The Project Development worker is central to the development of friendships and relationships for people with learning disabilities across the Kirklees area.

We are currently looking to add a temporary Support Worker to our passionate and committed team in the rural village of Blythe Bridge, on the outskirts of Stoke-on-Trent.
We are looking for a warm, caring and compassionate individual to join our Supported Living service in Blythe Bridge, Stoke-on-Trent. Our service provides support to one gentleman living alone, with learning disabilities and Autism. This service user is non-verbal so patience and excellent communication is an integral part of his care plan. He enjoys a range of activities including swimming, walks in the park, visiting restaurants and using his own hot tub! 1 to 1 support is needed inside this 3 bedroom home with 2 to 1 support needed in the hot tub and the wider community.
The ideal candidate will be respectful and able to provide excellent support including: personal care, administering medication and help with domestic tasks such as cooking and cleaning. You will also be passionate about enabling and encouraging our service users to enjoy a fulfilling and valued life, and to develop their abilities to the fullest.
Support Worker Duties:

Flexible working hours to include daytimes, evenings and weekends

Assisting with day-to-day living duties such as budgeting, shopping and supporting with medication and any personal care that may be required

You will collaborate with service users, families, carers, and involved professionals to provide a coordinated and consistent service that maximises outcomes for service users

Encourage and support service users in expressing needs, views and concerns, enabling them to make their own choices and decisions
Due to the rural location of the service, car drivers are preferred however not an essential requirement for this role. Previous care experience is desired but by no means essential, as full ongoing training is provided from our Creative Support Training Academy.

About the role:
Luv2meetU is a friendship and dating agency for adults with learning disabilities operating predominantly in the North of England covering 9 different Local Authority areas. We support up to 300 members across all the areas. We have approximately 15 members of staff within Luv2meetU and related projects.
The Project Development worker is central to the development of friendships and relationships for people with learning disabilities across the Sheffield and Derbyshire area.
The post holder will work with existing Luv2meetU members along with the people Hft supports across the Sheffield and Derbyshire area, to enable them to explore new opportunities within their local community, develop and support existing and new friendships and relationships and become more active.
Hours are flexible and will include evening and weekend work based on the needs of the project.
Salary: £17,969 pro rata
Job Type: Fixed Term Post - 1 year - with the possibility of funding being extended beyond this.
Hours: Part Time, 18.75 hours per week
Location: Hft’s offices, Sheffield and working from home and out in the community
Experience of working with vulnerable people including adults with learning disabilities or autism, together with an understanding of the needs, experiences and aspirations of people with learning disabilities would be advantageous.

To support the development of the Thames Ward resident-led health outreach project. The purpose of the project is to connect both local people and the local groups who are interested in improving the health of their community by working through the Thames Ward Locality Board to jointly create health programmes with LBBD Public Health, social care and lifestyle teams; NHS clinical commissioning groups (CCGs), primary care networks (PCNs) and local provider trusts (including BHRUT and NELFT); and the community and voluntary sector (including local groups and regional / national organisations).
The postholder will support and enable the mobilisation, development and implementation of the project. They will play a catalysing role, helping to deepen and embed the emerging model of care, working alongside residents, community groups, commissioners and health professionals.
