About the Role

• You will have substantive experience in providing immigration advice and casework, with the relevant Level 2 OISC qualification (or OISC level 1 working towards Level 2). Please note only candidates with the relevant OISC qualification and legal experience will be considered.

Lester Court is a supported living service, serving the London Borough of Tower Hamlets. The service will supports seven adults with learning disabilities to thrive.

Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. We’re Causeway, the UK-wide charity who believes in the power of lasting change. In 2018, we collaborated with South Yorkshire Police in an initiative called Plan B. This helped to launch the LifeNavigate department. Using, a trauma-informed approach, we are fully committed to breaking the cycle of crime, alongside other organisations such as South Yorkshire Police and Merseyside Police. Our vision is to ensure a thriving future for marginalised and vulnerable people, and it would not be possible without our dedicated team of staff and volunteers.

We currently have an exciting opportunity for a Male ReFocus Advocate within our LifeNavigate Team based in Merseyside. The aim of this project is to support and signpost individuals who meet relevant criteria and show a motivation to change after being identified as being at potential risk of committing low level Domestic Abuse. A key element of this project is that it is survivor focused and looks to address abusive behaviours at their source, at an early intervention phase, via support and education. This means working with both those who have carried out such acts and those who have been made subject to them. The successful candidate is expected to support a caseload of individuals in order to jointly assess and manage any initial risk and to ensure all safeguarding protocols have been followed. The role is not that of and IDSVA, although some of the skills will overlap, the is to provide on-going support to individuals in areas
of need in which they feel disempowered and hence build confidence and skills. This process will happen in partnership with the organisation DMAT who will run ‘See the signs’ course which will look to educate and equip individuals to feel safe and empowered in this area. You will be working in an agile and responsive team environment and be part of the team providing support and pathways to those who have experienced domestic abuse. When capacity dictates, there will also be a requirement to work with people displaying potentially abusive or coercive behaviours (perpetrators), offering education and support to promote long term change. Due to the nature of working with victims of gender based abuse it’s important we have the flexibility to offer victims the opportunity to be supported by someone of the same gender, because of this we are currently recruiting for a male applicant.

Responsibilities:
• Providing support for individuals who are subject to low level Domestic Abuse (primarily male victims).
• The full time Advocate will oversee up to 16-20 clients of varying levels of support need at any one time.
• To work effectively as a team, including giving responsive feedback and handover information to Line Manager, and colleagues where there is a shared caseload, especially in regard to partnership with DMAT.
• To independently meet deadlines, including accurately recording and logging details of support, documenting relevant service user information and completing other administrative paperwork in adherence to the terms of the contract.
• To risk assess using the DASH model and take appropriate steps to safeguard service users.
• To create and follow individual support plans and ensuring all service users safety and well-being is paramount at all times.
• To arrange appointments for individual service users, and liaise with organisations and individuals regarding service users in a professional manner.
• Signpost service users to appropriate support.
• Supporting service users face to face (where suitable) in an appropriate environment, including using remote sessions where needed.
• To facilitate engagement of service users on the ‘See the Signs’ course, including journey planning and move on strategies, within the required time frame.
• To regularly attend supervision meetings with Line Manager. Adhering to all internal procedures regarding Causeway and any relevant legislation especially with regard to the Domestic Abuse Act (2021).
• When required, to work with perpetrators of low level domestic abuse, facilitating their engagement with the ‘See the Signs’ course, ongoing risk assessment, and provision of holistic support to promote lasting change.
• Promoting Causeway in adherence to the terms of the contract – this may include attending events on behalf of Causeway as a whole.

Job Title: Caring Dads Practitioner

Hours: 22.5 hours per week across 3 days.

Caring Dads exists to change current practice to better include fathers in efforts to enhance the safety and well-being of their children. The practitioner will work directly with fathers with an aim to improve relationships with their children, families, and co-parents/carers. This will include promoting the service to a range of teams across Hampshire and Southampton, to generate programme referrals. You will work closely with Hampton Trust domestic abuse perpetrator service to identify client suitability for the Caring Dads programme.

As the Caring Dads Practitioner, you will be required to deliver the Caring Dads and Accredited Domestic Abuse Prevention Training (ADAPT) Programme. This includes one to one support and group work. Groups are delivered on weekday evenings; you will be required to deliver one group work sessions per week.

You will undertake treatment and risk assessments of individuals causing harm in their relationships; this includes active case/risk management of families living with domestic abuse. Although this position is based in Ashurst, you will be co-located into social work teams across Hampshire three days per week, with an aim to providing support and advice in identifying suitable families for the Caring Dads and ADAPT programme.

Support Coordinator

This is an exciting opportunity to join our team in Worcestershire.  We’re looking for an enthusiastic, motivated and well organised individual to join us in delivering a comprehensive Stroke Recovery Service across the region.

Position: S11158 Stroke Association Support Coordinator

Location: Home-based, Worcestershire, however, extensive travel will be required as part of this role (may include team meetings or other work-related meetings)

Hours: Part-time, 24 hours per week

Salary: Circa £17,546 per annum (FTE circa £25,500 per annum)

Contract: This is a fixed-term contract until 31 March 2025. Our services are contracted, we currently have funding for this contract until 31 March 2025.

