You will provide remote one-to-one support for special guardians and other kinship carers within local authorities who have commissioned Kinship Reach. You will also provide remote one-to-one support to kinship carers, including digital up-skilling and developing and facilitating virtual peer support groups.

Children and Family Support Worker
To develop and deliver family support and play sessions to children, young people and their mothers in refuge and the community. To lead on domestic violence awareness raising sessions with children/young people and professionals.
Duties and Responsibilities
Support Work:
- To work in partnership with mothers/carers to identify children and young people’s support needs and to ensure those needs are met.
- To undertake needs analysis for each child and young person, to ascertain the level of support needed and the type of activities required to meet their needs.
- To develop and deliver a comprehensive programme of one-to-one and group therapeutic and play sessions, and shared activities within refuge.
- To provide school holiday play schemes in refuge.
- To supervise and accompany children and young people on trips and visits outside of refuge.
- To provide one to one support and family support with families living in the community.
- To seek approval of all programmes with the refuge manager and ensure that these are discussed with other relevant members of the staff team before implementation.
- To participate in the development of informative literature for women and children, including a children’s introduction pack, which is to be kept up-to-date.
- To purchase/hire such equipment as necessary to undertake programmes within budgetary constraints and with approval from the refuge manager.
- To carry out appropriate methods of communication with women, children and young people living in refuge to keep them informed about opportunities and updates on work that is taking place, including house meetings.
- To carry out detailed monitoring and evaluation of all support work, and keep an accurate record of notes and comments in line with relevant legislation.
- To communicate regularly with mothers/carers about the work with their children.
- To support mothers/carers to enable them to meet the schooling, health and social care needs of their children.
- To link with health workers, schools, nurseries, social workers and to implement procedures for referral to other agencies
- To work with the BSWA floating support and resettlement teams to identify the needs of women and their children leaving refuge after being offered permanent accommodation.
- To advocate on behalf of children, where necessary, throughout adhering to Child Protection procedures.
- To ensure that Health and Safety records are kept up to date.
Administration:
- To work within BSWA’s administrative and financial systems.
- To ensure that adequate records are kept for all service users.
- To keep accurate records of financial transactions in line with BSWA’s procedures.
- To provide monthly reports to the Refuge Manager about work that has taken place, feedback from service users and produce statistical information about participation.
- To contribute to the preparation of reports for funders.
Communication:
- To ensure that all staff team members are aware of any matters that are relevant to the welfare and security of the refuges, staff, volunteers and service users by:
- maintaining BSWA communication records
- following safety procedures
- attending team meetings.
- To attend support and supervision sessions with your line manager.
- To develop a professional working relationship with external agencies.
Premises and Equipment:
- To ensure that all play areas, equipment and activities comply with Health and Safety requirements, the Children Act and other relevant legislation.
- To deliver inductions and orientation to all children and young people entering the refuge.
- To offer support and training to volunteers and ensure that they are aware of all legal Health and Safety requirements.
Training:
- To deliver positive relationships and domestic violence awareness programmes in local schools and youth groups.
- To raise awareness of the impact of domestic violence on women and children and of BSWA services through the delivery of training and briefing sessions to agencies and community groups.
- To work alongside colleagues to organise in-house training and information sessions on a range of issues for service users.
General:
- To maintain the confidentiality of the women, children and young people using the service.
- To maintain confidentiality in all matters relating to the organisation.

The Role
The role of the Family Support Co-ordinator is to build the knowledge, skills and confidence of families with profoundly disabled children to interact musically with their child to benefit their development, personal and social interactions and build family resilience, enhancing wellbeing and reducing isolation for parents/carers. A key part of the role will be to measure the longitudinal progress and development of children and young people with profound and multiple learning disabilities supported by Soundabout, to help the charity develop and improve our practice to ensure the best outcomes.
The role is also the first point of contact for families when enquiring about Soundabout, sharing ideas for opportunities to engage with our services and offering additional support to our families as and when needed, trouble-shooting any access or other issues or concerns.
The post holder must be willing to lead on our Safeguarding agenda (either through pre-existing experience or by attending appropriate training).
The Candidate:
You will be a warm, flexible, and energetic team player with significant experience of working with families of profoundly disabled children and a commitment to reducing isolation for families and building support networks and connections. You may have an understanding of how music and sound can be used as a developmental tool and to build family cohesion and resilience. Key responsibilities:

To be the primary point of support for all families with children who have profound and severe learning difficulties and are part of Soundabout’s community; whether attending music-making sessions, participating in 1:1 support sessions with our music practitioners or as part of Soundabout’s Inclusive Choir and associated groups.

