· Foster partnerships with voluntary and statutory services to ensure cohesive support.

· Regularly identify and address barriers to progress, working collaboratively with each individual.

Our Organisation 

Sydenham Garden, based in Lewisham in South-East London, is a special, unique and beloved wellbeing centre utilising its gardens, nature reserve and activity rooms to help people living with and recovering from mental and physical ill-health. We were established in 2002 and will celebrate our 25th anniversary in 2027. We provide nature and arts based creative, social and therapeutic activities for co-workers (the name we give our primary beneficiaries). People are referred to us from community organisations and health professionals and we use an integrative approach where those with different degrees of mental and physical ill health come together with volunteers from the local community to reduce social exclusion and prejudice. Each year we work with on average over 250 co-workers and over 70 volunteers. 

About you 

You’ll have experience in building relationships with individual donors, increasing income from community sources and engaging and enthusing volunteers and supporters around a cause. You’ll have excellent communication and people skills and feel comfortable both thinking strategically and managing competing priorities day-to-day. Most importantly, you’ll thrive in a small team working to make a big impact for people living with mental ill-health and dementia.

Living Well Health and Wellbeing Coordinator Role:

Referrals and assessment

• Triage and co-ordinate client referrals from a range of sources, primarily health and care, in conjunction with the Living Well Manager.  These can be for the Living Well service alone, or for “Living Well Plus” where additional support is given to those clients who may require hoarding or de-cluttering support
• Communicate with referral partners to understand the trigger for referral and liaise with clients to build trust and encourage initial assessment
• Work closely with Adult Social Care, regularly hot desking at Merton Civic centre and joining GP surgery Multi-disciplinary meetings to accumulate more referrals and build professional relationships
• Actively engage in taking part in community events and meetings to promote the Living Well service and increase referrals/referral pathways
• Hold ‘guided conversations’ to assess clients’ needs and home risk, these will be conducted on telephone and through home visits

Action planning and support provision

• Create person-centred action plans to enable clients to set realistic goals to improve their overall health and wellbeing.
• Connect individuals to support services catered to their needs at both Age UK Merton and external partners
• Provide ongoing support and co-ordination for clients to ensure that action plans are progressing, including regular liaison with partners across health, care and voluntary sector to achieve successful client outcomes
• Work with partners across health, care and voluntary sectors to build strong understanding of the service offer for older adults in the London Borough of Merton.
• Receive and respond to enquiries related to the programme by phone, in person and by e-mail
• Work closely within Community Support Service, carrying out joint home visits with appropriate staff to ensure better outcomes for clients as well as ensure streamlined/triaged support.

General

• Record information on the CRM database, Charity Log, including service monitoring data and provide accurate and timely monitoring reports as required for funders
• Engage in exporting personal monthly KPIs as required by the Living Well Manager
• Cover the Community Navigator Role when needed with the Community Support service team on a rota basis
• Create two Case Studies quarterly, reflecting on successful outcomes for clients
• Carry out reviews with clients to capture the client’s journey from assessment to closing the case; showcasing how the client has improved their health and wellbeing outcomes
• Understand the process for raising safeguarding concerns and the route of accountability
• Comply at all times with the policies and procedures of Age UK Merton 
• Ensure that Age UK Merton’s Equal Opportunities policies, principles and practices are observed and implemented throughout service delivery
• Attend staff meetings, personal supervision and appraisal meetings
• Be aware of own training needs and participate in training/education to improve performance considered relevant to the post and to achieve agreed targets
• Act as a representative of the values, beliefs and principles of AUKM at all times
• Undertake any other duties that are requested and commensurate with the grade and remit of the post

Purpose of the Role
To lead the effective, compassionate and compliant delivery of Tibet Relief Fund’s Sponsorship Programme, ensuring high-quality supporter care, strong overseas partnerships and robust administrative and financial management.

Key Responsibilities

1. Supporter Care & Communications

• Deliver high-quality, personalised supporter care via email, phone, post and in person

• Manage communication between sponsors and sponsored individuals, ensuring exchanges are appropriate and safeguarding compliant

• Support sponsor recruitment, onboarding and retention

• Organise and oversee the annual exchange of letters and updates

• Produce engaging sponsor communications and case studies based on partner information
   

2. Programme Administration & Data Management

• Maintain accurate sponsorship records and ensure the integrity of the CRM database

• Safeguard personal and financial data in line with data protection requirements

• Maintain a pool of potential sponsorship candidates

• Acquire and manage updates from partner organisations, including reports and photographs

3. Financial Coordination

• Reconcile sponsorship payments in collaboration with the Accounts Manager

• Monitor renewals and follow up with sponsors in a timely manner

• Support the accurate processing of sponsorship-related grants to partner organisations

4. Partnership & Reporting

• Build and maintain effective working relationships with partner organisations in India and Nepal

• Confirm sponsorship and grant details with overseas partners

• Provide regular sponsorship programme updates and reports to the Charity Manager

• Contribute to improving processes and sharing best practice

5. Safeguarding & Compliance

• Promote and safeguard the welfare of sponsored children and adults at risk

• Ensure all communications and processes align with TRF’s Safeguarding Policy

• Uphold confidentiality and professional boundaries at all times

Person Specification

Essential

• Excellent written and verbal English

• Strong communication and interpersonal skills

• Experience delivering high-quality customer or supporter care

Desirable

• Experience working in the charity sector

Student Voice Coordinator (Student Leadership)

£25,809 gross per annum, 35.5 hours per week

The Role

This role is part of the Student Voice & Representation department working collectively with colleagues delivering the Guild’s student advice, representation, democracy, policy and campaigns work. The Student Voice & Representation Team is part of the Community & Representation Directorate.

The Student Voice Coordinator (Student Leadership) role is to support delivery of the Student Voice & Representation departmental plan. We expect Coordinators to help to create an inclusive and engaging environment for all.

The role is required:

To support the Guild to become an expert in the student experience at Birmingham and to develop high impact campaigns that make changes for students on the issues that matter the most.

To support the development and training of student leaders, who are engaged with democracy, campaigns, and other change making activity, at Birmingham and equip them with skills, tools, and knowledge to deliver their change.

To provide elected Officers and student leaders with appropriate support in their roles to represent students at Birmingham.
