivated and clear about priorities.
- Support the Head of Services with team meetings, supervision and annual appraisals.
- Ensure all team members are trained and equipped to the required standard, identifying learning needs and coordinating relevant development opportunities.
- Foster a positive, inclusive and supportive working environment, addressing interpersonal issues promptly and constructively.
4. Safeguarding and Inclusion
- Act as Deputy Designated Safeguarding Lead (DDSL), receiving, responding to and recording safeguarding concerns and escalating to the DSL (Head of Services) in a timely manner.
- Ensure robust safeguarding practices are embedded across all delivery settings, including schools and community venues.
- Champion equality, diversity and inclusion, ensuring activities are accessible, inclusive and representative of Buckinghamshire’s diverse communities.
5. Partnerships and Representation
- Maintain day-to-day relationships with schools, youth organisations, community groups and VCSE partners to coordinate delivery and share information.
- Represent Carers Bucks at local forums, events and school-based meetings as agreed with the Head of Services.
- Promote awareness of young carers and young adult carers and the support available through Carers Bucks, sharing information and good practice with partner organisations.
- Provide feedback from schools, young and young adult carers and community partners to inform service improvement and planning.
6. Performance, Data and Reporting
- Record and monitor service data accurately, ensuring compliance with GDPR and organisational procedures.
- Track team performance against agreed objectives and highlight areas requiring support or improvement.
- Analyse data and feedback to identify trends and contribute to quarterly and annual reports.
- Provide case studies and operational insights to evidence impact and support funding and reporting requirements.
About you
Qualifications and Experience
- Experience of supervising or leading a team.
- Background in youth work, education, social care or the voluntary sector.
- Experience of working directly with young people and/or unpaid carers.
- Experience of partnership working with schools, community organisations or health and social care professionals.
- Experience of handling safeguarding concerns and maintaining appropriate records.
- Experience of report writing, record keeping and using client databases or CRM systems.
Knowledge and Understanding
- Strong understanding of young people’s development.
- Robust knowledge of safeguarding principles for both children and adults.
- Understanding of equality, diversity and inclusion, and how to embed these in practice.
- Awareness of the issues affecting young carers and their support needs.
- Awareness of local services and support available for young people and families in Buckinghamshire.
- Understanding of how compassion, collaboration and reliability build trust and improve outcomes for carers.
Skills and Competencies
- Excellent verbal and written communication skills, adaptable across audiences.
- Strong organisational skills with the ability to prioritise and manage a varied workload.
- Ability to support, motivate and develop a small team through encouragement and shared learning.
- Confident in analysing information and using data to support service improvement.
- Competent IT user with experience of standard office packages and electronic record systems.

About the Role
We are seeking an experienced, motivated and highly organised Support Worker to join our dedicated Young Carers Service team. Our team of support workers focus on supporting young carers throughout their caring journey, ensuring they feel understood, valued and empowered whilst also providing the opportunities and practical support the young people need to thrive and continue their caring role.
As a Support Worker you will be responsible for assessing the needs of young carers and ensuring they receive appropriate support through Carers Bucks services or other local agencies. The role includes engaging in one-to-one support, delivering targeted group sessions and facilitating clubs and activities. Support Workers will identify and engage young carers through partnerships with schools, health and social care and VCSE communities.
Your role will be a mixture of working in the community, from home and in our Aylesbury office. Your principal geographic area will be South Bucks.
Key Responsibilities These include but are not limited to:
1. Operational Coordination and Planning
· Receive referrals and undertake YC assessments in your geographical area. Allocate young people to the appropriate level of support they require to fulfil their caring role.
· Provide planned and time limited one to one support for carers with clear outcomes, using tools such as ‘my life now’ to measure impact of support.
2. Service Delivery and Quality
· Deliver group programmes in response to the needs in your local geographical area, whilst working to available resources.
· Work with staff and volunteers to deliver regular clubs and activities which provide young carers with the opportunity to have time away from their caring role.
3. People and Team Development
· Build a strong relationship with young carers and families in your geographical area and work with the Young Adult Carer Support worker to transition them through services.
· Work effectively as part of a high performing team to ensure that needs of carers are met.
4. Safeguarding and Inclusion
· Have a strong understanding of how to receive, respond to and record safeguarding concerns and escalate to a line manager in a timely manner.
· Demonstrate a strong commitment to implementing and upholding Carers Bucks’ safeguarding policy in everyday practice.
5. Partnerships and Representation
· Provide support to schools in a geographical area to run young carers groups, targeted support sessions or drop in’s.
· Collaborate with the Adult Carers team and partner organisations to further support carers and make referrals to appropriate services accordingly.
6. Performance, Data and Reporting
· Maintain accurate and up to date records on all aspects of work carried out for monitoring and evaluation purposes.
· Produce case study summaries to evidence the impact and outcomes of work, in line with requests from the line manager.
About you
Qualifications and Experience
· Background in youth work, education, social care or the voluntary sector.
· Experience of working directly with young people and/or unpaid carers.
· Experience providing information, advice and guidance to young people and their families.
Knowledge and Understanding
· Strong understanding of young people’s development and the challenges faced by young carers.
· Strong understanding of safeguarding principles and demonstrated experience in implementing effective safeguarding practices to protect individuals.
· Understanding of how compassion, collaboration and reliability build trust and improve outcomes for carers.
Skills and Competencies
- Excellent verbal and written communication skills, adaptable across audiences.
- Strong organisational skills with the ability to prioritise and manage a varied workload.
- Competent IT user with experience of standard office packages and electronic record systems.

- Full UK driving license and access to own vehicle (travel across Bucks required.)
- Willingness to work flexibly, including evenings and weekends. Candidates will typically work Thursday evenings until 7pm (usually two times per month). TOIL will be provided for out-of-hours working.

