Hours worked between: 9.30am - 5.30pm Monday - Friday pro rata

PURPOSE OF THE JOB

The Barnet Wellbeing Hub is an exciting new initiative that utilises a social prescribing model to support adults over 16 years old with common and long term conditions and/or social care needs to become involved in community activities. The Wellbeing Hub is an integral component of the overall Integrated Wellbeing Service Strategy for Barnet. A vacancy available for the right candidate to support the continued growth and development of the Social Prescribing offer in Barnet.

The hub acts as a referral Single Point of Access between statutory providers and community settings. A high volume of people are referred from statutory providers, other community organisations, or could self-refer to the hub. Staff will support individuals to engage with community activities.

The role of the wellbeing navigator team will be to provide a holistic service from referral through to discharge. They will work with partners to accept referrals into the service. They will welcome each person, provide a short triage to ensure that their conditions are appropriate ones for the service to assist, and then make appointments for them to visit for more in-depth assistance where necessary. They will also carry out background administration for this service such as logging individual details onto the service database.

MAIN TASKS

Individual Support

• Working with other team members, to provide first point of contact to all those accessing the service, whether face to face, by telephone or email.
• As part of a team, make contact to all people referred into the service.
• Provide a basic Triage service to people who are referred, either over the telephone or via appointments in the office.
• For those people with appropriate needs, book appointments for them to meet.
• Assist people to engage with community activities.
• Follow up with people who have used the service to ensure that they are engaging successfully.
• Provide a professional service that maintains boundaries, but is warm and respectful.
• Maintain individual confidentiality and respects equal rights and diversity.

Outreach work

Links with other community organisations and relationships with statutory services are integral to the success of the Wellbeing Hub. You will be working as part of the team to formulate an outreach work plan to support the Integrated Wellbeing Service working with our partners,

• Log all individuals accessing the service onto the central database in an accurate and timely fashion.
• Liaise with our partners and encourage collaborative and partnership working
• Build relationships and rapport with external partners
• Seek and generate referrals to the Wellbeing Hub and improve the community access into the Wellbeing Hub service
• Work flexibly at other premises, including GP practices, to deliver Emotional Health Checks to individuals based around the borough where appropriate

Administrative Duties

• Log all individuals accessing the service onto the central database in an accurate and timely fashion.
• Information logged should include people’s demographic information, presenting needs and any other case notes deemed to be appropriate.
• Log appointments for people to meet Wellbeing Navigators.
• Ensure that all paper or computerised records and documentation regarding the service is effectively and securely stored.

General duties

• To maintain accurate records to fulfil reporting requirements
• To adhere to all policies including equal opportunity policy and volunteer policy at all times
• Support the team in the setup and facilitation of events throughout the year and support the continued development of our activities
• Work with the team to support the recruitment and management of volunteers to create a team of hub champions to support the work of the navigators
• Attend and contribute to regular team meetings and supervision, as well as training sessions
• Carry out such other duties as deemed appropriate and may be reasonably required by the Service Manager.

The Role
Our Practitioner Led Assessment Service is the entry point for our beneficiaries seeking support from the Charity. Our Assessment Practitioners work in partnership with our beneficiaries to identify goals, individual needs and appropriate solutions.

The team provide advice, information and support and work collaboratively with the multi-professional team to ensure our beneficiaries are offered the right support at the right time.

Reporting into the Beneficiary Experience and Continuity Lead you will be responsible for day-to-day management of the practitioner led assessment service, holding a small case load and reviewing and resolving complex cases with the team.

• Develop relationships with key stakeholders in the NHS, health care and social care professionals to inspire and enable them to feel engaged and supported to co-create and prescribe a Nature Prescription, and to connect RSPB with the health sector to help create doors for future work.
• Engage with local healthcare professionals and community groups to co-design Nature Prescriptions materials with local people, community groups and healthcare professionals so that they reach a large diversity of people, and those that need and can benefit the most.
• Develop the project plan and ensure that the right governance structure for project management is in place and RSPB internal project management practices are followed, so that the project stays on track, comes in on budget and risks are identified, escalated, and managed within tolerances.
• Work with local environmental networks to build a picture of nature-based activities available and identify opportunities for partnership working.
• Lead on developing ways to evaluate and measure the impact of the project, including engaging with those prescribing and local communities to gather stories and case studies that demonstrate the benefits to wellbeing and connectedness to nature.
• Build internal relationships with RSPB employees and volunteers working with nature prescribing and nature and wellbeing across the UK.
• Develop a strategy and plan for Nature and Wellbeing work legacy aiming to learn from the work done in Scotland and England and from the communities and health care professionals in Cardiff and surrounds to support development of future work in Cardiff and beyond.

