Role Overview:

Main Duties/Responsibilities:
Person-centred care and support
- Deliver effective social prescribing appointments using a holistic and person-centred approach that helps people focus on what matters to them.
- Empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans.
- Signpost and refer people to local voluntary, community and statutory services that help meet their needs.
- Support people to overcome their personal barriers to access VCSE organisations and follow up to ensure they are satisfied, able to engage, and receiving good support.
- Treat everyone with dignity, and respect different cultures, religions, ways of life and other personal characteristics at all times.
Communication and Collaboration
- Build excellent relationships with primary care/adult social care teams, providing regular updates, sharing knowledge of local VCSE organisations and working together to develop the service.
- Provide support and develop partnerships with local VCSE organisations, community groups, and statutory services to make safe and effective onward referrals.
- Work with the Enable Health and Wellbeing Team and local partners to identify unmet needs within the community and gaps in service provision.
- Promote the social prescribing micro-commissioning fund and support VCSE organisations to apply.
- Act as a champion of social prescribing, increasing awareness and promoting its benefits to a wide range of audiences.
- Work with a team of volunteer buddies to provide additional support to clients.
Service Quality and Development
- Collect and record outcome, monitoring and impact data using required systems, contributing to the evaluation of the service.
- Manage own case load to ensure clients receive appropriate levels of support and expectations are met.
- Gather client feedback to evaluate the quality of the service and to inform service changes and developments.
- Identify how the service could be developed and improved and, drawing upon knowledge of service delivery, make suggestions about how social prescribing could be embedded to a greater extent across Wandsworth.
- Keep up to date with key changes in the health and social care sector in order to shape and develop the service, including those relating to good practice and policy.
- Lead by example by demonstrating excellent customer service, delivery skills, organisation and professionalism.
- Complete Quality Assurance Assessments for VCSE organisations accepting social prescribing referrals to ensure they are safe, effective, and have the support they need to be part of social prescribing.
- Work with your line manager to identify your own support needs and undertake continual personal and professional development.

Skills and Experience:
- At least 2 year experience in a role that involves delivering face-to-face support in a relevant health or social care environment.
- Experience adopting a person-centred approach in order to meet the needs of service users and of empowering them to make informed choices.
- Experience of working with people of all ages with a range of social needs.
- Experience of working with the VCSE sector (in a paid or voluntary capacity), including with volunteers and small community groups.
- Motivational interviewing, behaviour change, or health coaching qualification (desirable) or willingness to undertake training (essential).
- Motivational interviewing, behaviour change or health coaching experience (desirable).
Knowledge
- Knowledge of the wider determinants of health, including social, economic and environmental factors.
- Understanding of personalisation and the skills required to support self-care.
- An understanding of the principles of confidentiality and how these apply when handling service-user information.
- Local knowledge of VCSE and community services in Wandsworth (desirable).

This is a senior leadership role managing our two flagship frontline services - Antifreeze and Gateway. You will lead 6-8 key workers and 30+ volunteers across two sites, overseeing day centre operations, street outreach, one-to-one keywork, group activities, and strategic initiatives.
As a member of the senior leadership team, you'll shape trauma-informed, dignity-centred services whilst contributing to our longer-term vision of tackling root causes of poverty. The Antifreeze Programme exists to break the cycle of social and spiritual poverty amidst homelessness, providing tangible steps towards a permanent home and long-term stability. Since 1998, Antifreeze has been a safe and warm space for members, offering faith-based, practical support services that restore dignity and hope. At our Day Centre, we provide hygiene facilities (showers & laundry), medical drop-ins, and essential survival provisions to meet immediate needs. Through 1-to-1 sessions, we support members with housing applications, benefits, and financial stability, while goal-based workshops equip them with skills to attain permanent homes and ethical employment. Our discipleship sessions encourage an advanced understanding of the Christian faith, offering fellowship and spiritual growth. To address digital poverty, we offer computer access for job searches and reconnecting with loved ones.
Beyond practical support, Antifreeze fosters healthy community relationships, improves mental health awareness, and helps members move toward ethical employment, reduced reliance on substances, and a forever home.
Main duties
Operational Management
- Oversee daily delivery of day centre, street outreach, keywork, and activities
- Manage £300k budget across both programmes
- Ensure safe staffing ratios through effective workforce planning
- Maintain compliance with H&S, safeguarding, GDPR, and quality standards
- Design and deliver projects aligned with fundraised income
Strategic Leadership
- Establish and drive a strategic vision for both programmes aligned with the Housing First approach
- Lead evidence-based service design informed by client needs and best practice
- Represent OTF at city-wide forums; build partnerships with local authorities and commissioners
- Hold budget responsibility of £300k ensuring financial sustainability
- Provide monthly KPI reports to senior leadership, Board, and funders
Team Leadership