To induct new families into the organisation, exploring with them the opportunities available and best suited to each child and family’s needs. Introduce families to our evaluative processes to be undertaken by Soundabout practitioners and track progress, providing bi-annual updates to families via progress reports. Signpost families to other opportunities and forms of support as appropriate.

As Diabetes Support Worker, you will play a key role within the team, connecting people living with or at risk of developing diabetes, and their families and carers, with the information and support they need to improve the quality of their lives. You will work with our network of volunteers, groups, community organisations and local partners building relationships throughout local communities, helping to increase our reach and support for people living with diabetes across the region.

Main Responsibilities
The Diabetes Support Worker will
- work closely with the engaging communities and volunteer manager and the wider team, to support the development and delivery of an effective and meaningful community engagement and volunteering plan across the region
- work with the Engaging Communities and Volunteer Manager to map and build strong links with community organisations, local partners and providers of health, social and community services and develop and maintain a database of support services relevant to people living with diabetes in their local community
- support and enable Diabetes UK volunteers, groups, and communities to build effective local connections with the above services and their wider diabetes communities to grow our reach at a local level
- assist people to identify their needs and access the right support and services within their local community and/or diabetes UK, with particular focus on the more deprived or less often seen and heard communities in the region.
Ideal Candidate
As a Diabetes Support Worker, you will be self-motivated with excellent administrative and organisational skills. You will be able to communicate effectively with a range of audiences, with an understanding of the importance of diversity and inclusion. You will have the ability to listen and empathise with others, forming effective working relationships with a range of health and social care professionals, community groups and people with lived experience. The NHS Long Term Plan includes commitment that ‘by 2023/24 children and young people with a learning disability, autism or both with the most complex needs will have a designated keyworker. Initially, keyworker support will be provided to children and young people who are inpatients or at risk of being admitted to hospital. Keyworker support will also be extended to the most vulnerable children with a learning disability and/or autism, including those who face multiple vulnerabilities such as looked after and adopted children, and children and young people in transition between services. Birmingham and Solihull are now the pilot phase of this commitment.
The BSOL system was awarded funding from NHS England And Improvement in April 2021 to deliver a new Key Worker service for young people and families. The cohort for the service is 0–25-year old's living with Autism and or a Learning Disability. Locally this is where we see highest admissions and most gaps in support. Young people will be identified through the dynamic support registers (DSR.) There will be a single point of access for which the referral process will be defined as part of mobilisation.
Each young person referred to the service will have an named assigned Keyworker. The team will provide expert planning, support navigation and advocacy to the young person and their family at a level appropriate to the person's needs. A “step up and step down” process will ensure young people receive when the right level of they need it and can re access their key worker as required.
You will have (Essential Criteria):
- Professional qualification in healthcare, social work, or nursing.
- Experience of working with young people with autism, learning disabilities and mental health.
- Experience of working in social care, health or education setting.
- Experience of multi - agency working / partnership working.
- Evidence of report writing and case management.

The Role
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A key part of the role will be to support service users and the delivery of the service.
Reporting to the Stroke Association Service Delivery Coach, the Stroke Association Support Coordinator will:
- Support new stroke survivors and their carers from hospital discharge into the community.
- Provide personalised information, advice and support.
- Support clients to make informed lifestyle changes which will help them to prevent further strokes
- Be accountable for volunteers, so experience of recruiting, training and managing volunteers would be an advantage.
- You will have excellent IT skills and demonstrate a flexible approach to your role.
About You
The post holder will have experience/background in:
- A caring profession ideally with experience of supporting people with disabilities.
- Setting up and running virtual groups.
This role is home based, but extensive travel is required across the local area and occasionally further afield.