Essential skills, knowledge and experience:

• Experience of working within or alongside the health and wellbeing sector.
• Project management experience with experience of leading on and developing and delivering projects with diverse communities.

• Experience in delivering workshops or training, ideally with a co-design element.

Snow Camp is a ground-breaking national youth charity, and we are looking to recruit a fantastic new North West Wellbeing Manager and Programme Support to join our North West team on a 4 day a week contract (30 hrs).

Our accredited programmes are unique and attractive to young people, taking them from beginner to qualified snowsports instructors in just one year with volunteering and apprenticeship progression routes available at the end. Alongside learning to ski or snowboard, young people also have access to wellbeing support with the emphasis on improving young people’s mental health and wellbeing.

You will report to the North West Programme Manager & Head of Snowsports and will be responsible for the delivery of our wellbeing workshops and providing 1to1 wellbeing support sessions to young people. You will also work as part of the North West team to assist with the planning and delivery of snowsports and life-skills focused youth activities run by the Programme Manager and assist with the apprenticeship programme throughout each year.

Experience of working successfully with under-served young people in a range of settings, with a focus on supporting young people’s wellbeing will be essential to the post. Passion for snowsports and supporting young people generally will be a huge advantage!

Youth work, wellbeing support experience, project management, logistical skills, managing relationships, communication and presentation skills will make up the role. Along with experience of delivering group workshops to help young people work towards meaningful goals to improve their emotional wellbeing.

Job Description

• You will spend roughly 2.5 days will be in the Wellbeing Manager role and 1.5 days will be in the Programme Support role each week.
• In consultation with the Programme Manager, to manage the delivery of the Wellbeing Workshops and 1to1 mentoring support needed for young people and apprentices.
• Work with the programme manager to gain the young people’s trust and confidence.
• To assist with delivery and development of The North West’s full snowsports focused journey of youth programmes which take place at Chill Factore.
• Build a good relationship with the North West Programme Manager to help provide support and guidance to young people.
• Support with the management of (currently) 3 x North West apprentices on their 1-year Snow Camp Apprenticeship programme. particularly helping to manage their wellbeing and signpost to other services where needed.
• Ensuring the apprentices have clear personal development plans and objectives.
• Provide effective and inspiring leadership to apprentices and young people.
• Providing weekly 1to1 support sessions for each apprentice.
• Having an understanding and an involvement in the development of their weekly working rotas.
• Supporting with NVQ work & assisting with any challenges that may be faced.
• Ensuring apprentices are involved and supported working on all Snow Camp programmes as the North West staff team.
• Complete Personal Development Plans and Wellbeing Assessments as required with apprentices.
• Attend regular meetings with the other regional Wellbeing Managers to provide a consistent approach and building resources focused on improving young people’s wellbeing.
• Outside of programme days, provide one-to-one mentoring sessions to young people, in-person or remotely, focusing on their well-being and offering support so they can cope with the Snow Camp programme and improve their general wellbeing.
• Be a key team member on the overseas residential for the Excel programme during the Easter holidays. *Valid passport for travel in Europe & Andorra required.
• Use our UPSHOT online database system to keep records and ensure all monitoring and evaluation is carried out.
• Attend external training as required (e.g. Safeguarding and Mental Health Awareness tutor training courses).
• Ensure that all activities and provision you deliver is done safely and in line with organisational policies.
• Support the Programme Manager to ensure all programmes run smoothly and effectively by supporting the delivery of each programme – managing venue instructors, guest speakers, apprentices, youth staff, sessional staff and volunteers as required.
• Support the Programme Manager with recruiting young people for each programme through visits to youth projects, promotion through email to all relevant youth programmes and services and presenting at key stakeholder meetings on youth service provision. Finding new young people in the North West area to support by Snow-Camp will be key.
• Support the Programme Manager with programme administration including finances, SLAs, attendance records, risk assessments, bookings and young people’s records on Upshot.
• Support with management and delivery of trips and overseas residential’s (Snow-Camp Excel, Apprenticeship & Youth Forum Residential’s) as part of Snow-Camp programmes. Including support with all the necessary risk assessments.
• Additional tasks may vary depending on different opportunities throughout the year.