Staff Support & Consultation
· Provide a consultation service for support staff and managers working directly and indirectly with clients at risk of homelessness and complex needs.
· Offer group reflective practice sessions and individual support to prevent staff burnout and reduce vicarious trauma.
· Facilitate debriefs after incidents to help staff process events, manage emotions, and build resilience

· Experience of having successfully undertaken a similar role and a background of work within the sector
· Experience of working with people who have substance misuse and complex trauma
· Experience of evidence based therapeutic interventions including (one or more of) Systemic Therapy, Cognitive Behavioural Therapy, Positive Behaviour Support, Mentalisation Based Treatment (MBT), Dialectical Behaviour Therapy and Acceptance & Commitment Therapy

Our client is the UK’s national LGBTQ+ Youth Homelessness charity. Their mission is to ensure that every LGBTQ+ young person facing or at risk of homelessness or a hostile environment has somewhere safe to stay, and work with over 800 vulnerable young people a year providing both face to face and online support. They are now recruiting a dedicated Supported Housing Manager to oversee the opening of a brand new LGBTQ+ service based in Lambeth.
Reporting to the Director of Services, as Supported Housing Manager you will have responsibility for the opening and day to day management of a new six unit LGBTQ+ housing scheme, working to create a safe, welcoming and psychologically-informed home environment for all beneficiaries. As Supported Housing Manager you will oversee a small, committed team of frontline staff, and will work to ensure that the support delivered is person-centered, holistic and to the highest quality. Acting as Designated Safeguarding Lead, you will ensure that team members fulfil their duties in safeguarding children and young people in line with company policies, and will work in partnership with a range of agencies to achieve best possible outcomes for all service users. You will also provide training to professionals and support to LGBTQ+ people, ensuring that professionals are working in an LGBTQ+ informed and inclusive way.

Alongside delivering our existing 1-1 counselling, self-development workshops and therapeutic support groups for women who have experienced domestic abuse, we are focused on developing our innovative mental health services for young women and girls, delivering new peer-led support groups and providing therapeutic groups to children and their mothers. Background
The lack of recognition of domestic abuse as a mental health issue within the NHS leads to delayed and inadequate support for survivors. This gap in understanding and response often result in survivors only being able to access and receive appropriate care when their mental health deteriorates to the point of requiring secondary mental health services (Women’s Aid 2021). Meta analysis suggests that CBT, one of the NHS’s most prescribed approaches for mental ill-health, is not the best approach for domestic abuse survivors as it does not recognise and take into account the external factors and dynamics of abuse, specifically power and control, the role of the perpetrator and the resulting trauma for the survivor.
In March 2025, WT published its first major report, ‘Living Without Hope’ which consolidates existing and increasing academic, government and sector research demonstrating the significant impact of domestic abuse on women’s mental health. Outlining the current agency responses at national, regional and local levels and the gaps in provision; the report also makes recommendations for change and improvement across the VAWG and health sectors.
Building on the research highlighted in our ‘Living Without Hope’ report, WT has secured funding to work with key stakeholders in the Royal Borough of Kensington and Chelsea to examine the links between domestic abuse and mental health, the specific experiences of Arab and Moroccan women and their access to safe, appropriate services that meet their needs.
Purpose
This is an exciting new role and will be focused on developing Woman’s Trust’s (WT) response to addressing the systemic issues female survivors of domestic abuse face when trying to access support.
Working with key stakeholders in the London Borough of Kensington and Chelsea (LBKC) and specifically Al-Hasniya to ensure that professionals have a clearer understanding of the link between domestic abuse and mental health and its impact on survivors.
Contract & hours: Part-time, 22.5 hours per week (0.6 FTE). Fixed term as dependant on funding.
