Job Purpose
This role sits within our new Men’s Minds project, based in Hounslow. The aim of the service is to improve men’s mental health outcomes by developing a mental health network supported and directed by the voices of its members.
This will be achieved through three service strands: 1) Peer support network, 2) Psychoeducation workshops, addressing common needs of men’s mental health 3) Outreach support to raise awareness of men’s mental health and promote this service.
The Men’s Minds Project Coordinator will work closely with the members in shaping the service and ensuring voices of men in Hounslow are heard and acted upon. This service will be staffed with 1 part-time (25hrs per week) Project Coordinator and a pool of peer led volunteers. The Project Coordinator will be responsible for overseeing the volunteers.

The Role
The role of the Men’s Minds Project Coordinator is to arrange and coordinate a range of psychoeducation, and peer-led workshops which will be delivered weekly. With the addition of attending various community and stakeholder events to deliver the outreach strand of the service.
The role will require hybrid and flexible working: onsite for all the peer/psychoeducation sessions but the with expectation to be attending various community events and services to promote the service.

- Minimum of 1 year working in mental health services and with clients experiencing mental health distress

About the Role
Working at an enhanced practice level with support from senior colleagues to deliver a co-ordinated and comprehensive specialist dementia service to patients and their families/carers.
This will include developing knowledge, skills and experience with support from senior colleagues to undertake independent and autonomous practice: comprehensive holistic assessments, agreeing specialist palliative treatment management plans and advance care planning to be delivered through community and out-patient settings.
The post holder will develop to become an expert role model, supporting senior colleagues to deliver education and advice regarding dementia care issues to other health and social care professionals. Working with all members of the Hospice Dementia Team to stimulate and direct the development of clinical practice across the local community.
There is a genuine occupational requirement for the post holder to drive and have access to appropriate transport during the course of normal duties.

We are looking for a passionate and enthusiastic IV Coordinator, who will be responsible for the day to day running of the independent visitor service.
You will be responsible for managing and developing a pool of volunteers which includes the recruitment, training, and induction of new volunteers to become volunteer independent visitors, before matching them with a young person who has been referred to the service.

Position: S11158 Stroke Association Support Coordinator
Location: Home-based, Worcestershire, however, extensive travel will be required as part of this role (may include team meetings or other work-related meetings)
Hours: Part-time, 24 hours per week
Salary: Circa £17,546 per annum (FTE circa £25,500 per annum)

As a coordinator you will provide personalised information, advice and support with regular reviews throughout.
Key responsibilities will include:
· Working with the local Health and Social services to receive referrals to support stroke survivors and their carers
· Providing personalised information, advice and support to address any needs identified
· Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan
· Providing regular reviews to support people in establishing and achieving their own personal goals.
· Completing 6-month Post Stroke Reviews
About You
You will:
· Have a background in a caring and/or charity profession supporting people with disabilities
· Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
· Have experience of working with people who may have additional communication support needs
· Have the ability to use basic Microsoft system
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

Young Healthwatch supports young people aged 8-18 and up to 25 for vulnerable groups, to influence services in the Wakefield District through Young Lives, Healthwatch, and partners.
You will have at least 2 years of experience working directly with children and young people, with a proven ability to involve young people from diverse backgrounds in decision-making.
You will play a key role as Young Healthwatch Coordinator in a job share capacity to undertake the co-ordination of Young Healthwatch activities.
You will be responsible for the development of Young Healthwatch Engagement and Activity Programme for children and young people (8–25-year-old), ensuring they are supported to carry out their roles within Healthwatch.
We are looking for a flexible and creative hands on professional, who is versatile, well organised, positive and motivated. You will have experience of participation activities appropriate to children of different age groups, including children with Special Education Needs and disabilities, be values driven and committed to supporting children and young people.
