We are seeking Community Support Workers to join one of our community-based services in Stockport, which aims to provide extensive care, specialist positive behaviour support and promote independence and inclusion within the local community, in order to empower the individuals to take control of their own lives. The main focus is to provide the highest level of support in order to enable greater independence, choice and fulfilment for the individuals.

Salary – £20,296.90 (£12.01ph) for a 32.5 hour week, plus enhanced pay for weekend hours of +25% hourly rate. (Full time equivalent salary of £23,412 for 37.5hrs)

Working Pattern – 32.5 hours (full time) with part time hours also available. Hours range from - 9.00am through to 5.00pm Monday to Friday. Some evening and weekend work is also required which is on a rota. An enhanced payment is paid for weekend hours of +25% hourly rate.

Location – Specialist Day Service Provision, Kershaw’s Tanner, Water Street, Stockport, a Together Trust Service.

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.

This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.

We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.

Main Responsibilities:

· Undertake 1:1, group, detached and outreach work with young people

· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.

· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.  

· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.

· To maintain confidential, up to date and accurate records of all sessions and plans.

· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:

o mental health and wellbeing of children and young people.

o Supporting vulnerable groups who would not otherwise receive support (levelling-up).

o Improving children and young people’s engagement with EET.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.

· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.

· Experience of building positive, impactful relationships with young people responding to their needs

· Excellent, up to date knowledge and understanding of safeguarding needs and risks young people may face.

We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. Windsor Road service offers 11 beds to people experiencing mental ill health.  We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days.  We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.

We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in. Responsible for helping and inspiring the people who use our services to achieve independence in the community, we’ll rely on you to work with them to come up with a person-centred support plan that will see them achieve their goals and aspirations, as part of your key worker role. Whether it’s developing their domestic and finance management skills or accessing work or volunteering or educational opportunities, one thing’s for sure ‐ you will have every chance to shine.

Care home, mental health support or social care experience is preferred. You will be required to administer medication and complete a medication qualification and the care certificate. Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

Home-Start Lambeth offers a unique service, recruiting, training, and supervising volunteers to support families with pre-school children on a regular basis. Our staff and volunteers provide friendship and practical help to families experiencing stress or difficulties, including poverty, post-natal depression, domestic abuse, mental ill-health, isolation, and other challenges leading to crisis.

We are advertising for a Family Coordinator to join our small, amazing charity. Working alongside our team of Family Coordinators, you will oversee the work of our home-visiting volunteers, ensuring that the support they provide to families is targeted, high quality and empowers them to reach their goals.

You will also lead the delivery of our domestic abuse work. This includes the development and delivery of the Freedom Programme; a 7-week course to help survivors of domestic abuse understand and recover from their experiences. 

We are looking for a well-organised person, familiar with office systems, who can demonstrate empathy and good communication and interpersonal skills. Parenting experience, experience of working with disadvantaged families and survivors of domestic abuse would be advantageous.

This role involves significant amounts of travel around Lambeth visiting families and attending meetings. Reasonable travel expenses, other than from home to base, will be reimbursed. Please note that currently the Freedom Programme is delivered remotely via Zoom on Tuesday evenings from 7-9pm via Zoom.

Crisis House is a community-based crisis service, working to a recovery model with the aim to reduce the amount of A&E and Hospital admissions through evidence-based CBT interventions. We provide a range of intervention support and are partnered with the NHS, working alongside the Home Treatment Team who offer clinical governance. Additionally, we offer a range of services for people with mental ill health to become more connected to their local community, building on their social engagement. We signpost to local services and host awareness days that truly reflect the community we support to break down stigma and discrimination. Offering support 24/7 with a staffed service to our residents aged 18 and upwards.

No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. Whatever your background, you’ll need to be happy to work both independently and within a team and willing to be part of a weekly rota system and available for on call duties.

Here at Peabody, we have an opportunity for a Outreach Support Worker on a Permanent basis in Brentwood, Epping & Harlow areas.

A bit about the role:

As a Outreach Support Worker, you will provide a service where planned support and crisis intervention is delivered to vulnerable people with low, medium and high support needs living in independent accommodation with the aim of them maintaining their home.

In conjunction with colleagues and Senior Managers, you will be responsible for the effective operation of the Support Service, in line with its policies and procedures, including encouraging a co-operative and supportive environment within the Service, listening to customers’ views and developing new services to meet these needs.

Some of the key results for the role include: 

• To provide pre tenancy guidance and ongoing support to customers on all tenancy matters, including assistance to furnish and move into properties.
• To enable customers to abide by their tenancy agreements, giving advice and assistance on dealing with police and courts where this relates to housing matters
• To provide basic Welfare Benefits advice, assisting customers in claiming all benefit entitlements and assisting with accessing specialist support. 
• To work with customers to identify the skills required to manage their own tenancy, including assistance to deal with difficult situations.

This role will require an Enhanced DBS check, a UK driver’s licence and access to a vehicle.