Personal Specification

• Sound knowledge of MS Office tools including Word, Excel, PowerPoint and Outlook.

Management and support

The North West Wellbeing Manager & Programme Support will report to the North West Programme Manager & Head of Snowsports Morv Bett who will provide support and guidance.

The Trustee board will determine the overall direction of Snow Camp and will set the overall priorities in consultation with the Director and Senior Team.

The Junction is an out of hours “calming café” service which is open 365 days of the year to support people experiencing pre and/or mental health crisis. The aim of the project is to alleviate the need for people within the Swindon locality to access A&E and/or acute mental health services.

As a Wellbeing Practitioner, you will be providing a preventative, person centred approach to people accessing the service who may be experiencing a mental health crisis.

The service is open from 4PM-11PM and there are positions available for part time hours and full time hours and  working patterns will include some weekends.

You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. The purpose of the role is to engage young people in a variety of interventions, using a young person centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.

Based in the Transitions team, you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. The caseload will include a mixture of community and multi-agency referral pathways including health, social care, education and criminal justice.
 
A core part of the role will be working with young people and their families to address holistic support needs across all aspects of the young person’s wellbeing. This could include diverse work related to homelessness, family work, exploitation, physical and emotional health as well as substance misuse.
 
An energetic and confident self-starter, you have proven experience within health and social care, particularly working with young people and supporting them through mental ill health.

This is a permanent full-time role requiring the post holder to work 37 hours per week.

The Active Wellbeing programme (AW)

The Active Wellbeing programme is a specialist mental health exercise referral programme. This programme is designed to engage and promote health and wellbeing to people diagnosed with a severe mental illness (SMI). The programme involves delivering a 10 week, 1:1 personal training programme to clients with severe mental health illnesses (SMI) which can include: Bipolar, Schizophrenia, Severe Clinical Depression.  

The Active Wellbeing programme – Learning Disabilities (AW)

The Active Wellbeing programme is expanding to work with adults with learning disabilities who also have a long and enduring mental health condition. The programme LD involves delivering a 10 week, 1:1 personal training programme to clients with long and enduring mental health conditions and individuals with learning disabilities.  

Main Duties and Responsibilities:

• Design and deliver a weekly physical activity session for each client considering safety and progression with each of the 10 sessions. 
• Monitor clients exercise intensity, type, and ability to ensure clients are exercising safely.
• Monitoring clients mental wellbeing during sessions to ensure safety and communicating any concerns to the line manager.
• Responsible for arranging weekly sessions with clients (and in some cases carers alongside)
• High level of communication between instructor and client (in some cases carers or support workers) to confirm session times and locations.
• High level of communication between instructor and line manager to report any concerns with a clients mental and physical wellbeing during sessions.
• Provide motivation and encouragement for clients to engage in sessions and stay engaged in their programme. 
• Responsible for the setup of equipment for outdoor or online sessions.
• Responsible with checking in and out of sessions with the admin team to ensure safety of both instructor and client. 
• Responsible for checking the safety of equipment. If equipment is faulty and needs to be replaced, responsible for communicating this with the Active Wellbeing programme coordinator. 
• Responsible for administering first aid in the event of an accident and ensuring that safety requirements are met in all areas.

A 4 day per week post promoting the health and wellbeing of people in Birmingham made homeless and destitute by the asylum system.

Working alongside Hope's legal advice and housing teams you will help people register with a GP and access essential healthcare; find essential services; and take part in activities, such as sports, arts or volunteering, that build their wellbeing and resiliance. You will also promote the wellbeing service among refugee communities and key service providers.