The Role

Reporting to the Stroke Association Service Delivery Coach, the coordinator will work with the local Health and Social Care services to receive referrals and support stroke survivors and their carers. As a coordinator you will provide personalised information, advice and support with regular reviews throughout.

Key responsibilities will include:

· Working with the local Health and Social services to receive referrals to support stroke survivors and their carers

· Providing personalised information, advice and support to address any needs identified

· Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan

· Providing regular reviews to support people in establishing and achieving their own personal goals.

· Completing 6-month Post Stroke Reviews

About You

You will:

· Have a background in a caring and/or charity profession supporting people with disabilities

· Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.

· Have experience of working with people who may have additional communication support needs

· Have the ability to use basic Microsoft system

This role requires extensive travel across a large geographical locality to visit people at home and in community settings.  Candidates must be able to demonstrate how they can meet this requirement of the role.

About the Peer Mentor Service

Shelter is delivering a Peer Mentor Service in partnership with the Forensic Intensive Recovery Support Team (FIRST) within Birmingham and Solihull’s Mental Health Foundation Trust. The FIRST service is a comprehensive recovery focussed multi-disciplinary team addressing the individual needs of our service users through tailored packages of care. It includes Consultant Psychiatrists, Psychologists, Advanced Nurse Practitioners, Community Psychiatric Nurses, Occupational Therapists, Support Workers, Peer Mentors, Substance Use workers and Social Workers. Each service user will have an allocated Care Coordinator and Responsible Clinician who monitor, and review care and support needs and interventions.

The Peer Mentor role will engage with clients on the ward and within the community - with a primary focus on supporting clients with their transition away from staying on the ward to living in the community. The Peer Mentor service’s aim is to help people with multiple and complex needs to enhance the support available through the FIRST Pathway.

About the role

As a Peer Mentor, you'll support people with mental health concerns who are looking to create sustainable lives in the community by improving engagement with services with a view to sustaining wellbeing and reducing reoffending. Your own experiences will be invaluable as you engage with and motivate individuals to set goals around their recovery. This will include building a relationship of trust with individuals and understanding the importance of empowering them to make their own choices. Along the way, you will have the opportunity to access personal support to develop new skills and improve your self-confidence to do the job.

About you

We’re looking for people who have had mental health issues, are ex-offenders or who have been through the criminal justice system. Lived Experience of homelessness and substance use is useful but not essential. You’ll need to have accessed support services, have an understanding of how these experiences have affected you and be able to tap into these experiences to identify gaps in services and barriers to recovery and how things can be improved. You’ll need to have been stable for at least 18 months and be willing to share details of your successful journey with others to help them. You will be looking to go into a career supporting vulnerable adults and this will be a great opportunity to gain experience. Basic computer skills will be needed, as well as the interest to develop and learn new skills.

Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment.  The role is working within the nationally recognised IPS (Individual Placement and Support) model within an NHS secondary care mental health team based in the heart of Camden.

Who we are: 

Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. We support people with mental health conditions into training, volunteering, and employment opportunities. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity.  Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.

The Role: 

The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.

A variety of skills are needed in this post including but not limited to -  a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health. 

The postholder will be directly employed by Hillside Clubhouse but work under an Honorary Contract from Camden and Islington Foundation Trust.  They will be managed directly by the IPS Operations Manager but work as team member within the clinical team. The role is dynamic and involves being integrated in a clinical team, working within the community, and we operate a flexible working policy where working from home is also permitted up to two days per week.

Overall aim: 

To enable people with mental health issues to gain and sustain competitive employment by working within the evidence based Individual Placement and Support (IPS) model.

Primary Duties: 

• Manage a caseload of up to twenty clients and work within and apply the 8 principles of IPS in all aspects of role. 
• To work with and enable clients to find work using a mix of conventional approaches and employer engagement. 
• To generate ongoing referrals from the clinical team and to manage those effectively. 
• To engage directly and collaboratively with the clinical team while maintaining the existing multi-disciplinary approach to assisting clients seeking work. 
• To report and meet regularly with clinical team service managers and the IPS Operations manager, and to participate in both casework and clinical supervision from IPS Operations and clinical team services managers. 
• To be an active member of both the clinical and Hillside teams 
• To be responsible for own management of administrative duties tied to both Hillside Clubhouse and clinical team and adhering to existing Hillside and NHS protocols. 
• Source job opportunities specific to client needs from a range of sources including those advertised via current and conventional means, as well as through contact with local and Pan London employers. 
• To build up a network of employers to secure available and future vacancies. 
• To maintain existing and seek out new relationships with a range of external agencies of use to the post. 
• To provide ongoing support to both client and employers (where appropriate) when starting work and until such time as the client makes the decision to end the support provided. 
• To work with clients to collaboratively manage job searching activities and engagement with the service. 
• To manage and meet all KPIs, statistics and documentation as required, including but not limited to: referrals, first meetings, job outcomes, job sustainments of 6, 13 and 26 weeks, and work retention.