About the Role
In this newly created role, you will work directly with the CEO, providing oversight and direction to Team Leaders to ensure that services operate effectively and meet contractual requirements. You will be passionate about making a difference to the lives of unpaid carers in Buckinghamshire, with the ability to make an impact quickly. You’ll bring proven hands-on experience in a similar role working with a team to establish effective and innovative ways to deliver services.
Main Responsibilities
1. Strategic and Operational Leadership
· Hold overall accountability for service performance and quality, working through the Adult, Young and Hospital Carers Team Leaders who lead day-to-day operations
· Ensure alignment with our strategic priorities and our commissioned outcomes.
· Contribute as an active member of the Senior Management Team, supporting organisational strategy, planning and delivery.
· Ensure that all carer services operate cohesively, with smooth transitions between age groups and consistent quality standards.
· Use data and feedback to drive service improvement and innovation.
2. Service Delivery and Quality
· Oversee the effective management of the Adult, Young, Young Adult and Hospital teams.
· Embed a culture of high performance, reflective practice and continuous learning.
· Ensure strong safeguarding practices are central in all services
· Ensure systems are used consistently for case management, data recording and reporting.
· Work with the Diverse Communities Worker and other leads to ensure services are inclusive, culturally sensitive and responsive to the needs of underrepresented groups
· Oversee the effective involvement of volunteers within carer services, working with the Volunteer Manager to ensure they are well-supported, trained and valued as integral members of the team.
3. People Leadership and Team Development
· Foster collaboration and a supportive team culture built on the Carers Bucks values.

As the leader of our home-based pet operations, you’ll oversee the smooth running of daily activities, acting as the subject matter expert in practical animal handling and assessment. You’ll work closely with your team to ensure timely pet assessments, behaviour management, and training, while providing full support to foster carers.

Make a Real Difference to Local Families
Home-Start Barnet, Brent and Harrow is a dynamic local charity supporting families with young children through challenging times. Our volunteers and staff offer emotional and practical support to help parents build confidence, strengthen relationships and give their children the best start in life.
We’re looking for a Family Engagement Coordinator to join our School Readiness project in Grahame Park, Barnet. You’ll be part of a friendly team at Home-Start Barnet, working closely with colleagues and partners at Barnet Mencap to support families who are just starting their journey with the education system.
In this role, you’ll help us reach families who may be facing extra challenges or need more personalised support. You’ll spend time building strong, trusting relationships with local primary schools and become a familiar, welcoming presence within their school communities. Your work will help ensure that parents feel informed, supported and ready to help their children thrive as they start school.
The common thread throughout the project is the engagement of families and the parent volunteers, who widen the supportive “community” around the families, and connect the school to the home, and the partners to each other. Approximately 70% of this role will be based in local schools in Grahame Park.
As our Family Engagement Coordinator, you will:
- Build strong relationships with primary schools in Grahame Park, in Barnet.
- Promote Home-Start’s services through school and community events.
- Support communication between schools, families and partner agencies.
- Share updates through newsletters, WhatsApp, email and community channels.
- Signpost families to local services and activities that support wellbeing.
- Receive referrals and assess family needs.
- Deliver school-readiness workshops and parenting groups.
- Liaise with schools, health and community services
About You
We’re looking for someone compassionate, proactive and highly organised, with a genuine passion for supporting families.
You will have:
- Experience working or volunteering in schools, nurseries, family support or similar settings.
- Parenting experience or experience caring for young children.
- Understanding of early years development and primary school systems.
- Strong communication and relationship-building skills.
- Knowledge of safeguarding responsibilities.
- The ability to engage with families facing complex challenges.
- Experience of delivering workshops or groups.
- Understanding of neurodiversity and SEND support.
- Knowledge of legislation relating to children and families.

Job Summary
The Head of Operations holds responsibility for our day-to-day delivery, strategic development of our services, and operational team leadership. As our new Head of Operations you will:
- sit on the PiP Senior Management Team and lead our team of circa twenty delivery staff.
- ensure continuous operational improvement, working with SMT to develop our service and systems, through new projects or ways of working, and champion a learner-led approach to everything we do.
- champion PiP meeting our regulatory and reporting requirements, manage key relationships including with local authorities, and hold budget responsibility across our service.

What You’ll Need
- Significant experience of working with people with learning disabilities, or a similar vulnerable client group, including knowledge of Safeguarding of Vulnerable Adults (SOVA) processes.
- Significant experience of leading and managing a staff team.
- Strong understanding of and commitment to a person-centred approach to working with people with learning disabilities, including planning, progress monitoring and review.
- Experience of liaising with a wide network of agencies and sector professionals in support of student needs.
- Knowledge of national and local legislation and policy affecting people with learning disabilities, including personal budgets. Alongside knowledge of key legislation and processes including on Health & Safety and Risk Skills and abilities
- Able to communicate effectively, both verbally and in writing, including the ability to maintain accurate records and produce reports for a variety of internal and external audiences.
- Able to take a lead role in the implementation of major tasks, including contributing to the delivery of key performance management targets and making organisational and systems improvements.
- Able to manage and complete projects successfully, including delegating tasks and monitoring and evaluating progress against outcomes.
- Able to establish, develop and maintain constructive and professional relationships with a wide variety of professionals and carers both internally and externally.
- Able to liaise and work closely with student families, carers and other stakeholders in planning, implementing and monitoring support.
- Able to take a flexible approach to work, including flexible and out of hours working in response to service needs and developments.
- Willing and able to participate actively in a variety of fundraising events and activities, including some evening and weekend events.
