Living Well Health and Wellbeing Coordinator Role:
Referrals and assessment
- Triage and co-ordinate client referrals from a range of sources, primarily health and care, in conjunction with the Living Well Manager. These can be for the Living Well service alone, or for “Living Well Plus” where additional support is given to those clients who may require hoarding or de-cluttering support
- Communicate with referral partners to understand the trigger for referral and liaise with clients to build trust and encourage initial assessment
- Work closely with Adult Social Care, regularly hot desking at Merton Civic centre and joining GP surgery Multi-disciplinary meetings to accumulate more referrals and build professional relationships
- Actively engage in taking part in community events and meetings to promote the Living Well service and increase referrals/referral pathways
- Hold ‘guided conversations’ to assess clients’ needs and home risk, these will be conducted on telephone and through home visits
Action planning and support provision
- Create person-centred action plans to enable clients to set realistic goals to improve their overall health and wellbeing.
- Connect individuals to support services catered to their needs at both Age UK Merton and external partners
- Provide ongoing support and co-ordination for clients to ensure that action plans are progressing, including regular liaison with partners across health, care and voluntary sector to achieve successful client outcomes
- Work with partners across health, care and voluntary sectors to build strong understanding of the service offer for older adults in the London Borough of Merton.
- Receive and respond to enquiries related to the programme by phone, in person and by e-mail
- Work closely within Community Support Service, carrying out joint home visits with appropriate staff to ensure better outcomes for clients as well as ensure streamlined/triaged support.
General
- Record information on the CRM database, Charity Log, including service monitoring data and provide accurate and timely monitoring reports as required for funders
- Engage in exporting personal monthly KPIs as required by the Living Well Manager
- Cover the Community Navigator Role when needed with the Community Support service team on a rota basis
- Create two Case Studies quarterly, reflecting on successful outcomes for clients
- Carry out reviews with clients to capture the client’s journey from assessment to closing the case; showcasing how the client has improved their health and wellbeing outcomes
- Understand the process for raising safeguarding concerns and the route of accountability
- Comply at all times with the policies and procedures of Age UK Merton

We are currently seeking to appoint a part time Community Engagement Co-ordinator for our charity based in central London.
The Community Engagement Co-ordinator is responsible for developing with the Communications Manager a community engagement strategy for ScotsCare, in order to increase awareness and engagement with ScotsCare services and support, with the aim of increasing the number of people using ScotsCare services. A particular focus is on building relationships with organisations that engage with potential clients (ScotsCare service users), including councils, NHS, local community partners, service providers and other charities.

The role

The Minerva London service provides holistic, trauma-informed support for women in touch with the Criminal Justice System, through dedicated one to one support, safer women’s centre spaces and specialist interventions and partnerships.
As the Minerva Activities Coordinator you will be responsible for enhancing the offer to women referred into the Minerva service by supporting them to engage with their local women’s centre provision. You will be based at your regional women’s centre five days a week, as the first point of contact for women entering the centre space or contacting the service via the centre helpline. As Activities Coordinator you will create and deliver a relevant and lively activities programme for women to attend, in partnership with Minerva’s specialist delivery partners.

As Community Project Coordinator, you will be part of a dedicated team overseeing the efficient day-to-day operation of our Pet Food Bank project and wider community-based initiatives. Working closely with the Community Project Lead, you will help deliver projects that improve the lives of people and pets, with dignity at the heart of everything we do. This includes supporting a national network of pet food banks and partners, increasing awareness of Blue Cross, and ensuring practical, compassionate support reaches pet owners in short term crisis.
More about the role
This is a home-based role within Oxfordshire, ideally located within one hour of our Burford offices, as you will be required to travel to Burford on a weekly basis to manage deliveries at our Warehouse. Due to the rural location and less frequent public transport, travelling by car is often the most practical option. While the role is home based, you will be contractually attached to the Burford site. Travel to Burford will therefore be treated as normal commuting and expenses will not be payable for travel to this site.
Working across a defined geographic region, you will support the development and smooth running of the Pet Food Bank project and other community initiatives. This will involve close collaboration with pet food banks, suppliers, local authorities, community groups and volunteers to ensure effective coordination, communication and logistics.
You will secure donations of pet food and services, oversee the movement of food and equipment, and regularly travel to food bank partners and other organisations. A key part of the role involves liaising with volunteers and colleagues by phone and in person, ensuring everyone is supported, informed and working together effectively.
Alongside core project delivery, you will support the planning, preparation and delivery of a large-scale summer event in June 2026, working in partnership with one of our corporate partners. This event will play an important role in expanding our outreach and support for pet owners in need. You will also provide essential administrative support, contribute to reporting, and help deliver a clear and effective communication plan to support project and event activity.
Occasional travel to other partner organisations or Blue Cross locations will be required. Travel to locations other than your attached Burford site will be claimable in line with Blue Cross expenses policy.
You will act as a positive ambassador for Blue Cross, ensure policies and procedures are followed, and support the Community Project Lead with income delivery and media activity where required.
